Dou Ding (Poisonous Carbuncle resembling Smallpox) differs from ordinary smallpox lesions; it is colloquially known as "Thief Pox" (Zeidou). Its characteristics include a purple, dark, or blackish color and a hardness like stone. Various symptoms arise simultaneously, making suppuration difficult. There are many types of Ding carbuncles that must be memorized, and one must carefully examine the site of onset to identify their forms.
Juanlian Ding (Curtain-Rolling Carbuncle) arises at the base of the tongue; its size varies and it is easily identified. Huozhu Ding (Fire Pearl Carbuncle) occurs inside the nostrils, causing nasal obstruction, a sensation of spitting fire, and facial redness. Wangji Ding (Forget-Sucking Carbuncle) appears on the eyelids, swelling like a frog with aggravated irritability and heat. Huanhu Ding (Taming-Tiger Carbuncle) is seen in the ear, formed by Kidney toxin attacking the ear. Yanwo Ding (Swallow's Nest Carbuncle) develops in the axillae, accompanied by facial redness, delirium, and swelling pain. Zhuming Ding (Life-Injecting Carbuncle) emerges on the sole of the foot, with purple veins penetrating directly into the thigh. Touchang Ding (Intestine-Penetrating Carbuncle) originates internally in the anus, causing pain akin to needle pricking. Qilong Ding (Black Dragon Carbuncle) forms at the urethral meatus, accompanied by fever, delirium, and urinary/fecal retention. Treatment should be administered according to the specific name of the Ding carbuncle, which will naturally resolve all symptoms.
[Note] This condition has numerous names and varying treatment methods. Dou Ding typically appears between the fifth and sixth days of eruption, occurring in clusters of three to six or five to seven pieces, mixed among other pustules. The color is purple and dark; severely, it becomes black and hard like stone. This leads to a surge of various symptoms and an inability to suppurate.
For example, Juanlian Ding arises at the base of the tongue, small as a black bean or large as a grape, causing the child's tongue to curl and throat pain. Urgently use a silver hook to pierce it, drain all foul blood, rinse the mouth with bitter tea, apply Bading San (Carbuncle-Drawing Powder), and then blow a fine powder made from Bingpian (Borneol), Pengsha (Borax), Qingdai (Indigo Naturalis), Huanglian (Coptis Rhizome), Bohe (Mint), Jingjie (Schizonepeta), and Chao Jiangcan (Stir-fried Silkworm) into the lesion.
Huozhu Ding arises inside the nostrils, causing obstruction and a fiery sensation with red face and eyes. It is also pierced with a silver hook, treated with Huanglian Gao (Coptis Ointment) plus Borneol dropped into the nose, and internally administered Xiejiao San (Metal-Draining Powder).
Wangji Ding arises on the eyelids, swelling like a frog with irritability, heat, and purplish face. It should be lanced and treated with juice from chewed Yanzhi (Vermilion/Cinnabar), and washed with a decoction of Gongying (Dandelion) and Juhua (Chrysanthemum).
Huanhu Ding arises in the ear; when pus matures, it should be lanced and smeared with Bading San.
Yanwo Ding arises in the axilla, presenting with swelling, hardness, facial redness, and delirium. If the Ding lurks in the left axilla, the smallpox lesions on the right side of the body will sink and lose color, and vice versa. It should be lanced to remove the root, smeared with Bading San, and Decoction for Disinfecting Poison (Xiaodu Yinzi) taken internally.
Zhuming Ding arises on both soles, swelling and hardening like coins, beans, or peppercorns, with purple veins extending to the thighs. After lancing and draining blood completely, apply Tianluo Shui (Snail Water). Subsequently, soak Shenhuo Cao (Aconite Root, processed) and Lvdou (Mung Bean) until swollen, crush them, and apply as a poultice.
Touchang Ding arises beside the anus, becoming hard and cone-like around the fifth or sixth day. After lancing, wash with a decoction of Jinyinhua (Honeysuckle) and Fangfeng (Saposhnikovia). Then apply a powder of Qingfen (Calomel), Zhenzhu (Pearl), Borneol, and Bailian (White Peony Root). Internally administer Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity).
Qilong Ding arises at the urethral meatus. Around the fifth or sixth day, symptoms include fever, delirium, upward turning of the eyes, cold limbs, abdominal distension, and urinary retention. Urgently prepare a powder from Chansu (Toad Venom), Niuhuang (Calculus Bovis), Borneol, and Shexiang (Musk). Then, decoct Huanglian (Coptis) with strong tea, let it cool, take half a spoonful to mix with the powder, dip a soft rice straw core into it, and insert it into the meatus. Taking Xiaodu Yinzi is very effective.
Xiejiao San (Metal-Draining Powder)
Xijiao (Rhinoceros Horn, sliced) [Note: Modern substitutes like Shuiniu Jiajiao are used], Niubangzi (Arctii Fructus, stir-fried and crushed), Honghua (Carthami Flos), Shengdi (Rehmanniae Radix), Jiegeng (Platycodonis Radix), Chishao (Paeoniae Radix Rubra), Zisu (Perillae Folium), Gancao (Glycyrrhizae Radix et Rhizoma, raw, each 1 qian).
Decoct in water and take orally.
[Formula Song] Xiejiao San treats Fire-Toxin Ding, with red face, red eyes, and nasal pain; Xi, Bang, Honghua, Shengdi, Jiegeng, Chishao, Zisu, and raw Gancao compose the strain.
Xiaodu Yinzi (Decoction for Disinfecting Poison)
Baifuling (Poria Cocos), Shengdi, Lianqiao (Forsythiae Fructus, heart removed), Niubangzi (stir-fried and crushed), Honghua, Gancao (raw), Xijiao (sliced), Mutong (Akebiae Caulis), Chishao (each 1 qian).
Add twenty roots of Dengxin (Junci Medulla), decoct in water, and take orally.
[Formula Song] Xiaodu Yinzi contains Ling, Shengdi, Qiao, Bang, Honghua, Gancao, Xi, Mutong, Shaoyao, and Dengxin together; it effectively expels Ding-Toxin and Fire syndromes.
