因劳累而致的发热,常伴有神思疲惫、多困倦;
感受外邪客热,则见口燥及心烦;
内伤且有瘀血阻滞而成潮热;
外感发热无固定时间,热度时高时低;
平时经前若有实邪积滞需仔细查看;
经后出血过多则按虚证论治;
实证适宜用清热解表法,虚证适宜用温补法;
脉象洪大数急且有力,是实热;
[Verse]
Heat due to overwork is often accompanied by mental fatigue and drowsiness;
External pathogenic heat causes dry mouth and irritability;
Internal injury with blood stasis leads to tidal fever;
External contraction fever has no fixed time, with fluctuating temperatures;
If there is excess accumulation before menstruation, it must be examined carefully;
Excessive bleeding after menstruation should be treated as deficiency;
Excess patterns are suitable for clearing heat and resolving the exterior, while deficiency patterns are suitable for warming and tonifying;
Chills, high fever, and delirium are discussed in a separate category.
[Discussion]
Wang Taipu said: If there is severe cold that does not respond to warming treatment, it indicates a lack of Mingmen Fire (Mingmen Fire Deficiency), and the Heart should be treated. If fever comes and goes unpredictably, appearing during the day and hiding at night, or occurring at night and stopping during the day, this also indicates a lack of Mingmen Fire (Mingmen Fire Deficiency), and the Heart should be treated. If there is extreme heat that does not respond to cooling treatment, it indicates Kidney Yin Deficiency (Kidney Water Deficiency), and the Kidneys should be assisted. Exuberant Heart Qi generates heat, exuberant Kidney Qi generates cold, Kidney Deficiency causes cold to move within the Middle Jiao, and Heart Deficiency causes heat to hide internally.
Li Shi said: Tidal fever with a fixed time belongs to internal injury and deficiency heat; fever without a fixed time belongs to external contraction and excess heat. For deficiency patterns, use Da Wen Jing Tang (Great Warming of the Menses Decoction). For excess patterns, use Si Wu Tang (Four Substances Decoction) added with Chai Hu (Bupleuri Radix) and Huang Qin (Scutellariae Radix). For amenorrhea, use Zi Xue Tang (Nourishing Blood Decoction). For bone-steaming fever, use Da Hu Lian Wan (Great Coptis Pill) and Wu Ji Wan (Black Chicken Pill). For five-palm heat (heat sensation in palms, soles, and chest), use Si Wu Tang added with Hu Lian (Coptidis Rhizoma) and Chuan Lian (Coptidis Rhizoma). For those without sweat, use Fu Ling Bu Xin Tang (Poria Nourishing Heart Decoction). For those with sweat, use Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder).
[Pulse Diagnosis]
A pulse that is surging, rapid, and forceful indicates excess heat;
A pulse that is fine, rapid, and weak indicates deficiency heat.
A floating pulse governs external pathogens, while a deep pulse governs interior heat; through the pulse, exterior/interior and excess/deficiency can be distinguished. The diseases of zang-fu organs and meridians are all contained in the Pulse Formulas.
[General Fever]
Fever occurring generally always leans towards the Yang aspect. In emaciated individuals, Yin Deficiency causes deficient fire to run rampant. If one abstains from pungent foods just as they would eat bland food, it is superior to decocting and taking Di Huang Tang (Rehmannia Decoction).
[Fever Before Menstruation]
Loss of blood during menstruation leads to Yin Deficiency and endogenous heat, causing dry mouth, irritability, and emotional instability. Treatment differs between the beginning and end of the flow; external excess and deficiency-type tidal fevers have distinct names.
[Overwork Fever]
Fever caused by labor is called overwork fever, characterized by limb soreness, abundant night sweats, restlessness, and mental fatigue. Good prescriptions should be administered urgently to prevent the condition from becoming severe. Fever in the morning belongs to the Qi aspect; treat with Qing Xin Lian Zi Yin (Clear Heart Lotus Seed Drink). Fever in the afternoon belongs to the Yin aspect; treat with Di Huang Tang (Rehmannia Decoction). For those who overuse their mind, use Tian Wang Bu Xin Dan (Emperor of Heaven's Heart-Nourishing Pill). For those who overexert physically, use Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder).
[External Contraction Fever]
External contraction fever arises from Yuan Qi (Original Qi) deficiency and shallow Wei Qi (Defensive Qi), allowing external pathogens to reside. Symptoms include irritability, thirst, and dry skin; Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) treats the excess. *Liang Fang* states: During menstruation, when the body is weak and warmth is excessive, external pathogens take advantage, hence called "external contraction fever." Its presentation is deficient heat in the Upper Jiao chest and diaphragm, with dry mouth, irritability, and intense heat in the hands and feet.
[Tidal Fever]
One must know that tidal fever is often Yangming Excess. Fever arrives like the tide without missing its schedule. If it is due to blood stasis stopping in the Taichong channel, Tao Ren Cheng Qi Tang (Peach Kernel Cathartic Decoction) can be used.
[Excess Heat]
A surging and forceful pulse is named excess heat. Heat occurs before menstruation begins, accompanied by constipation, cracked lips, and thirst. Yu Zhu San (Jade Candle Powder) can be decocted to harmonize blood and qi.
[Deficiency Heat]
A weak pulse indicates deficiency. When Yin is insufficient and Qi is relatively excessive, it is certainly due to excessive bleeding after menstruation. Nourishing blood will naturally eliminate it.
[Chills and Fever]
Although chills and fever are said to be caused by external wind-stroke, cases of Yin Deficiency and Yang Weakness are different. One must further seek to distinguish excess and deficiency in Qi and Blood; chills and fever alternating is another category. The Classic says: Yang Deficiency causes aversion to cold, Yin Deficiency causes fever. Insufficient Yang leads to chills first then fever; insufficient Yin leads to fever first then chills. Because Yin and Yang are unharmonized, treatment should differentiate based on Qi and Blood excess and deficiency. A faint pulse at the Cun position indicates Yang insufficiency, where Yin Qi ascends into Yang; Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) is appropriate. A weak pulse at the Chi position indicates Yin insufficiency, where Yang Qi sinks into Yin; Yi Yin Di Huang Wan (Nourish Yin Rehmannia Pill) is appropriate. If anger stirs Liver Fire, use Xiao Chai Hu Tang (Minor Bupleurum Decoction). For alternating chills and fever, use Sheng Di Huang Wan (Raw Rehmannia Pill).
