If bleeding from the wound continues without stopping, apply the hemostatic herbs from the formula externally. If, after washing and opening the wound, the ulcer cavity is very large, first sprinkle Zhujing Xiang (Daemonorops draco), Longgu (Fossil Bones), and Moyao (Myrrha) onto the area. The interior will immediately form an ulcer surface; cover the cavity with You Bei (oiled paper) to allow pus and blood fluid to drain, preventing blockage. In summer, apply Bohe Ye (Menthae Haplocalycis Folium) to the ulcer cavity, wash once daily with warm water, then apply the medicated powder. When the flesh grows over and covers the wound, if it feels like healthy flesh when pinched by hand and causes no pain, apply the wound-healing medication but do not cover it again. Once the pathogenic wind subsides slightly, the wound will naturally contract. However, if solid flesh has not yet grown, absolutely do not attempt to close the wound prematurely, as this may cause internal complications.
If the injury hole is large and intestines or adipose tissue protrude, reduce them into the body and suture. If the hole is small and only adipose tissue protrudes, first administer Qingxin Yao (Heart-Clearing Medicine), manually remove the adipose tissue, and do not suture. This protruded adipose tissue is of no further use and must not be returned to the abdomen, as doing so would cause disaster; it can simply be removed safely. This involves closing the muscle; proceed with removal confidently. For injuries to abdominal muscles, specifically use purgatives to promote bowel movement; do not wait for constipation in the large and small intestines, lest severe complications arise.
For those beaten, crushed by trees, or fallen from heights, such injuries disturb the four limbs and five zang organs, inevitably leaving evil blood (stasis) inside. There is a specific fear of nausea and vomiting. First, administer Qingxin Yao (Heart-Clearing Medicine), Huaxue Yao (Blood-Invigorating/Stasis-Resolving Medicine), and medicines to unblock the large and small intestines in sequence. Add Tongzi Xiaobian (Child’s Urine) to the decoction before administration for immediate effect. Specifically using medicines to unblock the intestines may cause stenosis and obstruction, which is extremely harmful. Combine Qingxin Yao with the previous formula and take medicines to unblock the large and small intestines; naturally, both will become unobstructed. There will be no mental distress, and no evil blood will turbid the heart-mind. Subsequently, administer analgesics to stop the pain.
For blunt trauma, cuts from knives or stones, and other severe injuries, first administer Qingxin Yao (Heart-Clearing Medicine), secondly administer medicines to clear and promote urination, and thirdly administer medicines to remove blood stasis. If the patient's blood has not yet coagulated, it flows out from the wound; if it has coagulated internally, use medicine to drive the stasis into the large intestine for excretion. For those beaten or crushed where evil blood has not yet accumulated, use medicine to dissipate the stasis in the limbs. If the stasis has gathered in the chest, or upper jiao, induce vomiting to expel it through the mouth. If it has gathered in the middle jiao, drive it into the large intestine for excretion. Use these methods for emergency rescue first, then administer analgesics, adjusting the prescription based on Ershiwu Wei Yao (Twenty-Five Ingredient Medicine).
