论曰痔 盖缘寒湿与夫房室醉饱间得之，其作也，肛边肿痒，甚则疼痛不可忍。或蔓生
数处，或似螺蚬，脓血与肌汁，绵绵而下，有孔不合，故谓之痔 。昔人论蛲虫居胴肠间，

治痔疼痛。烟熏方
白鳝鱼骨 熟艾（等分）
上二味锉碎，同和匀，用新盆子一个，盛药在内，以火烧药，候烟出，盆上坐熏之，烟
尽即止。
治痔 出脓血，疼痛不可忍。白矾散方
白矾（飞研半两） 木香（炮一分为末）
上二味研匀，每用少许，以鸡子白调涂痔 上。
治肠风痔 有头，下脓血，疼痛不可忍。止痛膏方
莨菪子（一合） 牛皮胶（一两）
上二味，先研莨菪子为末，次用胶清调成膏，摊纸上贴之，有头者自消。
治肠风痔 ，肛边鼠乳，疼痛不可忍。威灵仙丸方

煎荆芥汤下，服药后，忌茶半日，恐冷即腹痛，男子妇人皆可服。
治大肠痔 ，脱肛疼痛。方
虎脚指两节
上一味，以蜜二两炙令赤，捣罗为末，炊饼丸如梧桐子大。每服空心温酒下二十丸。
论述说：痔疮，大概是因为寒湿之邪，以及在房事、醉酒或饱食之后感受外邪而引发的。发作时，肛门周围肿胀瘙痒，严重时疼痛难以忍受。有的痔疮蔓延生长于多处，有的形状像螺蛳或蛤蜊，脓血与组织液连绵不断流出，形成久不愈合的孔洞，所以称之为“痔”。古人论述蛲虫居住在直肠之间，数量多时就会形成痔疮。医方中有使用熏蒸疗法，以及使用杀虫药物的记载。对此不可不仔细考察。
治疗肠风痔疮疾病，多年不愈，疼痛难以忍受的万金丸方：
安息香、乳香、丁香、木香、沉香（锉碎）、无食子、肉豆蔻（去壳）、当归（切后焙干）、麒麟竭（即血竭）。
以上十四味药，先将乳香、安息香、阿魏、麒麟竭、没药等细锉，加少量水浸泡一夜，次日细细研磨成膏状，其余药物捣碎过筛成粉末，与药膏一同研磨，制成如黄米大小的丸剂。每次服用一丸，空腹时用冷茶送服。
治疗痔疮疼痛难以忍受，以及便血的比金丸方：
藤子（原文可能有误，疑为牵牛子或其他，此处依原文直译）、附子（炮制裂开，去皮和脐）、硫黄（研细）、白矾（烧至汁液流尽）、螺皮（炙焦）、枳壳（去瓤麸）。

治疗五种痔疮疼痛难以忍受的荆兰汤淋渫方：
荆芥穗、贯众、甘草、蜀椒（去目）、泽兰、芍药（各二两）。
以上六味药，粗捣筛过，每次取三大匙，用水三碗煎沸，倒入盆中，先坐于盆上熏蒸，严密覆盖，勿使药气泄漏，温度适宜手触时即进行淋洗。
治疗五种痔疮疼痛剧烈的试虫散方：
臭椿根、地骨皮、景天（阴干各二两，护火草即是此物）、马牙硝（一两）。
以上四味药，捣成细散，用精猪肉一大片，将三钱匕药粉撒在肉上，坐在上面约一二小时，起来查看若有虫则去除，若无虫则停止。
治疗各种痔疮有头疽，疼痛难以忍受的熏洗方：
地菘（一斤）、槐皮（二两）、葱根（一握）、韭根（一握）。
以上四味药细锉，用水五升，煎至三升，盛于盆内，先坐于盆上趁热熏蒸，温度适宜手触时即淋洗，每日使用三次。
治疗五种痔疮疼痛，以及阴部潮湿瘙痒的荆芥汤洗方：
荆芥穗、臭橘、浓朴（去粗皮各半斤）。
以上三味药，粗捣筛过，每次取二两，用水五升，煎取三升，在避风处进行淋洗。
治疗各种痔疮有头疽，疼痛难以忍受的枳壳散熨方：
枳壳（四两）、诃黎勒皮（二两）。
以上两味药，粗捣筛过，同炒令热，用棉布包裹热敷患处，变冷后再炒热继续使用。
治疗痔疮疼痛的烟熏方：
白鳝鱼骨、熟艾（等分）。
以上两味药锉碎，混合均匀，用一个新盆子，盛装药物在内，用火焚烧药物，待烟雾冒出，坐于盆上熏蒸，烟尽即止。
治疗痔疮流出脓血，疼痛难以忍受的白矾散方：
白矾（飞研半两）、木香（炮制一分，研为末）。
以上两味药研匀，每次取少许，用鸡蛋清调和涂抹于痔疮患处。
治疗肠风痔疮有头疽，下脓血，疼痛难以忍受的止痛膏方：
莨菪子（一合）、牛皮胶（一两）。
以上两味药，先研莨菪子为末，再用胶清调和成膏，摊在纸上贴敷，有头疽者自会消散。
治疗肠风痔疮，肛旁鼠乳（痔疮的一种），疼痛难以忍受的威灵仙丸方：
威灵仙（净洗焙干二两）、木香（一两）。
以上两味药，捣罗为细末，炼蜜和丸，如梧桐子大小。每次服用二十丸，加至三十丸，不限时间。用煎好的荆芥汤送服，服药后半日内忌饮茶，恐怕受凉会导致腹痛，男女皆可服用。
治疗大肠痔疮，脱肛疼痛的方剂：
虎脚指两节。
以上一味药，用蜂蜜二两炙烤至赤色，捣碎过筛成粉末，用炊饼（馒头）和丸，如梧桐子大小。每次空腹用温酒送服二十丸。
Commentary states: Hemorrhoids are generally caused by cold-dampness, or contracting pathogens after sexual intercourse, drunkenness, or overeating. Upon onset, the perianal region swells and itches; in severe cases, pain is unbearable. Some hemorrhoids spread to multiple areas, while others resemble snails or clams, with pus, blood, and tissue fluid flowing continuously, forming non-healing fistulas, hence termed "hemorrhoids" (Zhi). Ancient texts discuss that ascarids residing in the rectum, when numerous, can form hemorrhoids. Medical formulas include fumigation therapies and vermifuge drugs. These must be carefully examined.
