Whenever encountering severe cases of throat obstruction (Hou Bi), winding throat wind (Chan Hou Feng), white membrane throat (Bai Hou), sand disease (Sha Zheng), single tonsillar swelling (Dan E), double tonsillar swelling (Shuang E), or wind-fire throat (Feng Huo Hou), where medication fails to alleviate symptoms, and the patient presents with nausea, extreme mental exhaustion, coma, incoherent speech, and unbearable pain, acupuncture may be used to bleed the blue veins on both sides under the root of the tongue (a severe manifestation of Bai Hou). One must absolutely avoid puncturing the central blue vein. The needle depth should be approximately one fen. After releasing the foul blood, rinse the mouth thoroughly with cool water, then puncture the Shaoshang acupoint. Shaoshang is located on the radial side of the thumb nail, one fen away from the nail corner. During the procedure, forcefully pull down along the arm towards the tip of the thumb for forty to fifty repetitions. Once the thumb is reached, insert the needle into the acupoint, first piercing the medial side and then the lateral side, twisting to drain foul fluid. For males, start with the left hand; for females, start with the right hand. Both hands should be treated. If the tonsils are swollen, bleeding via acupuncture is also indicated. However, on the 5th day of the lunar month, when the Ren Shen (Person Spirit) is in the mouth, needling is contraindicated. On the 6th day, when Ren Shen is in the hands, needling Shaoshang is contraindicated. On the 15th day, when Ren Shen pervades the whole body, needling is contraindicated everywhere.
