Yuan Yi (Round Opacity): (At the onset, patients perceive floating flies, flower-like shapes, or obstructions resembling hanging ants, thin mist, or light smoke. It typically affects one eye first and subsequently spreads to both eyes, forming a round opacity like an oil drop floating on water. It appears smaller under bright light and larger in dim light. It can be removed by needle dislocation with a gold needle.) Hua Yi (Slippery Opacity): (The opacity resembles a mercury bead; it is suitable for needle dislocation.) Se Yi (Rough Opacity): (The opacity has the color of congealed fat; it is suitable for needle dislocation.) Fu Yi (Floating Opacity): (The opacity hides deep within the eyeball and is visible only upon close inspection; it is suitable for needle dislocation.) Heng Yi (Horizontal Opacity): (The opacity is distributed horizontally, shaped like a sword ridge, thin on both sides and thick in the center. Needle dislocation should be performed at the thick central part. For the above five types of opacities, the disease usually starts in one eye and later affects both. At the onset, there is eye pain and dryness, dizziness, and frontal headache. Although opacities are present, needle dislocation is difficult. However, for Yan Yue Yi (Crescent Moon Opacity), Zao Hua Yi (Jujube Flower Opacity), and Hei Shui Ning Yi (Black Water Congealed Opacity) with only mild dizziness and frontal headache, gentle needle dislocation is appropriate.) Bing Yi (Ice Opacity): (At the onset, there is dizziness, frontal headache, pain in the eyelids, zygomatic bones, and nasal cheeks, and redness inside the eye. It starts in one eye, and the opacity becomes hard and white like ice. Acupuncture should be applied to the acupoints along the affected meridians. Bleeding is contraindicated. The opacity should be loosened by needling but not forcibly displaced.) Yan Yue Yi (Crescent Moon Opacity): (At the onset, there is mild dizziness and frontal headache. It starts in one eye and spreads to both. The opacity is half dense and half thin. Needling is indicated, starting from the denser part.) Zao Hua Yi (Jujube Flower Opacity): (At the onset, there is mild dizziness, eye dryness, and frequent itching and pain in the eyes. It starts in one eye and spreads to both. The opacity surrounds the pupil like sawteeth. It should be gently displaced without leaving short remnants. Acupuncture and moxibustion should be applied to the acupoints along the affected meridians.) San Yi (Scattered Opacity): (The opacity resembles butter spots, alternating between cyan and white. Needle dislocation is indicated.) Hei Shui Ning Yi (Black Water Congealed Opacity): (At the onset, there is dizziness, eye dryness, and visual disturbances with flowers. The color is unstable between yellow and black, and the opacity congeals into a cyan color. Needle dislocation is indicated.) Jing Zhen Yi (Shock/Vibration Opacity): (Caused by head trauma where evil blood flows into the eye, forming an opacity after 20-30 years. The opacity is white. The initially affected eye should not be needled to avoid damaging the subsequently affected eye. Needle dislocation is indicated.) Bai Yi Huang Xin (White Opacity with Yellow Center): (For opacities white on the periphery and yellow in the center, first administer Zhu Yi San (Opacity-Expelling Powder), then needle the acupoints along the foot meridians, and finally use a gold needle for gentle dislocation. If one eye is damaged first, both will eventually be affected.)
Even if there is no pain or itching, opacities that are yellow or red should not be needle-dislocated. Broken or scattered opacities should not be needle-dislocated. Opacities that are dense in the center should not be needle-dislocated. Those that cannot be displaced are called Si Yi (Dead Opacity) and dislocation is contraindicated. Only Bai Yi Huang Xin (White Opacity with Yellow Center) requires medication before needling. Conditions without opacity are called Feng Chi (Wind Redness) and should not be needled.
Wu Feng (Black Wind): (No opacity, but the pupil is small. An opacity forms within 3-5 years, appearing cyan-white. Needle dislocation is contraindicated. Blurred vision indicates deficiency; tonification with herbs is indicated, while purgation is contraindicated.) Gan Feng (Liver Wind): (No opacity. Patients frequently see illusory flowers in front of their eyes, which may be white, black, red, or yellow, or see double images. Both eyes are affected simultaneously. Urgent tonification treatment is required. Sexual intercourse and fatigue must be strictly avoided.) Wu Feng Bian (Transformation of Five Winds): (At the onset, there is dizziness and frontal headache. One eye may be affected first, or due to vomiting, both eyes become dark with pupils white as frost. Lü Feng (Green Wind): At the onset, there is dizziness and pain in the lateral forehead, extending to the eyelids, eyebrows, and nasal cheeks. There is internal eye pain and dryness. It starts in one eye and spreads to both. There is no opacity, but patients see flowers, which may be red or black. Hei Feng (Black Wind): At the onset, there is dizziness and pain in the lateral forehead, extending to the eyelids and nasal cheeks. There is eye pain and dryness. It starts in one eye and spreads to both. There is no opacity, but patients see black flowers.) Qing Feng (Blue/Green Wind): (At the onset, there is mild pain and dryness, dizziness, and brain pain. It starts in one eye and spreads to both. There is no opacity, but symptoms worsen with fatigue and exhaustion.) Lei Tou Feng (Thunder Head Wind): (At the onset, there is dizziness, nausea, and vomiting. It starts in one eye and spreads to both, causing injury. The pupil size varies, and the opacity resembles congealed fat turning white.)