For treating intestinal wind hemorrhoid diseases that have persisted for years without healing, causing unbearable pain: Wanjin Pill Formula
Benzoin Resin (Anxi Xiang), Frankincense (Ruxiang), Clove (Dingxiang), Costus Root (Muxiang), Agarwood (Chenxiang) [crushed], Myrobalan (Wushizi), Nutmeg (Roudoukou) [shelled], Angelica Sinensis Radix (Danggui) [cut and baked], Dragon's Blood (Qilinjie).
The above fourteen ingredients: First, finely chop Frankincense, Benzoin Resin, Asafoetida (Awei), Dragon's Blood, and Myrrh (Moyao). Soak them in a small amount of water overnight. The next day, grind them finely into a paste. Pound and sieve the remaining herbs into powder. Mix with the paste and roll into pills the size of yellow millet. Take one pill each time, swallowing with cold tea on an empty stomach.
For treating hemorrhoid pain that is unbearable and bleeding: Bijin Pill Formula
Tengzi (likely a typo for another herb, e.g., Qian Niu Zi), Prepared Aconite Root (Fuzi) [split by heat, skin and umbilicus removed], Sulfur (Liu Huang) [ground], Alum (Bai Fan) [burned until liquid evaporates], Concha Haliotidis (Luopi) [roasted until charred], Aurantium Fructus (Zhiqiao) [pulp and bran removed].
The above seven ingredients: Pound and sieve into powder. Use wine-boiled wheat paste to form pills the size of sophora seeds. Take ten pills each time, with rice drink or wine, at any time.
For treating hemorrhoid pain, piercing like awls or knives, which is unbearable: Nengxiao Pill Formula
Siberian Clematis Root (Weilingxian) [remove seedlings and soil, 4 liang, also known as Nengxiao], Serissa Japonica Herb (Juanbai) [remove roots], Saposhnikovia Root (Fangfeng) [remove forked tips], [Missing Character] Skin [burned to ash retaining nature], Donkey-hide Gelatin (Ejiao) [roasted].
The above six ingredients: Pound and sieve into powder. Refine honey to form pills the size of sophora seeds. Take ten pills each time, increasing to twenty pills, with Ginseng Decoction. Three times daily, at any time.
For treating hemorrhoid pain making walking impossible: Gualou Powder Formula
Large Trichosanthes Fruit (Dagualou) [one fruit, opened], Swallowwort Fruit (Zhuyazaojia), Alum (Baifan) [each half liang], Carp Skin (Liyupi), Turtle Plastron (Biejia) [remove skirt edge], Thorns [Missing Character] (each one [measure]).
The above six ingredients: Except for Gualou, coarsely crush the rest and place inside the Gualou. Seal firmly with salt mud and let dry. Roast with five jin of charcoal until red hot. Let cool, remove, pound and sieve into powder. Take two qianbi each time, mixed with walnut wine.
For treating hemorrhoid pain that is unbearable and intestinal wind bleeding: Qtong Powder Formula
Swallowwort Seeds (Zaojiazi) [one incomplete seed, stir-fried with two sheng of bran until bran is black, then remove bran], Mint (Bohe) [dried, 3 liang, crushed].
The above two ingredients: Pound and sieve into powder. Take two qianbi each time, mixed with rice drink, taken on an empty stomach.
For treating the five types of hemorrhoids with unbearable pain: Linxie Decoction Formula
Scutellaria Root (Huangqin) [remove black heart], Akebia Stem (Mutong) [crushed], Gardenia Seed (Zhizi Ren), Licorice Root (Gancao) [roasted and crushed], Ta Yin Xuanjing Stone, Polygonum Aviculare Herb (Jixue Cao) [each 2 liang], Mirabilite (Piaoxiao) [measure missing].
