Jú (Carbuncle) arises at the posterior aspect of the shoulder and the lateral side of the axilla, belonging to the Jianzhen (SI9) acupoint of the Small Intestine Meridian, formed by the condensation of Wind-Fire pathogens.
Jianfengdu (Shoulder Wind Toxicity) arises at the upper part of the shoulder, belonging to the Jianyu (LI15) acupoint of the Large Intestine Meridian, formed by Wind-Dampness pathogens.
[Note]
Jú (Carbuncle) arises in the posterior-inferior part of the shoulder, slightly superior to the postero-lateral part of the axilla, specifically between the gaps of the clavicle and scapula. It belongs to the Jianzhen (SI9) acupoint of the Small Intestine Meridian, caused by the condensation of Wind-Fire pathogens. Initially, it resembles a millet seed, being hard, swollen, painful, and tender. The shoulder is stiff and tight, unable to lift or raise. In the early stage, Jingfang Baidu San (Atractylodes and Saposhnikovia Decoction) should be taken orally; if accompanied by dry stool and excess syndrome, Shuangjie Guijin Wan (Double-Resolving Golden Pill) should be taken for simultaneous release of exterior and interior pathogens. Jianfengdu (Shoulder Wind Toxicity) arises at the bony prominence at the distal end of the shoulder, belonging to the Jianyu (LI15) acupoint of the Large Intestine Meridian. It is caused by pathogenic Wind deeply invading the bone gaps, lingering with Dampness, and transforming into Heat. Initially, there is swelling and redness; large lesions are like peaches, small ones like apricots. Pain radiates to the shoulder, accompanied by stiffness and tightness. In the early stage, Juantong Wuyou San (Pain-Relieving Worry-Free Powder) should be taken to induce sweating, which will dissipate the condition. If swelling and pain deepen daily and cannot be completely resolved, it indicates that pus is about to form; Tuoli Tou Nong Tang (Interior-Supporting Pus-Penetrating Decoction) is appropriate. After ulceration of either condition, internal and external treatment methods follow the principles outlined in the "Ulcerated Carbuncles and Sores" section.
Juantong Wuyou San (Pain-Relieving Worry-Free Powder)
Strychnos nux-vomica (Fried in sesame oil until floating) Angelicae Sinensis Radix (Washed with wine) Glycyrrhizae Radix et Rhizoma (Raw, 2 liang each) Ephedrae Herba (3 liang) Manis Squama (Stir-fried with aged earth) Aconiti Lateralis Radix Praeparata (Boiled in black soybean wine, skin and tip removed) Aconiti Kusnezoffii Radix (Boiled in ginger juice) Atractylodis Rhizoma (Soaked in rice wash water and stir-fried) Pinelliae Tuber (Processed, 2 liang each) Salviae Miltiorrhizae Radix et Rhizoma (1 liang)
Process each herb separately, grind into fine powder, mix uniformly. Take 5 fen to 1 qian per dose, mixed with clear wine. Drink more alcohol until intoxicated, then cover with blankets, lie down to sweat, and avoid wind. Adding 4 liang of Yangtse River Buttercup Herb (Akebiae Caulis? Note: Text says Naoyanghua, likely Datura metel flower) to this formula also treats Head Wind Pain.
