Chuanxiong (4 qian) Danggui (8 qian, soaked in wine) Gancao (5 fen, honey-fried) Ganjiang (4 fen, stir-fried until blackened but retaining its medicinal properties) Taoren (peeled, 10 grains)
Use two bowls of water, decoct until three-sevenths of the liquid remains. Add six or seven tea spoons of yellow wine, and take while slightly warm. The dregs should be combined with the next dose for further decoction. The three doses are to be decocted a total of three times within two shichen (4 hours). They should be consecutively decocted and taken before any food intake. Because the lochia (postpartum discharge of blood and mucus) in the lower jiao can be rapidly transformed into new blood if the medication is taken frequently and in sufficient quantity, dizziness symptoms can be naturally avoided. For those who were constitutionally weak during pregnancy or suffer from fatigue due to overwork postpartum, two additional doses should be prepared to prevent exhaustion. Moreover, after taking one dose, the patient's spirit will recover somewhat, so there is no need to dislike frequent medication. If a deficient person presents with critical symptoms, or has had an abortion due to heat syndromes, or suffers from body fever and headache due to severe fatigue, taking four or five doses may provide slight relief, but if blood stasis pain persists, continued administration is necessary to eliminate the pain.
If Bianhua Tang (Postpartum Transformation Decoction) was not taken within seven days postpartum and blood clots with pain persist, Bianhua Tang should still be used to dissipate clots and relieve pain. Within three days postpartum, if two or three doses of Bianhua Tang do not resolve the clot-related pain, continue taking several more doses according to the original prescription; naturally, the clots will dissipate and pain will cease. As new blood grows vigorous, spirit will naturally recover.
If blood clots remain unresolved within seven days postpartum, do not arbitrarily add Renshen (Ginseng Root), Baizhu (Atractylodes Macrocephala Rhizome), etc., which tonify qi and strengthen the spleen. If pain persists after using these, it may be because delivery just occurred, or clot pain has not stopped within one or two days. In such cases, the parturient's qi and blood may be depleted, presenting as fainting, excessive sweating with cold limbs, pale complexion, cold breath, incessant irritability and thirst, or panting and shortness of breath. Regardless of whether there is clot pain, temporarily use a larger amount of Renshen combined with Bianhua Tang to support critical conditions. In summer, parturients taking Bianhua Tang to remove clot pain should also use hot clothes to warm the abdomen. If protection from cold is lost, even with medication, the clot pain will not stop. Constipation lasting eight or nine days postpartum is due to blood deficiency causing intestinal dryness. It is appropriate to take more Bianhua Tang added with Haren (Hemp Seed) to promote bowel movement and moisten the intestines. Taking Chuanxiong and Danggui in large quantities will naturally restore regularity. For deficient patients, add 2-3 qian of Renshen; strictly avoid using Dahuang (Rhubarb Root and Rhizome) for purgation.
Within one or two days postpartum, if taking three or four doses of Bianhua Tang reduces the clots and the pain can be relieved by massage, this indicates a deficiency syndrome. It is suitable to add 3 qian of Renshen to Bianhua Tang.
If, within seven days postpartum, cold pathogens or cold foods cause blood clot condensation and pain, add Rougui (Cinnamon Bark) 5-6 fen to Bianhua Tang. If condensation occurs more than half a month or a month later, administer Six-Ingredient Pill for Eliminating Stagnation (Sixiao Wan) with a tonic soup.
For postpartum syncope, pulse depletion, physical collapse, cold mouth, and other critical conditions, only Renshen can save the situation as a temporary measure. For obese individuals with phlegm, or those who suddenly fainted due to extreme anger (stroke-like condition), add Zhuli (Bamboo Sap) and Jiangzhi (Ginger Juice) to Bianhua Tang.
