Commentary states: When women experience abdominal pain during menstruation, it is due to labor and injury to qi and blood, leading to physical deficiency. Wind-cold pathogenic factors invade the Bao Luo (uterine collaterals), damaging the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel), as well as the Hand Taiyang and Hand Shaoyin meridians. The Chong Mai and Ren Mai both originate from within the Bao Gong (uterus) and serve as the sea of the twelve meridians. The Hand Taiyang Small Intestine Meridian and the Hand Shaoyin Heart Meridian are exterior-interior pairs, governing menstruation from below. If the blood in these meridians is deficient, it becomes susceptible to wind-cold. Thus, at the time when menstruation is about to begin, the movement of blood and qi interacts with wind-cold; the collision between wind-cold and blood/qi causes pain. (For formulas, refer also to Volume 7, Chapter 15).
If the menstrual pathway is blocked, there is severe pain from cold hernia encircling the umbilicus, and the pulse is deep and tight. This is caused by cold pathogenic factors invading the Blood Chamber (Xue Shi), causing blood coagulation and stagnation. Accumulated blood is assaulted by qi, and new blood battles with old blood, thus generating pain. It is like water freezing into ice when the weather is cold and the earth freezes. It is appropriate to use Wenjing Tang (Warming the Meridians Decoction), Guizhi Tao Ren Tang (Cinnamon Twig and Peach Kernel Decoction), and Wanbing Wan (Ten Thousand Diseases Pill). (Formulas see Volume 1, Chapter 7).
