Xiagan (Chancre, a type of ulcer occurring on the genitalia) is formed by the stagnation of pathogenic lust and desire-fire. Its origins are threefold: First, when a man's sexual desire stirs, his penis becomes erect, and the lust-fire rages without release, causing spoiled essence and turbid blood to stagnate midway, condensing into a swelling; this is the first cause. Second, due to uncleaned stagnant essence and turbid qi in the female genitalia, intercourse leads to the transmission of infectious essence, resulting in the condition; this is the second cause. Third, the use of... hot-natured medicines... often causes fire-pathogens to stagnate without dispersing, leading to the condition; this is the third cause. For men suffering from symptoms of desire-stirring and fire-stagnation, the initial stage invariably presents with urethral astringency and dysuria, followed by the discharge of yellow, turbid spoiled essence. The penis gradually deteriorates, and in severe cases, swelling, pain, and rotting occur. The treatment principle should be to drain and regulate the pathogenic fire of the Liver and Kidney, using formulas such as Bazheng San (Eight-Herb Rectifying Powder) or Qinggan Daozhi Tang (Clear Liver and Guide Out Stagnation Decoction). For women caused by unclean genitalia, the initial stage presents with skin swelling that is bright and shiny, resembling crystal in severity. When the skin breaks, fluid leaks out, swelling and pain increase daily, and itching and numbness occur intermittently. Treatment should focus on resolving toxicity and dispelling wind, such as combining Longdan Xiegan Tang (Gentian Liver-Draining Decoction) with Pingwei San (Calm the Stomach Powder). For injuries caused by sexual practices in both men and women, resulting from accumulated toxins, the initial stage presents with penile itching and pain, hardness, purple discoloration, gradual formation of nodules, progressive rotting, dripping of bloody fluid, and frequent erections. Treatment should involve purging fire and resolving toxicity, using formulas such as Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity) or Lushui Wan (Aloe Pill). Externally, select from Yinfen San (Silver Powder Powder), Zhenzhu San (Pearl Powder), or Renzhongbai San (Human Tooth Ash Powder). There are also cases where Chancre (Syphilis/Time Plague) was initially contracted, but due to the mistaken use of fumigation strips and topical applications, toxins became knotted here; these are detailed in the Knotted Toxin Section and are not included in this general classification.
If at the initial stage there is no redness or swelling, the penis does not erect during sleep, the jade stem suffers only slight damage, and urination is smooth and self-regulating, it is a mild case. If already formed but with slight heat and swelling, bright skin color, reddish urine, accompanied by white turbidity, it is a moderate case. If tissue has been damaged but remains red and vibrant, without hard swelling, urination is painless, and defecation is not constipated, it is a treatable case. If at the initial stage urination is dribbling, subsequently damaging the yang organ, which becomes hard and painful, with rotting gradually spreading, it is a dangerous case. If already formed with putrefaction and ulceration attacking inward, damaging the jade stem, presenting with purple color and no pus, and pain like needle pricks, it is a severe case.
