Blindness at dusk. This is a common name. It is also called "chicken blindness." In this specialty, it is referred to as *Gaofeng Neizhang* (High-Wind Internal Obstruction). The patient cannot see in the evening but regains vision at dawn. Medical texts attribute this to the wood element (Liver) being born at Hai hour, flourishing at Mao hour, and perishing at Shen hour. By You and Xu hours, the wood qi is extremely weak, hence the inability to see. When the sun rises at Mao hour, the wood qi becomes slightly vigorous, hence vision returns. *Ge Fen Wan* (Clam Powder Pill), *Zhu Gan San* (Boiled Liver Powder), and *Jueming Yeling San* (Cassia Seed and Night Owl Powder) are effective; thereafter, regularly taking *Liuwei Wan* (Six-Ingredient Pill with Rehmannia) added with Danggui (*Angelicae Sinensis Radix*) and Shashen (*Glehniae Radix*) ensures lasting health. According to the *Neijing* (Inner Canon), the eyes can see only when nourished by blood. If blood is deficient, the Liver loses its nourishment, and vision fails. Night belongs to Yin, and human blood belongs to Yin. Yin is quiet and dislikes agitation. If Yin is deficient, Fire must be hyperactive; weak Yin cannot overcome strong Fire, so symptoms worsen at night, causing dimness and blindness. At dawn, Yang takes command; Yang is active, and the pathogenic Fire temporarily dissipates, so vision slightly improves. Treatment should focus on tonifying Qi and nourishing Blood. Consuming beef or pig liver leads to recovery, further indicating that Yuan Qi (Original Qi) is weak and Yin fluids are insufficient.
Damage to the True Eye's Glaze. This results from Heat injuring Kidney Water (Essence), leading to defects in the Shen Glaze (lens/vitreous humor). The manifestation involves pathology of the Feng Lun (Wind Wheel, i.e., cornea/pupil area), presenting as scars or depressions of varying lengths and sizes. Some depressions are small like needle pricks, others large like hairpin tips, some scars resemble nail scratches, and some involve long scars around the circumference of the Wind Wheel. All such conditions stem from stagnation in the collaterals of the Liver and Gallbladder channels, where severe Heat steaming burns and damages the Shen Glaze. Treatment should employ *Liuwei Wan* (Six-Ingredient Pill with Rehmannia) added with Danggui (*Angelicae Sinensis Radix*), Shijueming (*Haliotidis Concha*), Baijili (*Tribuli Fructus*), as well as *Bazhen Tang* (Eight-Treasure Decoction) or *Buzhong Yiqi Tang* (Tonify the Middle and Augment the Qi Decoction). Early treatment is urgent to prevent deep excavation into cavities, evolution into Xiejing protrusion (iris prolapse), or full opacification resembling ice flaws. Long-term administration of potent tonic agents is required to restore clear aqueous fluid and healthy glaze. If treatment is intermittent or neglected, white halos will inevitably remain for life.
Glaze Damage and Globe Depression. This refers to the sensation of the globe sinking low and lacking freshness or plumpness. It differs from various fistula-like conditions involving green or yellow protrusions. Causes vary: excessive sexual activity depleting Kidney Water; preference for pungent and dry foods damaging Jin Ye (Body Fluids); prolonged stagnation of Wind, Phlegm, Dampness, and Heat steaming and damaging the eye's glaze; erroneous injury to meridians during unnecessary bleeding or excessive blood loss preventing glaze fluids from moistening and nurturing; or damage to fluid juices due to excessive weeping. Generally, these are all cases of weak Yuan Qi (Original Qi) and insufficient glaze fluids. Treatment should primarily warm and nourish Blood and Qi. Cooling agents must strictly be avoided. If a person's eyes sink spontaneously without cause, death is imminent. If there are external malignant signs combined with internal damage to the eye's glaze, the condition is incurable.
