The manifestations of scabies are categorized into five types: dry scabies, damp scabies, sandy scabies, worm scabies, and pustular scabies. These conditions all arise from deficiency of Qi (vital energy) and heat in the Blood (blood), resulting in turbid Ying (nutritive) and Wei (defensive) energies. They typically begin in the web spaces between fingers and spread throughout the body, causing extreme itching. For instance: when Dryness is predominant, dry scabies occurs; scratching leads to dry skin and white scales. When Dampness is predominant, damp scabies occurs; it presents with sudden swelling and pain, breaks open to exude yellow fluid, resembling ulcerated acne, with pus-filled tops, varying in viscosity, accompanied by both pain and itching. If toxic accumulation transforms with Wind-Dampness, worm scabies arises; the itching penetrates to the bone, and scratching yields no sensation of pain. Although scabies appears as an Excess syndrome (repletion pattern), it frequently afflicts individuals with Deficiency patterns. Therefore, treatment should not focus solely on attacking the excess pathogen but must prioritize supporting the underlying deficiency. For robust constitutions, Danggui Yinzi (Angelica Decoction) is the primary formula; for damp and pustular scabies, Huangqi Yinzi (Astragalus Decoction) is used, both washed with Shenjiao Tang (Ginseng and Pepper Soup) and applied externally with Xiuqiu Wan (Embroidered Ball Pill). In cases of severe wet ulcers and pain, Erhuang Dan (Two-Yellow Powder) applied externally proves effective. Regarding worms generated over a long duration, these are transformed from Damp-Heat; clearing the Damp-Heat causes the worms to perish without direct expulsion. Internally take Kushen Wan (Sophora Pill), and externally apply Jinyin San (Honeysuckle Powder) for self-healing. Application of Xiuqiu Wan is also effective.
First, for chronic cases that fail to heal, characterized by extreme physical weakness and severe sores, internally taking Shenshen Zao (Divine Date) and externally applying Erhuang Dan can lead to recovery.
Second, for pediatric scabies, bathing with decoction of Jingjie (Schizonepeta) and using this water to wash clothes is most excellent.
Third, for recurrent scabies after healing or annual recurrences, regularly taking Dusheng Wan (Unique Holy Pill) for regulation is most excellent.
