The Cun (inch) pulse of the left hand is surging (Hong), indicating anger stagnating in the chest. The Guan (bar) pulse of the left hand is surging, indicating cold qi in the chest. The Chi (cubit) pulse of the left hand is surging, indicating Kidney deficiency. If the three positions (Cun, Guan, Chi) of the left hand are generally equal in size, it indicates symptoms such as hair resembling silk threads and abdominal distension. For women with diseases of the Blood Chamber (Xueshi), menstruation may come either early or late. The Cun pulse of the right hand is surging, indicating heat in the Upper Jiao, cough with phlegm, and tidal fever (Chao Re). The Guan pulse of the right hand is surging, indicating pain caused by cold qi. The Chi pulse of the right hand is surging, indicating pregnancy; if the Chi pulse is small, it indicates a cold uterus. If the six pulses of both hands are generally surging, slow, clear, and smooth, it indicates longevity. If all six pulses are small, as thin as a thread, it indicates weak Spleen and Stomach, inability to eat, lower back pain, headache, dizziness, and blood stasis/decay. If all six pulses are surging, large, floating, and wiry, it indicates Feng Qi (madness/epilepsy-like conditions). Except for these conditions and postpartum states, the pulse principles for women are the same as for men. If a man's left hand pulse is surging and the right hand pulse is weak, it indicates severe diarrhea. If a woman's right hand pulse is surging and the left hand pulse is weak, it indicates severe diarrhea.
A solid and large pulse on the left hand indicates a male fetus; a wiry and surging pulse on the right hand indicates a female pregnancy. Differences in the prominence of the two Chi pulses can distinguish between male and female fetuses; a slippery and solid pulse at the Mingmen (Gate of Life) indicates pregnancy.
A pulse moving forward (floating) indicates heat; a pulse moving backward (deep) indicates deficiency. Blood stasis should be broken; blood exhaustion should be tonified. Pain occurring frequently before menstruation indicates blood accumulation; pain occurring after menstruation indicates blood deficiency. Fever occurring at the time indicates blood deficiency with heat.
Women's normal physiological cycle (Kun Dao) requires blood to arrive on schedule, with harmonious Qi and Blood being most desirable. If Blood is abundant and Qi is deficient, it is treatable; if Blood is deficient and Qi is vigorous, it gradually deviates from normal. Heat in the Blood causes early menstruation; Wind-Heat causes purple menstrual blood. Qi and Blood stagnation during menstruation leads to pain after flow ceases due to Qi and Blood deficiency. To regulate menstrual blood, Si Wu Tang (Four Substance Decoction) is the primary reference; Wen Dan Tang (Warm Gallbladder Decoction), Zhi Zhu Wan (Aurantii Fructus and Atractylodes Macrocephalae Pill), and Er Chen Tang (Two-Cured Decoction) are also recommended. If the pulse is rapid, add Huang Qin (Scutellariae Radix) and Jing Jie Sui (Schizonepetae Spica); if Qi is deficient and strength is weak, use Ren Shen (Ginseng Radix); if the pulse is deep and slow with Qi stagnation, assist with Bing Lang (Arecae Semen); for pain and fullness/stuffiness, add Qing Pi (Citri Reticulatae Pericarpium Viride); for deficiency-heat, use Xiao Yao San (Free and Easy Wanderer Powder) or Heart-nourishing formulas. Clearing the abnormal menstrual blood will allow it to return on schedule.
Regarding amenorrhea (Jing Bi), it may result from abortion or multiple births damaging the blood, long-term tidal fever consuming the blood, long-term night sweats depleting the blood, disharmony of the Spleen and Stomach leading to poor appetite and insufficient blood source, or emotional distress injuring the Heart, causing Qi knotting and depression, thereby blocking the flow of blood. Treatment should focus on generating blood, nourishing blood, and regulating blood.
