The *Furen Liangfang* (Good Prescriptions for Women) states: When pregnancy reaches the month before delivery, one should calm the spirit and stabilize thoughts, walk frequently, and avoid excessive sleep, overeating, or consuming alcohol and miscellaneous medicines in excess. When labor is imminent, there should be no noise from many people causing panic. If amniotic fluid (referred to as "plasma water") appears and severe lower back pain occurs, this indicates that the fetus has detached from its meridian path [entered the birth canal]. Only then should medicine be administered to induce labor and the mother should ascend the birthing bed. One must not take labor-inducing drugs too early or ascend the bed prematurely; caution is essential.
Xue Lizhai stated: When labor is imminent, if one feels movement inside the abdomen, one should lie supine with the body straight, waiting for the fetus to turn downward. When intermittent abdominal pain begins, test by pinching the middle phalanx or proximal phalanx of the mother's middle finger; if a pulsation is felt, this indicates impending delivery. If movement is felt initially, one should not lie supine immediately, to wait for proper fetal positioning and avoid complications such as malpresentation (tumor displacement). If water flows frequently before actual delivery, this indicates rupture of the membranes (bao yi). If blood and fluids dry up first, there will inevitably be risks of dystocia due to abnormal presentation. If the membranes have ruptured but delivery does not occur, use Baosheng Wuyou San (Preserve Life and Ensure Safety Powder) to consolidate the blood. If blood has already been depleted, decoct Bazhen Tang (Eight-Treasure Decoction) with heavy doses of Motherwort Herb (Yimucao), and drink it frequently. Generally, upper abdominal pain in pregnant women indicates that delivery has not yet begun. Pain extending to the lower back indicates that delivery is imminent. This is because the Kidney governs the waist, and the uterus is connected to the Kidney. For pregnancies where breathing and vital energy are obstructed during delivery, instruct the midwife to only say that delivery has not yet started. In cases of twins, only mention matters related to the placenta/membranes. Fear may cause Qi to scatter, making delivery even more difficult. Generally, dystocia is common among those who suffer from depression and live in leisurely, wealthy households. Although treatment methods often state that one should regulate Qi before delivery and tonify Blood after delivery, one should not adhere rigidly to these rules. If the Spleen and Stomach are weak and Qi and Blood are insufficient, preventive regulation and tonification should be employed.
The *Furin Liangfang* states: Huarui Shi San (Flower Porphyry Stone Powder) is used for blood entering the placental membranes, causing them to swell and preventing expulsion, or for lochia attacking upward. Foshou San (Buddha's Hand Powder) treats critical conditions of Blood deficiency. Jiawei Xiong Gui Tang (Modified Ligusticum and Angelica Decoction) added with Turtle Shell (Guiban) treats non-separation of the pubic symphysis (Jiaogu). Castor Seed (Bimazi) treats retained placenta. Shixiao San (Pain-Relief Powder) treats abdominal pain from lochia retention and unconsciousness. A paste of滑石 (Talcum) mixed with vinegar and oil, applied to the vaginal opening, is a sacred medicine for facilitating smooth delivery. Qinghun San (Clear Soul Powder) treats various syndromes of syncope due to Blood loss. Pingwei San (Peaceful Stomach Powder), Mirabilite (Piaoxiao), and Mercury (Shuiyin) are agents for decomposing dead fetuses. These are all crucial medicines for the time of delivery. If Qi and Blood are deficient and damaged, modify Bazhen Tang (Eight-Treasure Decoction) or Shiquan Tang (Ten-Complete Decoction).
