Postpartum metrorrhagia (irregular uterine bleeding) and leukorrhea with red or white discharge are all caused by internal injury from the seven emotions (joy, anger, worry, thought, grief, fear, fright), or deficiency of the lower origin (kidney and mingmen). Wang Shuhe stated: Initially, the disease presents as metrorrhagia; over time, blood loss leads to the exhaustion of yang. Consequently, white slippery substances flow continuously without stopping. This indicates that the Blood Sea (Chong Mai) in the relevant meridian is first depleted, body fluids are exhausted and dried up, failing to nourish the sinews and bones. The principle of medication requires using herbs belonging to the meridian system as guiding agents (Yinshi). One should employ pungent, sweet, and greasy-natured herbs to moisten dryness and replenish body fluids; use greatly pungent and hot herbs to assist the Yang pathway and generate blood vessels; use cold and bitter herbs to drain Lung heat; use Ren Shen (Ginseng Root) to tonify Qi; and use slightly bitter and damp herbs as assistants. This constitutes the general method for treating such conditions.
Metrorrhagia (sudden heavy vaginal bleeding) results from damage to the Zang-Fu organs and the Chong and Ren meridians, due to Qi and Blood deficiency. These two meridians serve as the sea within the collaterals. The movement of Qi and Blood circulates externally along the meridians and internally nourishes the Zang-Fu organs. Because the Qi of the Chong and Ren meridians is severely damaged and deficient, it fails to restrain the blood of the Jueyin meridian, causing sudden profuse downward flow, termed "Metrorrhagia and sudden flooding." Treatment should involve large doses of herbs that greatly tonify Qi and Blood, elevate and nourish the Spleen and Stomach, and slightly add herbs that lift and anchor to clear Heart fire. Bu Xin Xie Huo Tang (Tonify Heart and Drain Fire Decoction) is suitable, after which the bleeding will naturally cease.
All cases of Lin syndrome (dysuria) and leukorrhea are caused by phlegm accumulation flowing downward and infiltrating the Bladder. Treatment should focus on lifting and raising to correct Qi sinking, using Er Chen Tang (Two-Cured Decoction) added with Cang Zhu (Atractylodis Rhizoma) and Bai Zhu (Atractylodis Macrocephalae Rhizoma). For obese individuals with much damp-phlegm, add Ban Xia (Pinelliae Tuber), Nan Xing (Arisaematis Rhizoma Praeparatum), Chao Huang Bo (Phellodendri Cortex Prepared), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Chun Pi (Toosendanii Cortex), and Qing Dai (Indigo Naturalis). For thin individuals with much fire, add Huang Bo (Phellodendri Cortex), Chun Pi (Toosendanii Cortex), Hai Shi (Spongia), Hua Shi (Talcum), and Ha Fen (Margarita Pulveratum). If red and white discharges occur together, add Chao Man Jing Zi (Vittariae Spora Preparata) powder, taken with wine, two qian per dose.
Lou Quanshan stated: Postpartum prolapse of the uterus occurs due to excessive straining during childbirth, resulting in the protrusion of the vagina, resembling rectal prolapse, with the vaginal opening protruding externally, accompanied by swelling and pain. Lifting heavy objects or sexual fatigue can also cause this condition. If clear fluid flows continuously and urination is dribbling, use Liu Huang (Sulfur) and Ou Ze Gu (Sepiae Endoconcha), each five qian, mixed with one fen of Wu Wei Zi (Schisandrae Fructus) into a fine powder, applied to the affected area. Simultaneously administer tonifying herbs such as Ren Shen (Ginseng Root), Huang Qi (Astragali Radix), Dang Gui (Angelicae Sinensis Radix), Gan Cao (Glycyrrhizae Radix et Rhizoma), and Sheng Ma (Cimicifugae Rhizoma) for self-healing.
Zhu Danxi treated a woman in her thirties who gave birth to a daughter. Two days later, a mass like a handkerchief protruded from the vaginal orifice, with a pointed tip hanging down. He reasoned that this was caused by labor and fatigue before delivery damaging Qi, or possibly Liver flaccidity. Fortunately, blood deficiency was not severe. At that time, it was late winter and cold; fearing cold and dryness would damage the tissue, he urgently prescribed Ren Shen (Ginseng Root) one qian, Bai Zhu (Atractylodis Macrocephalae Rhizoma) five fen, Dang Gui (Angelicae Sinensis Radix) one and a half qian, and Sheng Ma (Cimicifugae Rhizoma) five fen. After three doses taken consecutively, the organ retracted, and the patient felt relieved after sweating profusely. However, the part that had touched the bedsheet dried and shed a piece, which was identified as adipose membrane. Her appetite returned, she could sleep, and her pulse was wiry-rough (Se) generally, with the left pulse slightly wiry (Xian). Her physical constitution appeared solid. Thus, he prescribed Bai Zhu (Atractylodis Macrocephalae Rhizoma) and Bai Shao (Paeoniae Radix Alba) five fen each, Chen Pi (Citri Reticulatae Pericarpium) one qian, and one slice of Sheng Jiang (Zingiberis Rhizoma Recens). After three to four doses, she stabilized.
Another woman presented with a mass resembling an inverted bowl with two lobes protruding from the vagina after childbirth. Her husband sought treatment. I reasoned that this was the uterus, likely prolapsed due to Qi and Blood deficiency. Therefore, I prescribed a large dose of Sheng Ma (Cimicifugae Rhizoma) and Huang Qi (Astragali Radix) in two doses. Half a day later, the husband returned, stating that after taking two doses, he heard a sound, and upon inspection, the vagina had retracted completely. However, a piece of tissue, about the size of a palm, had adhered to and torn from the sheet due to drying from menstrual contraction. The wife was weeping, fearing the tissue was damaged beyond regeneration. I explained that this was not intestinal tissue but residual lochia (waste matter), so there was no need for alarm. Muscle tissue can heal; if Qi and Blood become abundant, it will regenerate fully. Thus, I prescribed Si Wu Tang (Four Substance Decoction) added with Ren Shen (Ginseng Root) for one hundred doses. Three years later, she successfully gave birth again.
