Compiled by Xu Hong, Imperial Physician Assistant, formerly appointed to inspect the Huimin Bureau of the Sichuan General Command.
Generally, when prescribing to treat diseases, one must first diagnose and understand the source of the disease, observe its excess or deficiency, and apply tonification or purgation accordingly. Differentiate the cold and heat of the region, observe the vitality of men and women, deeply understand the subtle properties of herbs and minerals regarding their sweet, pungent, bitter, sour, and salty flavors, as well as the roles of Sovereign, Minister, Assistant, and Courier, and the hot or cold nature of the drugs. Whether it is a disease originating from the meridians themselves or caused by external pathogens, located in Yin or Yang, in the Exterior or Interior, one must carefully examine the varying signs and symptoms, distinguish between excess and deficiency, trace the origin of the pathogenic factors, and know where the disease arises. If the Exterior is excess, purge the Exterior; if the Interior is excess, purge the Interior; if in Yang, treat Yang; if in Yin, treat Yin. By selecting herbs corresponding to the Five Zang organs and adapting them to the appropriateness of the Four Seasons, with balanced modifications and correct application of purgative and diaphoretic methods, no disease will fail to heal. If one does not thoroughly understand the principles of benefit and loss, acts arbitrarily based on personal bias, fails to distinguish contraindications, and mistakenly uses opposing substances together—for example, purging the Interior when the disease is in the Exterior, or dispersing the Exterior when the disease is in the Interior, treating Yang when in Yin, or treating Yin when in Yang—without understanding the logic and obscuring the root cause, the disease will not be cured, and the patient may be harmed. This is deeply to be guarded against. Therefore, a physician must clear their mind and focus their intent, explore profound and subtle principles, assess the depth of the disease condition, clarify the urgency and slackness of drug properties, formulate prescriptions with evidence, and align them with the disease. The essence of this art lies here.
In treating various diseases, one should first use decoctions (Tang) to cleanse and remove stagnation from the Five Zang and Six Fu organs, open the meridians, regulate Yin and Yang, allow righteous Qi to break through evil, moisturize dryness and decay, soothe the skin, and augment Qi and strength. Water can purify all things, hence decoctions are used. If limb diseases persist for a long time and wind-cold is triggered, powders (San) should be used next. Powders can expel pathogens, addressing wind-damp bi-syndrome where pathogenic factors migrate between the Interior and Exterior without a fixed location, thereby calming them. Pills (Wan) should be used subsequently. Pills can expel wind-cold, break up accumulations, dissolve hard masses, improve appetite, and harmonize Ying and Wei. Those who can integrate and apply these methods appropriately can be called superior physicians (Shanggong). Thus it is said: "A physician relies on intention." Generally, herbs for nourishing life often serve as Sovereign drugs; herbs for nourishing nature often serve as Minister drugs; herbs for treating disease often serve as Courier drugs. Using them judiciously ensures few errors.
Generally, treat cold syndromes with hot drugs; treat heat syndromes with cold drugs; treat indigestion with emetic or purgative methods; treat ghost infection and gu poison with detoxifying and antiparasitic drugs; treat carbuncles, sores, and tumors with specific anti-tumor drugs; treat wind-dampness with wind-damp dispelling drugs. Each follows its appropriate indication. Leigong said: "There are three grades of drugs and three stages of disease. Drugs vary in sweet/bitter and heavy/light properties; diseases vary in new/old and cold/heat. Heavy, hot, greasy, sour, and salty drugs and foods are suitable for treating wind diseases but not others. Light, cold, sweet, bitter, and astringent drugs, stones, and foods are suitable for treating heat diseases but not others. Light, hot, pungent, bitter, and bland drugs and foods are suitable for treating cold diseases but not others. This outline briefly reveals the source and flow; observing the symptoms allows one to know the rest. In practice, one must adapt to circumstances and recognize this key principle."
