The correct methods of tonification and sedation must first clearly understand the pathways of Ying and Wei (nutritive and defensive qi) circulating through the meridians of the left, right, yin, yang, interior, exterior, upper, and lower parts of the human body.
(I) For men before noon: The Yin meridians of the left hand descend, moving from the chest to the hand; the Yang meridians of the left hand ascend, moving from the hand to the head; the Yang meridians of the right hand descend, moving from the head to the hand; the Yin meridians of the right hand ascend, moving from the hand to the chest; the Yin meridians of the right foot descend, moving from the chest to the foot; the Yang meridians of the right foot ascend, moving from the foot to the head; the Yang meridians of the left foot descend, moving from the head to the foot; the Yin meridians of the left foot ascend, moving from the foot to the chest.
(II) For men after noon: The Yin meridians of the right hand descend, moving from the chest to the hand; the Yang meridians of the right hand ascend, moving from the hand to the head; the Yang meridians of the left hand descend, moving from the head to the hand; the Yin meridians of the left hand ascend, moving from the hand to the chest; the Yin meridians of the left foot descend, moving from the chest to the foot; the Yang meridians of the left foot ascend, moving from the foot to the head; the Yang meridians of the right foot descend, moving from the head to the foot; the Yin meridians of the right foot ascend, moving from the foot to the chest.
(III) The method of sedation needling before noon differs between men and women.
For men's left hand and foot, rotate the needle inward. Instruct the patient to inhale through the mouth or nose (inhale through the nose for hand needling, through the mouth for foot needling). On Yang days, use odd numbers (3, 7, 9, selected based on disease severity). On Yin days, use even numbers (2, 6, 8, selected based on disease severity).
For women's right hand and foot, rotate the needle outward. Instruct the patient to exhale through the mouth or nose (exhale through the nose for hand needling, through the mouth for foot needling). Use odd numbers on Yang days and even numbers on Yin days.
What are Yang days? Jia, Bing, Wu, Geng, Ren. What are Yin days? Yi, Ding, Ji, Xin, Gui.
(IV) The method of tonification needling before noon differs between men and women.
For men's left hand and foot, rotate the needle outward. Instruct the patient to exhale through the mouth or nose (exhale through the nose for hand, through the mouth for foot). Use even numbers on Yang days and odd numbers on Yin days.
For women's right hand and foot, rotate the needle inward. Instruct the patient to inhale through the mouth or nose (inhale through the nose for hand, through the mouth for foot). Use even numbers on Yang days and odd numbers on Yin days.
(V) The method of sedation needling after noon differs between men and women.
For men's left hand and foot, use the method of sedation needling for women before noon. Although the sides differ, the method of rotating the needle outward while exhaling is the same.
For women's right hand and foot, use the method of sedation needling for men before noon. Although the sides differ, the method of rotating the needle inward while inhaling is the same.
(VI) The method of tonification needling after noon differs between men and women.
For men's left hand and foot, use the method of tonification needling for women before noon. Although the sides differ, the method of rotating the needle inward while inhaling is the same.
For women's right hand and foot, use the method of tonification needling for men before noon. Although the sides differ, the method of rotating the needle outward while exhaling is the same.
(VII) The above describes the standard cases for men and women during the periods after Zi hour, before noon, after noon, and before Zi hour. This corresponds to the principle in the *Ling Shu* (Spiritual Pivot): "For men, inward; for women, outward. Firmly resist against exit, carefully guard against entry." It also states: "To meet and deprive is sedation; to follow and continue is tonification." Meeting and following, harmonized by intention, completes the art of needling.
(VIII) Summarizing the essentials: For men before noon, rotating the needle inward while inhaling constitutes sedation, and rotating outward while exhaling constitutes tonification. Men's before-noon method is opposite to their after-noon method. Women's after-noon method is identical to men's before-noon method. Women's before-noon method is opposite to men's before-noon method. This distinction must not be overlooked.
