The Shen (Spirit), Hun (Ethereal Soul), Po (Corporeal Soul), Yi (Intellect), and Zhi (Will) are the correct spirits residing in the Five Viscera. Joy, anger, sorrow, worry, and fear are the emotional manifestations of the Five Viscera. When the functions of a person's Five Viscera are smooth and harmonious, they feel comfortable and at ease; if emotions are suppressed or depressed, feelings of vexation and stuffiness arise. Therefore, when infants and children suffer from variola (smallpox) or varicella (chickenpox), they harbor intense heat-toxin. Because the rash fails to emerge smoothly to the exterior, the toxin remains stagnant within the Zang-Fu organs and cannot dissipate. This leads to erratic emotions, alternating between joy/anger and sorrow/fear, preventing the Spirit from finding peace, resulting in crying day and night. At this time, the physician must examine whether the accumulated toxin is in the Exterior or the Interior, and should treat according to the syndrome by promoting diuresis and purging heat. If there is fever, thirst with desire for water, flushed face, and dark red urine, it is appropriate to use a mild releasing-the-exterior method to allow the pathogen to exit. If there is no external heat but constipation and abdominal fullness are present, the stool should be unblocked. Generally speaking, if there is no stagnant Qi inside, Ying and Wei (Nutritive and Defensive Qi) will circulate freely, the Spirit will be tranquil, and the rash will emerge quickly and smoothly. Ignorant physicians fail to examine properly; seeing only the day-and-night crying, some attribute it to Heart Heat, others to phlegm accumulation. They either use cooling methods or purgatives, which fail to eliminate the heat-toxin in both the interior and exterior, instead causing cold and heat pathogens to combine, leading to multiple diseases. This point must not be overlooked.
Once the rash forms pus and scabs, scratching them open prevents scar formation. Although the scabs have formed now, the skin where the scars remain resembles moles or pits, causing unevenness. Thus, if the scabs are scratched off before they dry completely, scars will not form. Some patients experience recurrence of the rash after recovery. This occurs because, after the rash condition stabilized, proper clearing and draining treatments were neglected, allowing toxic gases and residual heat to stagnate between the muscles. It is advisable to use Mingjiao San (Mingjiao Powder), followed by Mituoseng San (Mituoseng Powder), Nifen San (Nifen Powder), Yingshibai San (Eagle Droppings White Powder), Muli San (Mussel Shell Powder), Machixian San (Purslane Powder), and Qinpi San (Qinpi Powder) applied externally, which is an excellent approach. Rather than applying medication after the rash has recurred, it is better to detoxify before the rash recurs.
The Spleen and Stomach receive Grain Qi, while the Large and Small Intestines excrete dregs and waste. These are the key areas for assessing deficiency/excess and cold/heat during daily life. By observing changes in diet intake, one can know the deficiency or excess of Grain Qi; by observing the slipperiness or obstruction of urination and defecation, one can determine the cold or heat status of the body. Heat causes constipation and obstruction without passage, while Cold causes slippery diarrhea without limit. Currently, if the stool is normal, it indicates that the Zang-Fu organs are peaceful, without severe heat or cold pathology, so neither tonification nor purgation is needed, nor should reckless attacks be made. Furthermore, the *Huoren Shu* (Book of Living People) states: "If urination and defecation are naturally harmonious, the stool is normal." It also says: "If the stool is hard and urination is frequent, Chengqi Tang should not be used for purgation, as the Body Fluids have already moved into the bladder." Therefore, using Maren Pi Yue Wan (Hemp Seed Spleen-Restriction Pill) is sufficient. If urination is clear, purgation is contraindicated, indicating no Interior Heat. If the stool is hard but urination is scanty, attack is premature, as Body Fluids should return to the Stomach, and the stool will pass naturally. In Yangming Disease with spontaneous sweating, if sweating is induced artificially and urination becomes excessive, purgation is contraindicated because both Exterior and Interior Body Fluids are depleted. It also states: "If urination is red/dark, purgation may be indicated; this presents both Exterior and Interior syndromes, treated primarily with Wuling San." It further states: "No bowel movement for six or seven days with headache and fever indicates an Interior syndrome requiring purgation; however, if urination is clear, the disease is not in the Interior but remains in the Exterior." It also states: "Hard stool with a deep and fine pulse indicates an Interior syndrome requiring purgation." From this, we see that ancient physicians always examined dietary intake and the slipperiness or obstruction of urination and defecation before treating. Inferring the Interior from the Exterior is not a method exclusive to treating rash diseases; whether treating rashes or other illnesses, these principles are beneficial. Especially for infants and young children who cannot speak, this method is particularly applicable.
