In the spring of the Guichou year, a woman aged forty-four or forty-five presented with symptoms of shortness of breath when speaking and weakness in the legs; after walking a few steps, her mouth felt as if holding frost. Within seventy days, she menstruated three times. During menstruation, she experienced coldness in the mouth, dizziness of the head and eyes, and cold feet leading to penetrating heart pain. Each time menstruating, her mouth felt cold, and her qi failed to continue. Occasionally, she had heat in the nose, flushed face, and sudden fever. Her body was numb and insensible, unable to walk, with hands and feet unresponsive, unable to bend or stretch. She suffered from cold bi syndrome and bone pain, and occasionally felt sorrowful; dreaming of accompanying her former husband caused severe upward rushing of qi, palpitations, instability of will, mental vagueness, and forgetfulness, yet she could eat. This condition persisted for about a year.
When physicians administered hot medicines, her face flushed suddenly with heat, chest became distended with qi, throat felt obstructed, and she experienced stifling syncope. When administering cold medicines, she suffered from diarrhea. Another physician prescribed *Shiquan Tang* (Complete Ten Decoction), which induced irritability and jumping palpitations. A third physician prescribed *Shuanghe Tang* (Double Harmony Decoction), causing feelings of fire burning on the face and in the abdomen, increased heart palpitations, desire to vomit without success, constipation, tenesmus. I was consulted for diagnosis. The six pulses were wiry and moderate. Seeing this pulse in spring is auspicious; this is a treatable disease. Before medication, she suddenly vomited and had diarrhea. After diarrhea, her anus felt like fire. Although she defecated six times, the volume was not large. At that moment, my understanding of the syndrome was incomplete, so I temporarily prescribed *Jiangqi Tang* (Descending Qi Decoction) by Scholar Yu for eight doses.
The next day, upon diagnosis, the pulse was slightly stronger. She reported that after taking the medicine, she felt heat in the nose, extreme mental restlessness and stifling sensation, and lockjaw. I prescribed *Rensu Yin* (Ginseng and Perilla Drink) for eight doses and approximately 2 liang of *Huanglian Wan* (Coptis Pill). Three days later, she reported that the illness remained unchanged despite the medication. I prescribed *Fuling Buxin Tang* (Poria Tonifying Heart Decoction), but it was ineffective. Upon detailed analysis of the pulse and symptoms, only *Paifeng Tang* (Wind-Expelling Decoction) seemed highly appropriate for this syndrome. Someone asked: How do you know it is this syndrome? First, ability to eat and drink indicates "wind hunger"; second, menstruating three times in seventy days indicates wind in the Ying (Nutrient) channel, causing blood to scatter due to wind; third, dizziness of head and eyes indicates Liver Wind; fourth, flushed face with sudden heat and sadness indicates Heart Wind; fifth, body numbness, inability to walk, and dreaming of the former husband indicate Spleen Wind; sixth, unresponsive hands and feet, lower back pain difficult to bend or stretch, cold bi bone pain indicate Kidney Wind. All these diseases cause heart palpitations, instability of will, mental vagueness, and forgetfulness; this is truly a *Paifeng Tang* syndrome. Someone asked: Wind pulses should be floating. Now the pulse is wiry, moderate, weak, and faint; perhaps it is not wind? I replied: Wind has no fixed pulse. Generally, this syndrome arises from extreme deficiency generating wind. However, the herbs in *Paifeng Tang* contain ingredients similar to *Shiquan Dabu Tang* (Complete Ten Great Tonifying Decoction), possessing a mild tonifying nature without being overly acrid or drying. After ten doses, she reported effectiveness. The pulse improved somewhat, but she still felt smoke-like sensations in the heart, slight heat, and constipation. This is indeed a wind syndrome. I prescribed another ten doses of *Paifeng Tang*, supplemented with *Niuhuang Qingxin Wan* (Calculus Bovis Clearing Heart Pill) and *Zaojiao Wan* (Gleditsia Sinensis Pill). Three days later, she reported that after one or two doses of the previous medicine, severe irritability occurred, and all aforementioned heat symptoms disappeared. Only leg weakness preventing support remained, and the pulse was weak. I prescribed *Mizhuan Jiangqi Tang* (Secret Transmission Descending Qi Decoction) for ten doses. Three days later, she reported that all symptoms receded, except for false fright in dreams, loose stools resembling food stagnation, frequent urination/defecation, and a still weak pulse. I prescribed *Wuji San* (Five Accumulations Powder) for several doses, added Ginseng and salt-fried preparation, combined with *Ganying Wan* (Responsive Pill), and she recovered. Afterwards, she stated she was fine. However, due to heaviness in the upper body and dizziness, she could not stand or sit for long periods. Taking *Paifeng Tang* brought complete relief. Therefore, based on the herbs of this formula and the above methods, one must not fabricate or guess prescriptions arbitrarily.
