The art of bone setting relies entirely on the practitioner's manual skill, requiring the ability to distinguish between joint dislocation (joint dislocation) and bone fracture (bone fracture).
Shoulder dislocation is the easiest to identify; the humeral head (humeral head) protrudes upward.
The arm cannot be raised to the head, pain is unbearable, and there is a complete loss of strength.
If encountering this condition, with skilled manipulation, it can be resolved in an instant.
Have assistants straddle and hold the patient, ensuring the patient sits upright without leaning to either side.
One hand pulls down the forearm, while the other pushes the bone on the shoulder.
Simultaneously pulling with both hands allows the bone to naturally return into the glenoid cavity (glenoid fossa) and fill the space.
If this method yields no result, a bamboo tube should be placed under the shoulder.
Lift both ends slightly, raise both hands as if drawing a bow,
and apply a sudden, forceful pull; the pain during reduction will not be severe.
Elbow dislocation (elbow joint dislocation) is the most difficult to recognize; flexion makes extension difficult, and extension makes flexion difficult.
Compare with the healthy limb; if dislocated, the bone will naturally appear flexed.
Have someone grasp the upper arm while you straighten the forearm by traction.
Use skill when raising the arm, tilting it slightly inward.
Flex and extend it repeatedly, then release it for the patient to attempt movement independently.
If swelling persists preventing flexion and extension, the underlying mechanism must be analyzed carefully.
Frequently rotate the body inward and outward, ensuring no crepitus (bone friction sound) is felt at the joint.
Absence of sound and obstruction indicates a good outcome.
Wrist dislocation (wrist joint dislocation) is also hard to identify; observe the prominent bone to confirm.
If the prominent bone is deviated, the joint is displaced; if aligned, the joint is reduced.
This prominent bone is not the radial styloid process used for pulse diagnosis, but the bone rising from the base of the wrist joint.
The physician must observe clearly; treatment involves traction and straightening.
Lift the bone upward, frequently moving it up and down.
Extend the thumb, then extend the bone below the thumb.
Only then will the tendons and bones achieve harmony; full recovery requires half a month.
A fracture of the arm bone near the joint, suspended half an inch, resembles a fracture.
Treat with a thin cedar splint, binding firmly to keep it straight.
Do not bind too tightly near the hand, as constriction may cause further pathology.
Another type is the clavicle (clavicle), which lies horizontally near the shoulder connecting to the side of the throat.
Dislocation here has no socket to contain it; fractures are easier to repair.
Whether dislocated or fractured, ensure it is flat. Apply a thick layer of external medicine evenly.
Wrap with cotton padding, press flat with mulberry bark,
bind the shoulders using a foot strap, and secure the affected arm across the chest and heart area.
How to verify spinal dislocation? The vertebrae are uneven and not level.
Have the patient sit on the ground, support the chest with cloth to lift the upper body.
Once the upper body rises, the dislocation becomes visible; immediately use hands to push it flat.
For rib fractures, press with fingers and listen for sounds at the fracture site.
Apply binding immediately upon pressing, wrapping the body tightly with cotton padding.
Hip and femoral dislocations are hardest to assess; sitting calmly shows few major issues.
Support the healthy leg standing straight; lowering the affected leg reveals the deformity.
Hip dislocation often presents laterally, while femoral head displacement may be medial.
Have the patient lie supine and upright, lift both legs and extend them forcefully.
Press the inner ankle with your foot, pull, and then flex and extend again.
The patella (kneecap) lacks a true socket; after trauma, restoration is difficult.
Secure with medication and circular bandaging; even if sequelae remain, serious harm is avoided.
The bone below the kneecap rarely dislocates.
If it does, it can be treated: extend forcefully, then flex.
Rotate inward and outward to guide the bone back; it will naturally align when reduced.
Ankle dislocation (ankle joint dislocation) is the most difficult to treat; once displaced outward, reduction is hard.
Sequelae are common; only 30-40% recover well, while 60-70% suffer chronic issues.
Treatment relies on the physician's finger strength.
Frequently dorsiflex the foot and massage the tendons; use the thumb to counter-pressure.
Bind immediately upon reduction; apply medicated plaster.
If irreducible, it becomes a lifelong disability. Another condition is "blurred injury,"
where swelling and pain exist without bony displacement. Though minor, rapid healing is rare.
Closed fractures of the four limbs with intact skin are easier to manage if mid-shaft.
Gently traction and align; immobilize with external medicine and splints aligned with the joints.
For fractures near the joint, splints must cross the joint line.
Avoid crossing splints over curved joint edges to prevent stiffness.
If swelling and blood stasis occur, puncture with a silver needle and apply flesh-regenerating powder.
After six to seven days, if bindings loosen, replace with fresh medication and re-tighten.
Open fractures with bleeding and exposed bone are critical.
Gently reduce and apply flesh-regenerating powder followed by plaster.
In summer, expose the wound and apply sesame oil regularly to prevent maggot infestation.
Small bone fragments need not cause panic; treat patiently.
Most joints are treatable, except for the calvaria (skull cap), where fractures are untreatable.
The axis/atlas region (neck bones) deep within the head, if displaced, is extremely dangerous.
Mandibular dislocation is verified by drooling and misaligned lower teeth.
Symmetrical deviation indicates bilateral dislocation; asymmetry indicates unilateral.
Have an assistant hold the forehead from behind; the physician cups the jawbones with both hands.
Insert thumbs into the mouth, gripping firmly against the gum line.
Gently pull downward and backward, then lift upward to reduce.
A click sensation indicates reduction; pause briefly to check if the condyles have returned to the glenoid fossa (mandibular fossa).
Skill and intuition are key.
Unilateral dislocation is treated with fingers using similar methods.
Carelessness or lack of focus leads to failure.
This secret technique has been passed down for a century, lost in fire, now restored.
Though expressed in vernacular language, the transmission is sincere.
May future generations inherit it, benefiting others and themselves alike.
