Abdominal distension and Qi stagnation, specifically deficiency-type distension, often occur after vomiting and diarrhea when purgative treatments have been overused. It is appropriate to use *Cortex Magnoliae Officinalis* (Houpo), *Atractylodis Macrocephalae Rhizoma* (Baizhu), *Exocarpium Citri Reticulatae* (Chenpi), *Cinnamomi Cortex* (Rougui), and *Poria* (Fuling). Cold-type distension arises from inherent coldness of the Middle Jiao Qi, with cold stagnation and constraint; symptoms include absence of fever or thirst, cyanosis of the face and lips, cold extremities, asthma, and abdominal distension. Initially, *Jieyuan Wan* should be used to eliminate stagnation, followed by *Yigong San* to regulate the Spleen and Stomach. Excess-type distension involves food retention already present in the abdomen, or food bound in the Middle Jiao due to overeating. External signs include heat at the epigastrium, chest, and whole body, thirst, lethargy, unwillingness to speak, and closed eyes. If mistakenly diagnosed as Slow Convulsion (Man Jing Feng) and treated with calming Spirit and resolving Phlegm herbs, death is inevitable. One must recognize that "great excess may present with deficiency-like signs," and urgent purgation is required using *Hangxie Dan* and *Sanxian Dan* from *Jicheng*, administered together. Heat-type distension results from Shaoyang/Shaoyin heat pathogens entering the interior, causing constipation, scanty dark urine, high fever, facial redness, and irritability; treat with *Hangxie Dan*. Another cause is generalized sores and scabies where washing and topical applications force toxins inward, leading to abdominal distension; mild cases use *Jingfang Baidu San*, severe cases use *Hangxie Dan*. If sores erupt and distension subsides, it is auspicious; if sores do not erupt, it is ominous. Formulas are detailed under the section "Dysentery."
For treating Qi distension and Water distension: Use two sheng of dry ram feces, stir-fry until scorched yellow, remove fire toxicity, and grind into fine powder. Pour three sheng of boiling water over the powder to extract the juice. Take one zhan per dose, mixed with five fen each of *Costus Root* (Muxiang) and *Areca Seed* (Binglang) powders. This is a unique and effective formula for middle-fullness and蛊 distension.
For treating Food distension and Qi distension: Take one liang of *Brassicae Semina* (Luobozzi/Radish seed), grind into fine powder, mix with water, filter, and take the juice. Take one liang of *Amomi Fructus* (Sharen), soak overnight in Radish seed juice, stir-fry until dry, repeat soaking and frying seven times, then grind into powder. Take one qian per dose, mixed with rice congee.
The face, eyes, and nails all appear yellow. There are two types of symptoms: The first is Yang Jaundice (Yang Huang), characterized by fever and irritability, restlessness, excessive hunger, hot painful urination, constipation, and a full, forceful pulse. This condition can result from external Wind-Damp invasion or internal dietary injury. Provided the child's Yuan Qi remains strong and the Spleen and Stomach are undamaged, clearing Fire and promoting urination is suitable. *Yinchen Wuling Yin* is the most stable choice, or *Weiling Tang* added with *Artemisiae Scopariae Herba* (Yinchen). If constipation and intense heat are present, *Hangxie Dan* is suitable (see Dysentery section above). The second is Yin Jaundice (Yin Huang), caused by depletion of Qi and Blood. Symptoms include preference for stillness over activity, preference for darkness over light, mental fatigue, faint speech, aversion to cold, poor appetite, limb weakness, difficult defecation, and turbid urine resembling gruel. This indicates Yang deficiency. The primary treatment is *Sijunzi Tang*, optionally adding one qian of *Zingiberis Rhizoma Preparatum* (Paojiang), or adding one qian each of *Dolichoris Semen* (Biandou) and *Angelicae Sinensis Radix* (Danggui).
