两手肘腕骨骼,俗名胖KT(此处指肘关节及周围软组织)。
[Zhuzhen San (Jade Chastity Powder)]
Arisaema Cum Bile (Nanxing), Angelica Dahurica (Baizhi), Saposhnikovia Divaricata (Fangfeng), Gastrodia Elata (Tianma), Notopterygium Incisum (Qianghuo), and Typhonium Rhizoma (Baifuzi) are taken in equal parts, ground into powder. Applied externally to protect the skin and ward off wind; it can also be taken orally with two qian.
[Fengliu San (Romantic Wind Powder)]
Aquilaria Sinensis (Xiangjie, four liang), Dragon's Blood (Xuejie, two tael five mace), Sappan Wood (Sumu, two liang), Frankincense (Ruxiang, five mace), Myrrh (Moyao, three mace), Fossilized Bones (Longgu, two mace), Safflower (Honghua, one mace), Platycodon Root (Jiegeng, a small amount), Juncus Effusus (Dengxin, one handful), and Chicken Eggs (ten, boiled in vinegar with shell and down, sealed with yellow mud, and simmered over alternating mild and strong heat). Each ingredient is ground into a fine powder, mixed evenly, and ground again. Sprinkled dry to stop bleeding. After bleeding stops, if dryness and pain occur, apply with refined oil. If bleeding does not stop, apply Xuejie powder alone to stop it immediately.
The head is the gathering place of all Yang meridians. Injuries to the Taiyang region, fontanelle, or cranial bones are life-threatening. Separate the hair, locate the wound, cut the hair near the injury, then apply medicine. If blood gushes out, chew Juncus Effusus into a ball, dip it in Taohua San, and plug the wound; bleeding will cease. For minor injuries, this is unnecessary. If the wound becomes foul and rotting, first decoct Xiaofeng San for oral administration, then decoct Xinxiang San for washing. Avoid drafts during washing to prevent exacerbation by cold-heat pathogens. If the face and head swell, it indicates wind entering the interior; take Xiaofeng San. For swelling at the site, mix Shenshen San with honey, or apply with ginger juice and wine. If marrow flows out, apply Ansui San mixed with clear tea. Using both medicines together is excellent. For depressed cranial bones, apply Baijin San mixed with Huaiwu San to lift the bone instantly, combined with oral medication for efficacy.
The face has seven orifices, with eyes being paramount. Treatment must be meticulous. If the eyeball protrudes from the eyelid, push it back while warm, apply Shenshen San to reduce blood stasis and swelling, and continue internal medication. If the cornea ruptures and fluid leaks, vision will be lost. If the eyelid turns inward, gently flip it back and apply Shenshen San. If blood invades the eye, use tender branches of peach and willow, Rehmannia Glutinosa (Shengdihuang), and Pheretima (Dilong) decocted in water to soak lean pork leg meat, then apply to the eye (secret method). Regularly take Huoxue Zhutong San and formulas for clearing the head and face; other cases follow standard trauma protocols with bleeding.
For dislocated jawbone, have the patient sit still, rub the area hundreds of times, and open the mouth. The practitioner inserts thumbs into the mouth, holds firmly, pulls down, then pushes up to reduce the joint. Secure with a cloth under the chin; remove after a short time.
For injured alveolar bone, palpate and manipulate to realign, apply Shenshen San, and secure the lower jaw with a silk handkerchief tied to the top of the head. Remove fallen teeth; straighten loose ones with chopsticks. For persistent bleeding, rinse with decoction of Chinese Gall (Wubeizi) and Alum (Baifan). Swallow Baijin San mixed with rice water, or pack with Taohua San.
For neck retraction due to fall from height, administer Xiaofeng San or Zhutong San with Biyao Hunhun San. Have the patient lie supine, secure the chin with a silk belt to the crown, gather hair with the belt, and ensure the head lies flat. The practitioner sits on the ground, extends legs, places feet on the patient's shoulders, and applies gentle traction to realign. Apply Shenshen San mixed with ginger juice, leek juice, wine, and vinegar. Bind securely and take Xuntong Zhutong San daily.
For fractured clavicle (shoulder bar), administer Xiaofeng San, Zhutong San, and Biyao Hunhun San. Manipulate to realign, then apply Shenshen San mixed with honey. Use a section of bamboo, sized to the injury, split into two. Place one piece against the bone, fill the armpit with cotton, and bind from the armpit across the opposite shoulder with a silk belt. Take medication for recovery.
