When the infant is just born, if the umbilical cord is cut late or bathing is delayed after cutting, exposure to cold may cause Cold invasion of the Middle Jiao (pathogenic cold invading the spleen and stomach). Additionally, if the cord is cut before bathing, water dampness may enter the body; or if exposed to water vapor during bathing, it also presents as signs of Cold invasion. The symptoms include severe colicky pain in the abdomen, twisting and crying out, cyanotic and dark complexion, and clear, colorless urine. One should use powder made from Angelicae Sinensis Radix (Danggui) applied on cotton wool, performing moxibustion and hot compress on the umbilicus and abdomen. This should be done frequently daily until the crying stops and the umbilicus dries. However, one must avoid excessive heat from moxibustion to prevent inducing convulsions.
After bathing and cutting the umbilical cord of a newborn, if the swaddling clothes become soiled, and after the umbilical cord falls off, the infant does not drink milk or urinate for several days, this is caused by difficult labor, delayed movement of swaddling causing cold invasion, or slight exposure to wind pathogens damaging the Zang-Fu Qi. Infants with these signs often develop Lockjaw (Neonatal Tetanus). Treatment must not be delayed.
Tongguan San: Treats newborns who do not suckle or urinate. Store two he of milk in a silver vessel, add four pieces of scallion white (one cun each), decoct to obtain one he of liquid, and administer in divided doses. Immediate effect is observed after completion of the dose.
Within seven days after birth, if an infant suddenly presents with a cyanotic face, inability to cry, puckered lips and mouth, inability to suckle, perioral cyanosis, vomiting of white foam, and cold extremities, these are signs of Umbilical Wind and Lockjaw. This results from improper cutting of the umbilical cord at birth causing injury, or invasion by wind-damp pathogens. Mild cases involve the skin, manifesting as non-healing umbilical ulcers; severe cases involve the internal organs, resulting in Umbilical Wind and Lockjaw. This is similar to adults suffering from tetanus due to wound infection, presenting with trismus, puckered mouth, inability to eat, rigidity, and cold extremities. Hence it is called Umbilical Wind and Lockjaw, which is the most malignant disease. *Qianjin Fang* states that among infants suddenly afflicted with Umbilical Wind and Lockjaw, fewer than one in a hundred survive, leading to death through inaction, which is truly pitiable. There is a highly effective method rarely known today. Upon occurrence, examine the infant's gums for rice-grain-sized blisters. Quickly dip a piece of cooked cotton in warm water, wrap it around a finger, and gently rub the blister until it breaks; the infant will immediately open their mouth and calm down.
Another method: Observe the upper and lower gums for white spots the size of a bean. Pinch the center with a fingernail, working from outside to inside in a circle, allowing slight bleeding. Also pinch the edges of the white spot to cut off internal and external qi flow, without piercing too deeply to avoid harming the infant. These two methods are similar. *Zimu Milu* records applying Centipede Powder (Wugong) to the punctured area for great efficacy. Relevant formulas are listed later.
Treating Neonatal Tetanus caused by improper care after cord cutting leading to wind-damp invasion, preventing suckling. It must be treated promptly, otherwise it progresses to Lockjaw.
Formula: Radix Angelicae Sinensis (Danggui) (remove roots, half liang, wash and dry); Tianjiangzi (remove shells, three pieces, lightly stir-fry); Human Hair (Luanfa) (one qian, burn to ash).
Grind into fine powder, add a small amount of Moschus (Shexiang), and apply to the umbilicus frequently.
Another Formula: Cortex Phellodendri (Huangbai) (cut); Fu Xia Mei Mei (soot from pot bottom); Human Hair (Luanfa) (burn to ash, retain nature).
Equal parts, grind together into fine powder. Apply one zi or half a qian to the affected area.
Jinliang Huang San: Treats non-healing umbilical ulcers in infants where wind pathogen transmission leads to Lockjaw or epilepsy.
Radix Coptidis (Chuanhuanglian) (remove roots, one fen, grind to powder); Hu Fen (lead carbonate, one qian, grind); Long Gu (fossil bone, one qian, red grind).
Grind together into fine powder. Apply a small amount to the site frequently.
Dingming San: Treats neonatal tetanus caused by wind invasion after cord cutting, presenting with cyanotic puckered lips, vomiting white foam, and cold limbs.
Scolopendra (Wugong) (half a strip, wine-processed until dry); Rhizoma Aconiti Lateralis Preparata (Chuan Wutou) tip (three pieces, raw); Moschus (Shexiang) (small amount).
Grind into powder, mix evenly with Moschus. Take half a zi, decoct Silver and Gold Mint Tang, and administer.
Treating Neonatal Lockjaw:
Ophichthys Skin (Shetui) (one fen, lightly stir-fried); Uncaria Rhynchophylla (Gouteng) (one fen); Dry Scorpion (Gan Xie Shao) (one fen, above ground parts first ground to fine powder); Cinnabar (Zhusha) (separately ground, water-sedimented); Moschus (Shexiang) (half a qian, separately ground); Real Bovine Calculus (Niuhuang) (half a qian, separately ground).
Grind together finely, mix well, and grind again. Take one zi per dose. Mix with one or two drops of Bamboo Sap (Zhuli) and breast milk, then administer. Another prescription uses only Ophichthys Skin, Cinnabar, and Moschus, one fen each. Take half a zi, mix with saliva, and apply to the mouth five to seven times daily.
Another Method: Place Salted Fermented Soybeans (Yanchi) on the umbilicus and moxibustion.
Another Method: Decoct branches of Albizia julibrissin (Yehua) strongly, use the juice to wipe the mouth and wash.
Another Method: Prick the sides of the infant's mouth until blood appears, then apply juice from crushed Scolopendra (Tianjiangzi).
Another Method: Crush fresh Geophilus (Shufu) and mix with Scolopendra juice, apply to the mouth. Gradually resume suckling for effectiveness. Geophilus is an insect generated in damp environments.
