Commentary states: The spleen, stomach, large intestine, small intestine, and bladder are the root of granaries and storehouses, the residence of Ying (Nutritive Qi), named as "vessels," capable of transforming dregs and flavors to excrete them. Generally, Ying receives the true essence from food and drink, serving as the root of Ying and Wei (Defensive Qi). The transformation and excretion functions of the large intestine, small intestine, and bladder all rely on Ying as their foundation. Therefore, the small intestine is called the "Official of Reception and Fulfillment." When food and drink enter, they have not yet separated; after digestion and transformation, fluids return to the bladder to become urine, while residues pass to the large intestine to become feces. It must be stated that "Qi transformation enables excretion." If Yin and Yang Qi are imbalanced, with cold and heat competing, or if Qi excess causes obstruction without passage, or Qi deficiency leads to leakage, or dryness causes accumulation, or heat causes constipation, these are all diseases of Yin-Yang disharmony. Treating these conditions should not generally rely solely on cold or hot methods. Cold purgatives may scour the bowels, while hot agents may cause dryness, both potentially damaging harmonious Qi and depleting body fluids. In cases of Guan-Ge (obstruction) where the San Jiao (Triple Burner) is constrained, the pathology is attributed to stagnation of Ying and Wei. For postpartum difficulty in defecation or constipation in the elderly due to insufficient body fluids, only lubricating and moistening methods should be used. Using harsh purgatives would fail to clear the feces and damage True Qi, a fact that must not be overlooked.
Commentary states: Obstruction of urination and defecation constitutes a disease of Yin-Yang Guan-Ge and constraint of the San Jiao. When Yin and Yang are in harmony and the San Jiao ascends and descends properly, food residues are transmitted and excreted at appropriate times. Currently, if Yin or Yang is excessively dominant, Qi becomes stagnant in the middle Jiao, causing Ying and Wei to fail to circulate. Consequently, Qi knots within the abdomen, leading to distension, fullness, and lack of passage, resulting in blockage of both the large and small intestines.
