[Discussion] The so-called "Lao Nue" (Chronic Malaria due to Exhaustion) occurs when malaria persists for a long time without resolution, leading to deficiency of both Qi and Blood. Although the disease has intermittent periods, it is triggered by physical exertion, hence the name. At this stage, pathogenic factors deepen daily, while True Qi (Zhen Qi) is increasingly depleted. With both exterior and interior deficient, symptoms such as reduced appetite, emaciation, sallow complexion, and weakness manifest, while chills and fever persist as before.
For treating Lao Nue with symptoms of dry and brittle hair, irregular chills and fever, reduced diet, muscle wasting, bluish-black complexion, weakness in both legs, cold feet at abnormal times, frequent urination, irregular bowel movements, and nocturnal seminal emission, use Biejia Zhu San Formula:
Bie Jia (Turtle Shell, remove skirt edge, vinegar-fried, Ban Liang) Qiang Huo (Notopterygii Rhizoma et Radix, remove radicle) Du Huo (Angelicae Pubescentis Radix, remove radicle) Chai Hu (Bupleuri Radix, remove sprouts, each Yi Fen) Huang Qi (Astragali Radix, sliced, Ban Liang) Chi Fu Ling (Poria Cocos with Red Hosta Root, remove black skin) Sheng Gan Di Huang (Rehmanniae Radix, dried, baked) Mu Xiang (Aucklandiae Radix, each Yi Fen) Niu Xi (Achyranthis Bidentatae Radix, soaked in wine, sliced, baked) Tian Ling Gai (Human Cranium, fried with butter) Wu Zhu Yu (Evodiae Fructus, each Yi Liang).
The above seventeen ingredients are crushed, sieved into powder. Each dose is San Qian Bi (approx. 4.5g), boiled with one Jian (approx. 200ml) of water with a little salt until reduced to seven-tenths. Take warm with dregs on an empty stomach; or refine honey to make pills the size of paulownia seeds, take forty pills with salted water on an empty stomach.
For treating Lao Nue, use Qinjiao Jiu Formula:
Qin Jiao (Gentianae Macrophyllae Radix, remove sprouts and soil) Chai Hu (Bupleuri Radix, remove sprouts) Gan Cao (Licorice Root, fried) Bie Jia (Turtle Shell, remove skirt edge, vinegar-fried, each Ban Liang) Chang Shan (Dichroae Radix) Cong Bai (Scallion White, each Er Liang) Chi (Soaked Soybean, Yi Fen).
The above seven ingredients are chopped like hemp seeds, soaked overnight in three Sheng (approx. 600ml) of unadulterated wine. Place near fire to keep slightly warm. Take one Zhan (approx. 200ml) per dose, at any time before onset. After taking, add one Zhan of wine to the medicinal residue, continuing until three Sheng are added, then stop.
For treating Lao Nue that persists indefinitely, causing gradual emaciation and critical condition, use Biejia San Formula:
Bie Jia (Turtle Shell, remove skirt edge, vinegar-soaked and fried) Chang Shan (Dichroae Radix, chopped) Shu Qi Ye (Leaves of Dichroa) Hai Piao Xiao (Cuttlefish Bone, remove shell) Fu Zi (Prepared Aconite Lateralis Radix, cracked, peel and navel removed, each Ban Liang) Zhi Mu (Anemarrhenae Rhizoma, sliced, baked) Shu Jiao (Sichuan Pepper, remove eyes and closed ones, stir-fry until oil appears, each Yi Fen) Huang Qi (Astragali Radix, sliced) Chai Hu (Bupleuri Radix, remove sprouts, each San Fen).
The above nine ingredients are crushed, sieved into powder. Each dose is San Qian Bi, soaked overnight in one Zhan of wine. Take warm at dawn, and another dose before onset.
For treating Lao Nue, use Chai Hu Tang Formula:
Chai Hu (Bupleuri Radix, remove sprouts, Yi Liang) Ren Shen (Ginseng Radix et Rhizoma) Gua Lou Gen (Trichosanthis Radix) Huang Qin (Scutellariae Radix, remove black heart) Gan Cao (Licorice Root, fried, sliced) Huang Qi (Astragali Radix, sliced, each Ban Liang).
The above six ingredients are coarsely crushed and sieved. Each dose is Wu Qian Bi (approx. 7.5g), boiled with one and a half Zhan of water, half piece of sliced ginger, and two dates pitted, until reduced to one Zhan. Strain and take warm. One dose on an empty stomach, one dose before onset.
For treating Lao Nue persisting for years with chills, fever, flaccidity, and emaciation, use Qulao Tang Formula:
Chai Hu (Bupleuri Radix, remove sprouts) Chang Shan (Dichroae Radix, chopped) Bie Jia (Turtle Shell, remove skirt edge, vinegar-soaked and fried) Zhi Mu (Anemarrhenae Rhizoma, sliced, baked) Qing Hao (Artemisiae Annuae Herba, dried) Gan Cao (Licorice Root, fried, sliced).
The above nine ingredients are coarsely crushed and sieved. Each dose is Wu Qian Bi, boiled with one and a half Zhan of water, seven willow branch hearts, and three cun of scallion white, until reduced to one Zhan. Strain and take warm on an empty stomach.
For treating Lao Nue with chills, fever, sallow complexion, thirst, dryness, and irritability, use Gancao Tang Formula:
Gan Cao (Licorice Root, fried, San Fen) Shu Qi Ye (Leaves of Dichroa, Ban Liang) Tian Ling Gai (Human Cranium, fried with butter, Yi Liang) Hei Dou (Black Soybean, raw) Tao Ren (Pruni Semen, soaked in hot water, peel and tip removed, ground) Wu Mei Rou (Mume Fruit Flesh, fried, each Yi Fen).
The above six ingredients are coarsely crushed and sieved. Each dose is San Qian Bi, boiled with one Zhan of water and three bamboo leaves until reduced to seven-tenths. Strain. Take one dose before onset on an empty stomach, and another dose just before onset.
For treating Lao Nue persisting long-term with fever, joint pain, inability to eat, red urine, and progressive weakness, use Chaihu Zhike Tang Formula:
Chai Hu (Bupleuri Radix, remove sprouts) Zhi Ke (Aurantii Fructus, remove pulp and bran-fried) Sheng Ma (Cimicifugae Rhizoma) Mai Men Dong (Ophiopogonis Tuber, remove heart, baked) Bie Jia (Turtle Shell, vinegar-soaked and fried until yellow, remove skirt) Gan Cao (Licorice Root, fried, sliced) Tao Ren (Pruni Semen, soaked in hot water, peel and tip removed, double-kernel separately ground, each San Fen).
