Regarding the etiology, the throat is the pathway through which Qi ascends and descends. The epiglottis is the gate of voice production. The tongue is the mechanism of sound generation. The lips are the fan for sound dissemination. When wind-cold pathogenic factors invade and lodge between the epiglottis, sudden loss of voice occurs. This is all due to injury by wind pathogens, hence it is termed "wind-induced aphonia" (Feng Shi Yin Bu Yu). The Yangsheng Fang states: Sleeping in the wind while drunk can cause aphonia. (From Guangji)
Formula for treating wind-induced aphonia and inability to speak:
Qianghuo (Notopterygii Rhizoma et Radix, 10 fen), Gancao (Licorice Root, processed with honey), Renshen (Ginseng Radix, 2 fen), Jingli (Amaranthus Juice), Zhuli (Bamboo Sap), Shengdihuang (Rehmanniae Radix Fresh, juice, 2 sheng each), Dafuzi (Aconiti Lateralis Radix Preparata, one piece, processed, 8 fen).
The above seven herbs are chopped. All herbs are placed into the three juices. Decoct to obtain 1 sheng 6 he of liquid. Remove the dregs. Divide into two warm doses. If not cured, administer another dose after four or five days. Aim for a mild purgation. Avoid hot noodles, Sargassum (Haizao), Brassica chinensis (Songcai), pork, cold water, Myristica (Wuyi), fish, garlic, and sticky foods. (From Volume I)
Shen Shi Fang's Fangfeng Yang formula treats stroke causing inability to open both eyes, inability to speak, shortness of breath, and a sensation of impending death:
Fangfeng (Saposhnikoviae Radix), Gancao (processed), Huangqin (Scutellariae Radix), Fuling (Poria), Danggui (Angelicae Sinensis Radix, 1 liang each), Xingren (Prunus Armeniacae Semen, 50 pieces, remove tips and skin), Qinjiao (Gentianae Macrophyllae Radix, 0.5 liang), Shengjiang (Zingiberis Rhizoma Recens, 5 liang), Ganzao (Jujubae Fructus, 30 pieces, split), Mahuang (Ephedrae Herba, 2 liang, remove joints).
The above ten herbs are crushed. Use clear wine and water totaling 4 sheng. Boil to obtain 3 sheng. Divide into three doses. Induce sweating. Avoid Sargassum, Brassica chinensis, and strong vinegar.
Additionally, Sini Tang (Four Reversal Decoction) treats sudden stroke with inability to speak, cold extremities without pulse, and spasm of hands and feet:
Shanzhuyu (Corni Fructus), Xixin (Asari Radix et Rhizoma), Ganjiang (Zingiberis Rhizoma, processed, 1 liang each), Gancao (processed, 3 liang), Maimendong (Ophiopogonis Radix, 1 sheng, remove heart).
The above five herbs are chopped. Use 7 sheng of water. Decoct to obtain 2 sheng. Divide into four doses. Avoid Sargassum, Brassica chinensis, raw green onions, and vegetables. (From Volume IX)
Zhou Hou Beiji Fang treats sudden aphonia:
Boil Mustard Seed (Jiezi) with Kujiu (bitter wine). Apply around the neck. Wrap with clothing. Remove after one day and one night. The disease will be cured. (Fan Wang and Qianjin Fang agree).
Another formula:
Boil Soybean (Dadou) and decoct its juice until thick. Hold in the mouth. Alternatively, boil strongly and drink. (Both from Volume I, consistent with Fan Wang).
Qianjin Fang discusses aphonia in Jue syndrome: Wind-cold qi lodges in the middle, stagnates, and cannot emit sound, resulting in aphonia and muteness. Throat pain and loss of voice are also caused by wind pathogens. Invasion of the Zang organs can be fatal. In cases of corpse-like Jue (Shijue), resembling death, the pulse beats normally; Yang pulses sink downward, and Yin pulses contend upward. This is due to Qi blockage. Treatment formula:
Take soot from the kitchen chimney (Zaotu Mo), size of a bean pill, mix with Jiangshui (fermented grain juice), and drink.
Another formula:
Decoct Cinnamon Twig (Guizhi) juice strongly and take 1 sheng. Cover to induce sweating. Alternatively, powder the herb, place under the tongue, and gradually swallow the juice. Avoid raw green onions. (Consistent with Fan Wang).
Another formula:
Decoct Fermented Soybean (Chiji) juice strongly and hold in the mouth; this is also good. (All from Volume VIII).
