Whenever a severe toxic ulcer has ruptured but the internal toxin is not yet fully eliminated, sudden application of tissue-regenerating (Shengji) agents will result in apparent external healing while internal decay continues. Severe cases may force the toxin to attack inwardly, while mild cases may exacerbate the ulceration. Even if the wound closes temporarily, large carbuncles may reappear in adjacent areas. Thus, it is understood that when toxins are not fully cleared, tissue-regenerating drugs should not be used abruptly. Instead, apply Beiyegao (Shell Leaf Ointment) frequently changing the dressing. Only when granulation tissue resembling pearls appears should tissue-regenerating medicines be used. For patients with weak Yuanqi (Vital Energy), substantial tonification is required to nourish the root.
Shengji Dingtong San (Tissue-Regulating and Pain-Relieving Powder): Indicated for ulcers presenting with redness, swelling, heat, pain, and necrotic tissue. This formula resolves necrosis, relieves pain, and promotes tissue regeneration.
Calcii Carbonas Preparatus (Shigao, Gypsum Fibrosum) (ground into powder, levigated five to seven times with Licorice Decoction, per tael added) Borax (Pingsha, Borneolum Syntheticum) Cinnabar (Chensha, Cinabris) Borneol (Bingpian, Borneolum)
Grind into fine powder and sprinkle on the affected area.
Dingtong Shengji San (Pain-Relieving and Tissue-Regulating Powder): Indicated for ulcers presenting with redness, swelling, heat, and pain without necrotic tissue. This formula relieves pain and promotes tissue regeneration.
Gypsum Fibrosum (Shigao, Calcii Sulfas) (1 liang) Frankincense (Ruxiang, Olibanum) Dragon's Blood (Xuexie, Sanguis Draconis) Calomelas (Qingfen, Hydrargyri Chloridum Mite) Borneol (Bingpian, Borneolum) (1 qian)
If there is exudate, add Angelica Dahurica (Baizhi, Radix Angelicae Dahuricae) and Fossilized Bone (Longgu, Os Mastodi et Ossis Femoris) (1 qian each). If the wound fails to close, add Chicken Gizzard Membrane (Jineijin, Endothelium Corneum Gigeriae Galli) (roasted, 1 qian).
Grind into fine powder and sprinkle on the affected area.
For cold-type ulcers failing to close: Sprinkle dried Ginger (Ganjiang, Zingiberis Rhizoma) powder on the wound. A sensation of warmth like baking will occur, smoothing the surface and facilitating healing.
For various ulcers causing itching: Sprinkle Alum (Kufan, Alumen) and dried Ginger (Ganjiang, Zingiberis Rhizoma) powder on the affected area. Then wash with a decoction of fine tea (Xicha, Camelliae Sinensis Folium) and salt (Yan, Natrii Chloridum).
Frankincense (Ruxiang, Olibanum) Myrrh (Moyao, Myrrha) Dragon's Blood (Xuexie, Sanguis Draconis) Catechu (Ercha, Catechu) Notoginseng Root (Sanqi, Panacis Quinquefolii Radix) (2 qian each) Borneol (Bingpian, Borneolum) (1 qian) Musk (Shexiang, Moschus) (2 fen)
If heat signs are present, add Coptis Rhizome (Huanglian, Coptidis Rhizoma) (1 qian). If necrotic tissue is present, add Calomelas (Qingfen, Hydrargyri Chloridum Mite) (1 qian). If there is exudate, add Fossilized Bone (Longgu, Os Mastodi et Ossis Femoris) (1 qian). To accelerate closure, add Pearl (Zhenzhu, Concha Margaritifera) (1 liang) or Crab Yellow (Xiehuan, 2 qian; method: steam mature female crab, extract yolk, dry and store).
Grind into fine powder and apply.
Alternatively, take the aforementioned seven herbs, add half a tael of Pig Fat (Zhuyou, Adip Suillum) and an appropriate amount of Beeswax (Huangla, Cera Flava). Warm slightly, spread onto cotton paper to make plasters, and apply to carbuncles, ulcers, and broken sores. For whip injuries, double the dosage of Notoginseng Root (Sanqi, Panacis Quinquefolii Radix).
Another method: Wrap fresh Deer Leg Bone in paper and roast in ash fire until yellow and brittle. If it turns black and charred, it is useless. Grind into fine powder and sprinkle; it regenerates tissue very rapidly.
Yuebai Zhenzhu San (Moon-White Pearl Powder): Indicated when new flesh has filled the wound but skin cannot regenerate, as well as for chancre with rotting and pain, and scalds/burns.
Pearl (Zhenzhu, Concha Margaritifera) (1 qian) Indigo Naturalis (Qinganghua, Folium Isatidis) (5 fen) Calomelas (Qingfen, Hydrargyri Chloridum Mite) (2 liang)
Grind into fine powder and apply. For chancre with rotting, mix with Porcine Spinal Cord (Zhujisuangui, Medulla Spinalis Sus Scrofa Domesticus) and apply.
Another method: Pour Egg White (Jiziqing, Albumen Ovi Galli) onto a clean tile, dry in the sun, collect the clear upper layer, grind into powder, and apply.
