Pediatric gan li (malnutrition with dysentery) arises from irregular breastfeeding or weaning, excessive consumption of raw and cold foods, which damage the spleen and stomach. This leads to dysfunction of the zang-fu organs, conflict between cold and heat pathogens, and weakness of the large intestine.
For pediatric gan li presenting with a stiff back like a saw, itching of eyes, mouth, and nose, self-biting of fingernails, dry and brittle hair, acute pain in the lower region (anus), use Qingdai San (Indigo Powder).
Ingredients: Qingdai (Indigo Naturalis, finely ground), Zhusha (Cinnabaris, finely ground), Xionghuang (Realgar, finely ground), Fuzi (Aconiti Lateralis Radix Praeparata, processed until cracked, skin and umbilicus removed), Lilu (Veratri Rhizoma, rhizome head removed), Huhuanglian (Coptidis Chinensis Rhizoma var. Otsuense, fine root), Xixin (Asari Radix et Rhizoma).
The above ingredients are crushed, passed through a fine sieve to make a powder, and thoroughly mixed. Each dose consists of half a qian (approx. 3-5g depending on era/region, here likely smaller for infants), taken with rice gruel, once in the morning and once in the evening. Dosage should be adjusted according to the child's age and size.
For pediatric gan li presenting with a distended abdomen, dry mouth, emaciated limbs, and persistent dysentery, use Shensheng San (Divine Holy Powder).
Ingredients: Gan chanma (Dry Bufo bufo gargarizans, one piece collected on the fifth day of the fifth lunar month, feet and viscera removed), Du ke suan (Single-clove garlic, crushed), Chuanjiao (Zanthoxyli Pericarpium, half a liang, eyes removed).
These two ingredients are finely ground together with previous preparations into a powder. Each dose is half a qian, taken with rice gruel, three to four times daily. Dosage should be adjusted according to the child's age and size.
For pediatric gan li presenting with emaciated limbs, abdominal distension, nasal itching, dry skin, and intermittent dysentery, use Lushui Wan (Aloe Pill).
Ingredients: Lushui (Aloes, finely ground), Xionghuang (Realgar, finely ground), Moshizi (Myrobalan nut), Shetuipi (Ophichthidis Corium, burnt to ash), Dingxiang (Caryophylli Flos), Xiongdan (Ursi Bile), Chanqiao (Cicadae Periostracum, lightly stir-fried, legs removed), Chansu (Bufonis Venenum).
The above ingredients are crushed, passed through a sieve to make a fine powder, and bound with refined honey into pills the size of yellow millet grains. Each dose is three pills, taken with rice gruel. Additionally, one pill is separately ground and insufflated into the nose. Adjust based on the child's condition.
For pediatric gan li that does not stop, presenting with emaciation, poor appetite, abdominal pain, and weakness, use Shexiang Wan (Musk Pill).
Ingredients: Shexiang (Moschi Fermentum, finely ground), Zhusha (Cinnabaris, finely ground), Lushui (Aloes, finely ground), Xionghuang (Realgar, finely ground), Mudingxiang (Cardamomi Fructus, female type), Heshi (Pyrrosiae Folium), Baifan hui (Alumen Ash), Mituo seng (Lithargyrum), Chanqiao (Cicadae Periostracum), Huanglian (Coptidis Rhizoma).
The above ingredients are crushed, passed through a sieve to make a powder, mixed thoroughly with the pre-ground ingredients, and bound with wheat paste into pills the size of mung beans. Each dose is three pills, taken with rice gruel, three times daily.
For pediatric gan li persisting day and night, presenting with emaciation, weakness, and inability to eat, use Muxiang Wan (Costus Root Pill).
Ingredients: Muxiang (Aucklandiae Radix), Chanqiao (Cicadae Periostracum, lightly stir-fried, legs removed), Roudoukou (Myristicae Semen, shell removed), Huandan (Minii Oxidum Rubrum, lightly stir-fried), Zhusha (Cinnabaris, finely ground), Yemingsha (Gulodis Faeces, lightly stir-fried, each one fen), Wo Niu (Helicidae Concha, twenty pieces, stir-fried until finely ground and slightly yellow).
The above ingredients are crushed, passed through a sieve to make a powder, mixed thoroughly with the pre-ground ingredients, and bound with steamed dough soaked in water into pills the size of mung beans. Each dose is five pills, taken with warm rice gruel. Daily frequency is implied but text is truncated.
For pediatric gan li with unpredictable frequency day and night, and emaciated muscles, use Longgu Wan (Dragon Bone Pill).
Ingredients: Longgu (Fossilia Ossis Mastodi, half a liang), Xionghuang (Realgar, one qian, finely ground), Shexiang (Moschi Fermentum, one qian, finely ground), Zhusha (Cinnabaris, one fen, finely ground), Wo Niu (Helicidae Concha, twenty pieces, stir-fried until slightly yellow).
The above ingredients are crushed, passed through a sieve to make a powder, mixed thoroughly with the pre-ground ingredients, and bound with wheat paste into pills the size of mung beans. Each dose is five pills, taken with rice gruel.
For pediatric gan li due to deficiency-cold of the spleen and stomach, indigestion of milk and food, and umbilical/abdominal pain, use Xiongdan Wan (Bear Bile Pill).
Ingredients: Xiongdan (Ursi Bile, five fen), Fuzi (Aconiti Lateralis Radix Praeparata, one piece, processed until cracked, skin and umbilicus removed), Badou (Crotonis Fructus, seven pieces, skin and heart removed, ground and pressed between paper to remove oil), Dingfen (Cerussa, one liang, stir-fried until slightly yellow), Huandan (Minii Oxidum Rubrum, two points, stir-fried until purple), Pishuang (Arsenici Trioxidum, one qian, finely ground), Liuhuang (Sulfur, one fen, finely ground), Jiangjiang (Zingiberis Rhizoma Recens, one fen, baked until cracked and cut), Helile (Terminaliae Chebulae Fructus, one fen, baked, peel used).
The above ingredients are crushed, passed through a sieve to make a powder, and bound with steamed dough soaked in water into pills the size of yellow millet grains. Each dose is two pills, taken with cold water. Dosage should be adjusted according to the child's age and size.
For pediatric gan li that does not stop, use Huanglian San (Coptis Powder).
Ingredients: Huanglian (Coptidis Rhizoma, half a liang, mustache removed, lightly stir-fried), Baifuling (Poria, half a liang), Ejiao (Colla Corii Asini, half a liang, crushed and stir-fried until yellow and dry), Huangbai (Phellodendri Cortex, half a liang, lightly roasted and cut), Renshen (Ginseng Radix et Rhizoma), Moshizi (Myrobalan nut, two pieces).
The above ingredients are crushed, passed through a fine sieve to make a powder. Each dose is half a qian, taken with rice drink. Dosage should be adjusted according to the child's age and size.
