Huangbai (Phellodendri Cortex), Chuanxiong Rhizoma, Digupi (Lycii Cortex) (each 6 fen), Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Danggui (Angelicae Sinensis Radix), Tiantong (Asparagi Radix), Maidong (Ophiopogonis Radix), Baishao (Paeoniae Radix Alba), Gouqi (Lycii Fructus), Zhimu (Anemarrhenae Rhizoma), Shengdi Huang (Rehmanniae Radix Praeparata) (each 2 qian), Fuling (Poria), Huangbai (Phellodendri Cortex) (stir-fried, each 8 fen), Gancao (Glycyrrhizae Radix et Rhizoma) (5 fen), Wuwei Zi (Schisandrae Chinensis Fructus) (10 berries). This is a formula for making pills. For the above symptoms, if decoction is required instead of pills, add Jujubae Fructus (3 fruits).
For symptoms of bone-steaming tidal fever (a sensation of heat steaming from the bones outward due to yin deficiency), first administer Qinggu San (Clearing Bone Powder) as a decoction. If ineffective, then administer the following prescription:
Renshen (Ginseng Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), Shanyao (Dioscoreae Rhizoma), Gouqi (Lycii Fructus) (each 1 liang), Maidong (Ophiopogonis Radix), Shihu (Dendrobii Caulis) (wine-steamed, each 8 qian), Yinchaihu (Stellariae Radix) (6 fen), Shengdi Huang (Rehmanniae Radix Praeparata) (2 liang), Huhuanglian (Coptidis Rhizoma var. chinense or Coptidis Teetae Rhizoma) (5 qian), Huangbai (Phellodendri Cortex) (wine-stir-fried, 7 qian). Separately, mash Tianmendong (Asparagi Radix) and Shengdi Huang (Rehmanniae Radix Praeparata) into a paste. Add Zihewche (Placenta Humana). If the placenta has been steamed cooked, mash it separately. Then add the powders of the other herbs, mix evenly, and pound thoroughly to form pills. If the placenta is dried by baking and ground into powder, it is added directly to the medicine, mixed uniformly, and formed into honeyed pills.
Postpartum heart pain is actually epigastric pain. Because the epigastrium is located below the heart, pain arises from contracting cold pathogens or consuming cold substances, and since the qi mechanism is close to the heart, it is colloquially called "heart pain." However, one must not forget that the Heart is the sovereign official, governing blood circulation and qi movement, and commanding the zang-fu organs. When blood and qi are abundant, the body remains peaceful; when blood and qi are insufficient, palpitations (zhengchong) and startled palpitations (jingji) occur, leading to restlessness. How could the Heart itself suffer from pain? If it were true Zhenxin Tong (True Heart Pain), the hands and feet would turn blackish-blue, with death occurring in the evening if onset was in the morning, or in the morning if onset was in the evening. This has been discussed in detail previously. The treatment method should be to disperse cold pathogenic factors in the stomach and eliminate cold food stagnation in the stomach. One must primarily use Shenghua Tang (Invigorating Blood Decoction), assisted by herbs that dispel cold and promote digestion. There will be no case that cannot be cured. If the pain is continuous and dull, relieved by pressure, and there is no mass-like pain in the abdomen, one should analyze it as a deficiency syndrome and apply tonification. Regarding the two syndromes of postpartum heart pain and abdominal pain, these arise because cold pathogens, food stagnation, and rebellious qi attack upward toward the heart causing heart pain, or downward toward the abdomen causing abdominal pain. The treatment methods for these two syndromes are roughly the same; both should utilize Shenghua Tang added with Rougui (Cinnamomi Cortex) and Wuzhuyu (Evodiae Fructus) and other warming herbs to warm and disperse the cold.
([Wu's Commentary] The Heart is the sovereign official; if it truly suffers pain, it is incurable. All colloquial terms for "heart pain" refer to qi stagnation pain in the epigastrium. This is most common in women, sometimes even accompanied by vomiting, regurgitation, and cold extremities. Using Huyao (Curcumae Longae Rhizoma), Wulingzhi (Trogopterori Faeces), Danshen (Salviae Miltiorrhizae Radix et Rhizoma), and Danggui (Angelicae Sinensis Radix), named Jiawei Nian Tong San (Modified Relieving Pain Powder), has proven frequently effective.)
