Gan sores (ulcers associated with malnutrition or specific infections) include several types, such as Xue Gan (Blood Gan), Feng Gan (Wind Gan), Ya Gan (Dental Gan), and Xia Gan (Genital Gan). Xue Gan arises from deficiency and weakness of the internal organs, where pathogenic heat invades mutually. Externally, it penetrates between the muscle layers and manifests on the skin surface. Initially resembling purple mustard seeds, upon rupture blood flows out, and ulcers form wherever the pathogen travels across the body. This damages Qi and Blood, causing broken flesh to feel empty and itchy; if prolonged without healing, it may evolve into a Gan condition. Feng Gan occurs when the Foot-Yangming Stomach Meridian is invaded by wind pathogens and hot toxins. The pathogen lodges near the Ran Gu acupoint and flows to the side of the Cheng Shan acupoint. It initially appears like scabies or tinea; upon rupture, yellow fluid oozes and forms ulcers. Wind and dampness struggle against each other, and toxic gases accumulate and attack, gradually spreading over the whole body. Some cases occur behind the ears of children; this type of yellow fluid sore is also called Gan sore. Ya Gan is caused by a childhood preference for sweet, fried, roasted, and pungent-hot foods, leading to the accumulation of pathogenic heat in the Foot-Yangming Stomach Meridian. It manifests as swelling and pain between the upper teeth or within the tooth roots, forming rotting flesh and corrupting blood that invade further. This leads to the ulceration of the alveolar bone, foul odor making proximity difficult, eventual tooth loss, and if left untreated for a long time, becomes a major affliction. Xia Gan refers to sores developing on the male penis. This disease results from unfulfilled desires or unclean sexual intercourse, causing pathogenic toxins to soak and penetrate, forming toxic sores. If not healed for a long time, it may become a "Bian Du" (lymphadenitis of the groin) or corrode and damage the genitalia. The condition is severe and should not be taken lightly; it often leads to untreatable outcomes. The treatment methods for the above various Gan sores differ; the specific formulas are listed below.
