For all injuries such as falls, blows, crushes, falling from horses, overturning carts, falling from roofs, and sprains: conditions where urination and defecation are unobstructed are mild; those where they are obstructed are severe. In the initial stage, regardless of whether the patient is old or weak, and regardless of the presence of blood stasis stagnation or symptoms like coma, faint breath, and inability to speak, it is advisable to take a large bowl of hot *tongbian* (urine of a healthy young boy), assisted by wine. Its efficacy in eliminating the old and promoting the new is supreme, without damaging the Zang-Fu organs or injuring Qi and Blood, ensuring absolute safety. Subsequently, treatment should be administered according to the severity of the injury. If the skin and flesh are intact but there is distension, stuffiness, and pain in the chest, hypochondrium, abdomen, or belly, it must be due to blood stasis. For robust individuals, *Fuyuan Huoxue Tang* (Restoring Vitality Decoction) is used to attack and resolve the stasis; for the elderly and weak, *Siwu Tang* (Four Substance Decoction) with added *Taoren* (Peach Kernel), *Honghua* (Safflower), and *Chuanshanjia* (Pangolin Scales) is used to tonify while promoting circulation. Externally, fresh ginger and scallion whites are mashed together, mixed with flour, stir-fried until hot, and applied to the injured area, frequently replacing with freshly heated mixtures, providing miraculous pain relief. If the skin and flesh are broken, tendons and bones are damaged, bleeding is incessant, and restlessness ensues, this is termed "loss of blood" (*wangxue*). Immediately apply dry powder of *Huaruishishi San* (Pyrite Powder) or *Jinchuang Bixiao San* (Golden Wound Extremely Effective Powder) to stop bleeding. Internally, *Bazhen Tang* (Eight Treasure Decoction) with added wine-fried *Gusuibu* (Drynaria Rhizome), *Honghua*, and *Xuduan* (Teasel Root) is used for tonification. If stomach Qi is injured by pain causing vomiting, or if appetite is lost, *Liujunzi Tang* (Six Gentlemen Decoction) with added *Danggui* (Angelica Sinensis) and *Muxiang* (Aucklandia Root) is used for regulation. If the injury is mild, without other signs of blood stasis, but muscle pain persists, *Fuyuan Tongqi San* (Restoring Vitality and Promoting Qi Powder) is used to promote circulation.
In such cases, one must examine the severity of the injury, the presence of blood stasis, and the deficiency or excess of Yuan Qi. One must not indiscriminately use purgative methods, lest it lead to disastrous complications. Conditions involving severed tendons or shattered bones are handled by specialized bone-setting practitioners and are not detailed here.
1. For loss of consciousness due to falls/blows: Mash leaves of *Xianrenchai* (i.e., *Murraya paniculata*) to extract natural juice, one cup, and administer orally. Consciousness will return immediately. As long as there is slight warmth in the heart region and the patient can tolerate a small amount, survival is certain.
2. For dislocated or fractured joints: Immediately mash a large live crab, mix with hot aged wine, and drink several bowls. Intoxication is permissible. Apply externally to the injury site; within half a day, a sound within the bone may be felt. (Note: Dried live crabs can also be roasted into powder and taken with wine.)
3. For severed tendons due to falls/blows: Extract juice from *Jinfelugen* (i.e., *Inula japonica*), align the severed tendon ends, apply the juice, and bandage. The tendon will reconnect as before, with near-perfect success. Labor must be prohibited for twenty-seven days.
4. For pregnant women suffering falls/blows: Use *Ejiao* (Donkey-hide Gelatin) 2 qian, *Dangguishen* (Angelica Sinensis Root Head) 1.5 qian, *Aiye* (Mugwort Leaf) 1 qian, *Shengdihuang* (Rehmannia Root) 3 qian, *Xuduan* (Teasel Root) 2 qian, *Sharen* (Amomum Fruit) 0.8 qian (stir-fried), *Chuanxiong* (Ligusticum Rhizome) 0.5 qian (stir-fried), *Honghua* (Safflower) 0.3 qian. Decoct in equal parts water and wine, and take orally.
5. For tetanus resulting from falls/blows/wounds, presenting with fever, high swelling, and even trismus (lockjaw): Use *Qianghuo* (Notopterygium Rhizome), *Fangfeng* (Saposhnikovia Root), and *Cangzhu* (Atractylodes Rhizome) each 1.5 qian, *Baizhi* (Angelica Dahurica Root), [text missing/unclear], and mix with well water for external application.
6. For extremely severe falls/blows with obstruction of urination and defecation, which is a critical condition: First use *Tongdao San* (Unblocking Purging Powder) to move downward stagnant blood, then administer other medicines. This condition's medication must not be decocted with wine.
