Yandan (Eyelid Carbuncle) manifests as redness, heat, swelling, and pain in the upper and lower eyelids. If the swelling is soft and drooping, it indicates a predominance of Wind pathogen; if the color is red-purple and the texture is hard, it indicates a predominance of Heat pathogen. In such cases, administering Jingfang Baidu San (Aster and Saposhnikovia Decoction for Overcoming Toxins) can be effective.
[Note] This condition arises from Damp-Heat in the Spleen and Stomach combined with Wind invasion, presenting with redness, swelling, and pain. If the swelling is soft and drooping, impairing vision, it signifies a predominance of Wind, where edema is easy to resolve; if the color is red-purple and the texture is hard, it signifies a predominance of Heat, where swelling and induration are difficult to dissipate. At the initial stage, Jingfang Baidu San should be used to disperse Wind. If accompanied by thirst and dry stool, Neishu Huanglian Tang (Internal Draining Coptis Decoction) should be used to purge Heat; if the condition persists for a long time without resolving, Tou Nong San (Decoction for Promoting Pus Formation) should be taken, and once the pus matures, it should be punctured with a needle. For unruptured swelling, Yiru Jinhuang San (Aurora and Golden Yellow Powder) should be washed/applied; after rupture, Hupo Gao (Amber Ointment) or Bai Gaoyao (White Plaster) should be applied. This condition should be induced to rupture quickly; delay may lead to deep penetration through the eyelid, forming a fistula that is difficult to heal.
Zhenyan (Stye) presents as a bean-sized lump at the eyelash margin. In mild cases, washing can prevent pus formation and cause resolution; in severe cases, redness, pain, and pus formation occur, requiring puncture for healing; after rupture, Wind invasion may cause swelling.
[Note] This condition occurs between the eyelid hairs and lashes, caused by Wind-Heat in the Spleen Meridian, resembling a bean with a pointed tip. In mild initial cases, Yiru Jinhuang San should be used, rinsed with salt water, which resolves the condition before pus forms. If Wind-Heat is severe, with redness and significant pain, washing fails to resolve it, indicating pus has formed; once mature, it should be punctured, and Huanglian Gao (Coptis Ointment) applied. In some cases, after rupture, evil Wind invades the wound, causing facial swelling and red, gritty, painful eyes; external washing should continue, and Xiongpi San (Ligusticum and Tangerine Peel Powder) should be taken internally for recovery.
Xiongpi San
Chuanxiong Rhizoma (Liangqian, 2 liang) Qingpi Pericarpium Citri Reticulatae Viride (1 liang)
Grind into fine powder. Take 2 qian per dose, mixed with Chrysanthemi Flos (Ju Hua) decoction.
Externally, use Mume Fructus Calcined (Kufan) powder mixed with Albumen Ovum (Jidan Qing) applied to the swollen area.
Alternatively, use Arisaematis Rhizoma (Nanxing) powder mashed with Rehmanniae Radix Preparata (Sheng Dihuang) into a paste, applied to Taiyang acupoint for spontaneous resolution.
[Prescription Verse] Xiongpi San contains Chuanxiong, with half the amount of Qingpi being most effective; powdered and taken with Ju Hua decoction, it successfully treats Zhenyan.