The above seven ingredients: Coarsely crush and sieve. Each time use seven qianbi, with three bowls of water. Boil and pour into a basin. Sit over the basin to fumigate first. When warm enough to touch, perform washing/rinsing. Stop when cold. During fumigation, cover tightly to prevent leakage of medicinal vapor.
For treating the five types of hemorrhoids with unbearable pain: Jinglan Decoction Fumigation Formula
Schizonepeta Spike (Jingjisui), Cyathula Root (Guanzhong), Licorice Root (Gancao), Sichuan Pepper (Shujiao) [remove eyes], Lysimachiae Herba (Zelan), Peony Root (Shaoyao) [each 2 liang].
The above six ingredients: Coarsely crush and sieve. Each time use three large spoons, with three bowls of water. Boil and pour into a basin. Sit over the basin to fumigate first. Cover tightly to prevent leakage of medicinal vapor. When warm enough to touch, perform washing/rinsing.
For treating severe pain from the five types of hemorrhoids: Shichong Powder Formula
Toona Root (Chouchun Gen), Lycium Bark (Digupi), Sedum Acre Herb (Jingtian) [dried in shade, each 2 liang, Huohuo Cao is this], Sodium Nitrate (Mayaxiao) [1 liang].
The above four ingredients: Pound into fine powder. Take a large slice of lean pork, sprinkle three qianbi of medicine on the meat. Sit on it for one to two hours. Get up and check; if worms appear, remove them; if not, stop.
For treating various hemorrhoids with heads (abscesses) and unbearable pain: Fumigation and Washing Formula
Veronica Officinalis Herb (Disong) [1 jin], Sophora Bark (Huai Pi) [2 liang], Scallion Root (Conggen) [one handful], Leek Root (Jiu Gen) [one handful].
The above four ingredients: Finely crush. Add five sheng of water, boil down to three sheng. Pour into a basin. Sit over the basin to fumigate while hot. When warm enough to touch, wash. Use three times daily.
For treating the five types of hemorrhoids with pain, and damp-itchiness in the yin area: Jingjie Decoction Washing Formula
Schizonepeta Spike (Jingjisui), Toona Citrus (Chouju), Magnolia Bark (N Hou) [remove rough bark, each half jin].
The above three ingredients: Coarsely crush and sieve. Each time use two liang, with five sheng of water. Boil to obtain three sheng. Perform rinsing in a wind-sheltered place.
For treating various hemorrhoids with heads and unbearable pain: Zhiqiao Powder Hot Compress Formula
Aurantium Fructus (Zhiqiao) [4 liang], Chebula Fruit Peel (Helile Pi) [2 liang].
The above two ingredients: Coarsely crush and sieve. Stir-fry together until hot. Wrap in cotton cloth and apply as a hot compress. Reheat when cold.
For treating hemorrhoid pain: Smoke Fumigation Formula
White Eel Bone (Bai Shan Yu Gu), Prepared Mugwort Leaf (Shuai Ai) [equal parts].
The above two ingredients: Crush and mix evenly. Place in a new basin. Burn the medicine. When smoke emerges, sit over the basin to fumigate. Stop when smoke is exhausted.
For treating hemorrhoids discharging pus and blood with unbearable pain: Baifan Powder Formula
Alum (Baifan) [refined and ground, half liang], Costus Root (Muxiang) [roasted, one fen, ground to powder].
The above two ingredients: Grind uniformly. Take a small amount each time, mix with egg white, and apply to the hemorrhoid site.
For treating intestinal wind hemorrhoids with heads, discharging pus and blood, with unbearable pain: Zhitong Ointment Formula
Datura Seed (Langdazi) [one he], Cowhide Gelatin (Niupijiao) [1 liang].
The above two ingredients: First grind Datura Seed into powder. Then mix with gelatin solution to form ointment. Spread on paper and apply. Those with heads will spontaneously resolve.
For treating intestinal wind hemorrhoids, perianal rat-like protrusions (hemorrhoids), with unbearable pain: Weilingxian Pill Formula
Siberian Clematis Root (Weilingxian) [cleanly washed, baked dry, 2 liang], Costus Root (Muxiang) [1 liang].
The above two ingredients: Pound and sieve into fine powder. Mix with refined honey to form pills the size of sophora seeds. Take twenty pills each time, increasing to thirty pills, at any time. Take with Jingjie Decoction. Avoid tea for half a day after taking the medicine to prevent abdominal pain due to cold. Suitable for both men and women.
For treating large intestine hemorrhoids, prolapsed anus with pain: Formula
Tiger Foot Joints (Two segments).
The above single ingredient: Roast with two liang of honey until red. Pound and sieve into powder. Mix with steamed bun dough to form pills the size of sophora seeds. Take twenty pills each time on an empty stomach with warm wine.
