...Therefore, consuming foods that tend to induce wind pathogens and disturb qi will only trigger the underlying disease without resolving the stagnation; thus, symptoms recur with every meal. Xiaoyao San (Free and Easy Wanderer Powder) is a formula for soothing liver qi, resolving stagnation, and dispersing constrained fire, which is why occasional use leads to recovery.
A question was raised: In the aforementioned condition, internal heat is often triggered by food intake. Why does the stirring of internal heat result in chills and trembling? I replied: According to the *Su Zhu* (Commentary on the Suwen), the nature of fire tends toward dryness. When internal heat becomes severe, the defensive yang qi that protects the exterior all converges inward. Consequently, the exterior manifests as chattering teeth and shivering (gu li). This specific condition arises because food triggers internal fire, causing it to blaze intensely. The defensive yang qi, adhering to the drying nature of fire, converges entirely inward. This results in extreme deficiency at the exterior, leading immediately to chills. After a period, the fire subsides, qi calms down, and the defensive yang returns to the exterior, resulting in recovery as before.
A woman presented with amenorrhea (cessation of menstruation), fever, and coughing up black, ink-like phlegm. Treatment aimed at releasing the exterior exacerbated the condition. Pulse diagnosis revealed that the chi (cun-knee) pulse position was several times faster than the others. The pathology stems from menstrual blood regurgitating upward, mixing with phlegm. Because extreme fire mimics water (a concept where excessive heat manifests with cold/water-like signs), the color is black. Misusing exterior-releasing methods aggravated the condition. The Classic states: "There are cases of double death, but no case of double life." Sweat is the fluid of blood. If a patient has already suffered blood loss and is then subjected to sweating therapy, both blood and sweat fluids are depleted. This constitutes "double death," meaning there is no possibility of "double life" (recovery through such dual depletion). In this case, Siwu Tang (Four Substance Decoction) was administered with added Zhimu (Anemarrhena Rhizome), Huangbai (Phellodendron Bark), and Niuxi (Achyranthes Root). After four doses, menstruation resumed, and the patient recovered.
In the spring of the Yiyu year, Mr. Yang Shilin (courtesy name Yuncuo) from Shaoxing suddenly suffered from severe abdominal pain. For about two days, fecal matter and undigested food were vomited out. Physicians had no effective prescription. Fortunately, one physician diagnosed this as a disorder of yin and yang, with kidney qi rebelling upward. He prescribed five qian of Fuling (Poria) taken over several doses, resulting in a cure. Subsequent patients treated with this method also recovered promptly.
