The Qing Gao Formula from Qianjin Fang for treating headache, neck stiffness, and vexing pain in the four limbs caused by Shanghan (Cold Damage).
Danggui (Angelicae Sinensis Radix), Xiong (Ligustici Chuanxiong Rhizoma), Wuzhuyu (Evodiae Fructus), Fuzi (Aconiti Lateralis Radix Praeparata), Wutou (Aconiti Radix), Mangcao (Artemisiae Argyi Herba), Shujiao (Zanthoxyli Pericarpium) (each 3 liang), Baizhi (Angelicae Dahuricae Radix) (3 liang).
The above eight herbs are cut into pieces, soaked in strong vinegar for two nights, then decocted with 4 jin of lard over slow fire. When the color of Baizhi turns yellow, filter out the dregs. Take three pills the size of a jujube pit with warm wine, three times a day, aiming for sweating. If no sweat occurs, slightly increase the dose; it can be taken orally or applied externally with massage. For those who have just contracted Shanghan on the first day and suffer from severe headache and back stiffness, external massage is preferable. Avoid pork.
Furthermore, in Shaoyin disease, if mouth dryness and throat dryness occur two to three days after onset, urgent purgation is required; Chengqi Tang is suitable.
Furthermore, in Shaoyin disease, if abdominal fullness and constipation persist for six to seven days, urgent purgation is required; Chengqi Tang is suitable.
Furthermore, in Yangming syndrome, if the patient suffers from forgetfulness, there must be blood stasis accumulation. This is because pre-existing long-standing blood stasis causes forgetfulness. Although the stool is hard, it is surprisingly easy to pass, and the color will definitely be black; Didang Tang is suitable for purgation.
Furthermore, in Shanghan with fever and lower abdominal fullness, urination should be difficult; however, if urination is normal, this indicates blood stasis. Other medicines are not suitable; Didang Wan is appropriate.
Furthermore, in Taiyang disease, if the body is yellow, the pulse is deep and knotted, the lower abdomen is firm, and urination is difficult, this indicates no blood stasis. However, if urination is spontaneous and the patient exhibits mania-like behavior, this is definitive evidence of a blood syndrome; Didang Tang is suitable for purgation.
Furthermore, in Yangming disease, if the pulse is slow, sweating occurs without aversion to cold, the body feels heavy, there is shortness of breath, abdominal fullness, and wheezing, accompanied by tidal fever, the exterior syndrome is likely resolving, and interior purgation can be considered. If hands and feet sweat continuously, this indicates hardened stool; Chengqi Tang is the main treatment. If sweating is profuse with slight fever and aversion to cold, the exterior is not yet resolved; Guizhi Tang is suitable. If there is no tidal fever, Chengqi Tang should not be given. If there is significant abdominal fullness and constipation, a small amount of Chengqi Tang may be given to gently harmonize stomach qi, avoiding drastic purgation.
Furthermore, in Yangming disease with tidal fever and slightly hard stool, Chengqi Tang may be given; if the stool is not hard, do not give it. If there has been no bowel movement for six to seven days, suspecting dry feces in the stomach/intestines, one may administer Xiao Chengqi Tang to test. If flatus passes in the abdomen, dry feces are present, and purgation is permissible. If no flatus passes, this indicates only a hard head with loose stool at the end; purgation is contraindicated as it will cause abdominal fullness, inability to eat, and hiccups upon drinking water. If fever develops later, the stool will become hard again; use Xiao Chengqi Tang to harmonize. If no flatus passes, absolutely do not purge.
Excess conditions lead to delirious speech; deficiency conditions lead to repetitive murmuring (Zhengsheng refers to repetitive speech). If a patient has fixed gaze, delirium, and wheezing/fullness, death ensues; similarly, if diarrhea occurs, death ensues.
Furthermore, in Shanghan on the fourth or fifth day, if the pulse is deep with wheezing and fullness, a deep pulse indicates an interior condition. If sweating-inducing methods are incorrectly used, fluids escape, making defecation difficult. This results in exterior deficiency and interior excess, leading to delirium over time.
