During pregnancy, if there is dribbling, scanty volume, and dysuria (painful urination), it is because Qi and Blood gather to nourish the fetal origin but fail to adequately distribute and moisten the water passages, leading to stagnant heat in the Bladder. Another view states: During pregnancy, the Bao (uterus) is connected to the Kidney. Deficient heat within the Kidneys transfers to the Bladder, causing this syndrome, named Zi Lin (Dribbling Urine during Pregnancy). The primary formula is Jiajian Anrong San (Modified Anrong Powder). However, the ancient formula Anrong San contains Hua Shi (Talcum), which is a heavy-settling agent; it may cause miscarriage. It can be used near term, but before the seventh or eighth month, this ingredient should be removed, and Shi Hu (Dendrobium) and Shan Zhi (Gardeniae Fructus) added for greater safety. *Quan Shu* records: For Zi Lin, if Anrong San is ineffective, Bazhen Tang (Eight-Treasure Decoction) can be taken concurrently. If leg cramps occur with difficulty in urination, Bawei Wan (Eight-Ingredient Pill) must be used urgently; otherwise, it may be incurable. I argue that Rou Gui (Cinnamon Bark) and Fu Zi (Aconite Root) are contraindicated due to their potential to harm the fetus. This likely refers to a version of Bawei Wan containing Mai Dong (Ophiopogon Tuber) and Che Qian Zi (Plantago Seed).
Jiajian Anrong San treats pregnant women with short, scanty, and obstructed urine, or dribbling symptoms.
Ren Shen (Ginseng Radix), Dang Gui (Angelicae Sinensis Radix) [washed in wine, each 2 qian], Mai Dong (Ophiopogonis Tuber) [2 or 3 qian], Bai Zhu (Atractylodis Macrocephalae Rhizoma) [1 qian, stir-fried with soil], Tong Cao (Tetrapanacis Medulla), Fu Ling Pi (Poria Cocos Sclerotium Exocarpium) [each 8 fen], Sheng Gan Cao (Raw Glycyrrhizae Radix et Rhizoma) [5 fen], Deng Xin Cao (Junci Medulla) [5 fen].
Zhuan Bao (Twisted Uterus/Retention of Urine) during pregnancy presents as urgent pain below the umbilicus and complete inability to urinate. This occurs when one forcibly holds urine, walks rapidly when the urge is strong, holds urine after a full meal, or engages in intercourse while holding urine. These actions cause Water Qi to reverse upward, compressing the Bao, causing it to twist and preventing expansion. This is not a disease of the Small Intestine or Bladder, nor can it be treated simply with diuretic drugs. The method should focus on regulating Qi, after which recovery occurs. Women with weak congenital constitution, prone to worry and depression, irritable temperaments, or those consuming rich flavors often suffer from this. Because the Bao cannot rotate normally and is compressed by the fetus, lifting the fetus allows the Bao’s connections to flow freely, restoring urinary patency. Erchen Shengti Yin (Erchen Decoction for Lifting and Raising) can be used. Alternatively, Buzhong Yiqi Tang (Decoction to Supplement the Middle and Augment the Qi) may be used, followed by inducing vomiting to lift the Qi and restore flow. Immediately after flow is restored, using large tonics like Ren Shen may risk miscarriage. However, Zi Lin and Zhuan Bao are similar syndromes. Frequent urination with scanty drops and pain is Zi Lin, caused by deficient heat in the Bladder and Small Intestine. Deficiency fails to control water, and heat fails to promote free flow, resulting in dribbling. Frequent urination with scanty output but no pain is Zhuan Bao; occasional mild pain differs fundamentally from the stinging pain of Zi Lin. The "urgent pain" mentioned in discussions refers to pain felt when urgently needing to void, which stops upon urination. Occasional mild pain is distinct from the continuous stinging pain of Zi Lin.
Erchen Shengti San (Erchen Powder for Lifting and Raising) treats pregnant women with abdominal pain and obstructed, dribbling, or absent urination, or mild pain, distinguishing it from Lin syndrome. It is caused by Qi deficiency and fetal pressure on the water Bao.
Ren Shen (Ginseng Radix) [1 qian], Bai Zhu (Atractylodis Macrocephalae Rhizoma) [soil-fried], Sheng Di Huang (Rehmanniae Radix) [each 1.5 qian], Dang Gui (Angelicae Sinensis Radix) [2 qian], Chuan Xiong (Ligustici Chuanxiong Rhizoma) [8 fen], Ban Xia (Pinelliae Tuber) [6 fen, processed or oil-fried], Chai Hu (Bupleuri Radix), Sheng Ma (Cimicifugae Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), Zhi Gan Cao (Honey-fried Glycyrrhizae Radix et Rhizoma) [each 4 fen], Sheng Jiang (Zingiberis Rhizoma Recens) [1 slice].
Decoct and take orally. I believe that instead of inducing vomiting, it is better to administer lifting and tonifying herbs after urination has been restored.
