The manifestations of Taiyang malaria are described in the *Neijing* as causing lower back pain, heaviness of the head, cold sensation arising from the back, initial chills followed by fever, and profuse sweating upon defervescence that is difficult to stop. Treatment involves pricking the Weizhong (BL40) acupoint to bleed. This is because the Bladder Meridian descends to the waist, ascends to the vertex, and connects with the brain; thus, lower back pain, head heaviness, and cold arising from the back occur. Due to meridian deficiency allowing pathogenic factors to invade, chills precede fever. The difficulty in stopping sweating after fever subsides is due to the True Qi being insufficient to overcome the pathogens.
Treatment for Chills and Fever caused by Foot-Taiyang Malaria:
Chai Hu (Bupleuri Radix), Chang Shan (Dichroae Radix), Zhi Mu (Anemarrhenae Rhizoma) [baked], Bie Jia (Testudinis Carapax et Plastrum) [remove skirt edges, stir-fry with vinegar until yellow], Qing Hao (Artemisiae Annuae Herba) [dried], Gan Cao (Glycyrrhizae Radix et Rhizoma) [stir-fried], Zhi Ke (Aurantii Fructus Immaturus) [remove coarse skin].
Coarsely crush and sieve the above herbs. Take 3 qian per dose, add one lamp of water, seven willow branch hearts, and two cun of scallion white (cut finely). Decoct together until the volume reduces to 7/10. Strain and take warm on an empty stomach. Administer another dose before the onset of the attack.
Foot-Yangming Stomach Malaria: When the pathology is in the meridian, it causes initial aversion to cold with shivering (like after drinking alcohol); the cold persists for a long time before fever appears. Upon cessation of fever, sweating occurs. The patient prefers sunlight, moonlight, or fire warmth, feeling comfortable. Treatment should involve needling the acupoints on the dorsum of the foot along the Foot-Yangming Meridian. When the pathology is in the Stomach organ, it causes morning onset of illness with a sensation of hunger but inability to eat. After eating, there is distension and fullness in the chest/hypochondrium and abdominal enlargement. Treatment should involve needling the Foot-Yangming and Foot-Taiyin meridians and bleeding the transverse vessels. The Stomach gathers the Qi of the Chong Mai (Penetrating Vessel) and is known as the Sea of Water and Grain. When pathogens lodge within, the Chong Qi becomes disordered, leading to hunger without ability to eat, and postprandial distension and abdominal enlargement. When transmitted to the meridians, it manifests as alternating chills and fever. Because Yang Qi cannot overcome Yin, the patient prefers sunlight, moonlight, and fire warmth.
Treatment for Foot-Yangming Stomach Malaria presenting with Chest/Hypochondriac Distension and Abdominal Enlargement:
Hou Po (Magnoliae Officinalis Cortex) [remove bark, process with ginger juice], Chen Pi (Citri Reticulatae Pericarpium) [soak in hot water, remove white pith, bake], Bai Zhu (Atractylodis Macrocephalae Rhizoma), Ban Xia (Pinelliae Tuber) [soak in hot water seven times, processed, each 1 liang].
Coarsely crush and sieve the above herbs. Take 3 qian per dose, add one lamp of water, five slices of fresh ginger, and two jujubes (broken open). Decoct until the volume reduces to 7/10. Strain and take warm at any time.
Treatment for Stomach Malaria presenting with Epigastric/Abdominal Fullness and Anorexia:
Zhi Ke (Aurantii Fructus Immaturus) [remove coarse skin, stir-fry with bran, 3 liang], Hou Po (Magnoliae Officinalis Cortex) [remove bark, process with ginger juice], Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Ling (Poria) [each 2 liang].
Coarsely crush and sieve the above herbs. Take 4 qian per dose, add one and a half lamps of water, one piece of fresh ginger (size of a jujube, cut into large pieces). Decoct until the volume reduces to one lamp. Strain and take warm.
