Coughing accompanied by hematuria: if the pulse is fine and minute, there is hope for survival; if the pulse is large, death ensues.
Alternating chills and fever with convulsions: if the pulse is intermittent and absent, death ensues.
Exterior excess combined with interior heat causing vomiting and diarrhea: if the pulse is deep and fine, there is hope for survival; if the pulse is surging and large, death ensues.
Interior excess causing abdominal distension and pain, dry retching, and vexatious heat in the hands and feet: if the pulse is surging, large, and full, there is hope for survival; if the pulse is deep and fine, death ensues.
Exhaustion of both Yin and Yang, with the upper teeth appearing like ripe small beans (indicating gum recession or abnormal tooth exposure): if the pulse is restless, death ensues.
Body heat with a floating and choppy pulse: death ensues.
Sudden loss of voice without cause, with no palpable pulse: this is due to sudden Qi rebellion (Qi Jue); recovery occurs when Qi returns.
The patient feels hunger and cold, the pulse is fine, Qi is deficient, suffering from dysentery/diarrhea, and unable to take food: this is called "Five Deficiencies" (Wu Xu). Such patients will inevitably die.
The patient can swallow gruel which enters the stomach, but suffers from profuse diarrhea; upper body has high muscle heat, and both anterior and posterior orifices are blocked; the stomach is stifling with a vigorous pulse: this is called "Five Excesses" (Wu Shi). Such patients will inevitably die. However, if sweating occurs followed by bowel movement, there is hope for survival.
In the elderly, a minute pulse: if Yang is weak and Yin is strong, there is hope for survival; if the pulse is restless, large, and rapid, death ensues. If Yin is weak and Yang is strong, and the pulse presents as intermittent, death occurs on odd-numbered days.
When the disease is severe, a surging and large pulse indicates easier recovery; an irregular pulse indicates difficult recovery.
A patient with a full, large, rapid, and frequent pulse faces a dire prognosis.
The Cun Kou pulse on the left hand beats unevenly, fluctuating between large and small. From the Cun position to the Guan position, and from the Guan to the Chi position, all three positions tremble differently, unlike normal individuals. If this pulse is found in mid-summer, death will occur when peach leaves fall. If the pulse is small and urgent, accompanied by hemiplegia of the back and diaphragm, and the patient is under twenty years old, death will occur within three years. If the pulse strikes forcefully, accompanied by epistaxis and fever, death ensues.
The Cun Kou pulse on the right hand sinks and hides unevenly, fluctuating between large and small. In the morning it is floating and large, but by evening it sinks and hides. When floating and large, it exceeds the Yu Ji point; when sinking and hiding, it does not reach the Guan position. It comes and goes unpredictably, sometimes hidden then reappearing; death will occur when elm leaves wither.
If the pulses at all three positions are all choppy, all slippery, all urgent, all weak, all taut like a drawn bow, all minute and hidden, all fine and rapid, or all bead-like: long-term patients presenting these pulses will die.
Tai Chong acupoint is located two cun behind the root of the big toe of both feet, in the depression where the artery beats. (Another view says one and a half cun.) This is the point where Qi of the Foot-Jueyin Liver Meridian pours in. Diagnosing this pulse can determine life or death in men. (Alternatively, diagnose Tai Xi or Ming Men pulses; the acupoint is in the depression of the artery behind the medial malleolus on the heel bone.)
Chong Yang acupoint (also known as Hui Yuan, i.e., Fu Yang acupoint) is located on the dorsal aspect of the foot, five cun above, on the artery between the bones, three cun away from Xian Gu acupoint. (This is where shoelaces are tied on the dorsum of the foot.) Diagnosing this pulse is used to observe the presence or absence of Stomach Qi.