For fractured/dislocated clavicle, take Xiaofeng San, Zhutong San, and Biyao. Prepare splints. Wash the shoulder with Xinxiang San decoction using a cloth wrap to relax the joint. Have the patient lie on their side. An assistant stands before them, pulling the arm level with the shoulder, while pressing the胁 (flank) with a foot to extend the bone for reduction. The practitioner stands behind the shoulder, manipulates the bone into place, and tests mobility: arm should reach behind the head, pass the chest, and palm rest below the epigastrium without shaking. Apply Shenshen San mixed with ginger and leek juice. Cover with paper-wrapped fir bark, bind from the injured flank to the opposite shoulder with a silk belt. Pierce the bark for ventilation. Take Jianji Huoxue Xuntong San daily.
For scapular dislocation, where the lower segment acts as a pestle and upper body as a mortar, first take Zhutong San with Biyao. Prepare splints. Wash the area with Xinxiang San decoction to relax tendons and bones. For left-sided dislocation, have the patient lie down. An assistant sits by the left knee, bends the left leg to press the left flank, binds the elbow to the assistant's waist, supports the elbow, leans forward, arches back, and applies gentle traction to reduce the bone into the flank. If dislocated inward and stuck, have the patient lie sideways, use a footstool to clamp the instep for rotation, have an assistant sit on the stool, bind the elbow and thigh, suspend from the shoulder, press the flank with a foot, then lift the shoulder and elbow with gentle traction to reduce. Ensure the wrist is turned and tested: arm reaches behind the head, passes the chest, and reverses on the arm to confirm reduction. Apply Shenshen San. Bind from the injury to the opposite flank, and another strap from the flank to the shoulder, filling the armpit with cotton for stability. Take Xiaofeng San and Zhutong San daily.
For bilateral arm fractures or comminuted fractures, take Xiaofeng San, Zhutong San, and Biyao. Use three pieces of fir bark, stripped of rough outer layer, thinned to finger-width size, length adjusted to the injury. Wrap in cotton paper, secure with oil-soaked paper. Weave four strands of binding rope into a splint structure to allow ventilation and keep inner medicine dry. Soak in hot medicinal soup to relax tendons. Have the patient lie down, bind the elbow to the practitioner's waist, sit beside the knee, grasp the elbow, press the armpit with foot, arch back, and apply gentle traction to align the fracture. Palpate to ensure alignment. Apply Shenshen San mixed with ginger, leek juice, and vinegar onto oiled cloth. Apply external splints with appropriate tightness, sling the elbow under the neck, and encourage frequent flexion and extension to prevent tendon stiffness. Take Jianji Huoxue Zhutong San daily. If blisters form at splint ends, do not pop; apply Heishen San mixed with oil to resolve.
For elbow and wrist dislocations (colloquially "Pang KT"), take Zhutong San with Biyao. Soften tendons with medicinal soup. Have the patient lie supine, practitioner beside, bind arm to waist, press armpit with foot, grasp thigh, arch back, and apply traction to align. Press the wrist with the thumb, support the posterior aspect with four fingers, and support the bone interior with two fingers. Test flexion and extension of the humerus; if stable, reduction is achieved. Test both hands clapping together. Apply Shenshen San on oiled paper, add splints. The splint should be a wide fir bark piece covering the joint, extending to the mid-upper and lower arms. Carve a large hole opposite the joint for movement. Reinforce the sides with paper. Weave binding ropes: central large piece as main splint, two ropes at each end, remaining ropes woven into smaller splints for upper and lower segments. Bind upper splint to upper arm, lower to lower arm, leaving two fen space at the wrist to avoid collision. Allow free flexion. Take Huoxue Zhutong San daily.
Fractured wrist bones are difficult to treat; avoid overly cooling medicines. Do not keep slings hanging constantly; encourage frequent flexion and extension. When sitting, rest on a table; when lying, vary position. After three days, test mobility by reaching behind the head and turning hands on the back. Gradual testing ensures functional recovery. Even after three days, such movement is acceptable. Avoid overly cooling topical applications to prevent tendon coldness and stiffness. For comminuted dorsal hand fractures, take medication as above. Have patient lie supine, practitioner sit by knee, press armpit with foot, grasp middle three fingers, apply traction, and align with other hand. Apply dressing. Add splint: large fir bark piece covering the dorsum, length from mid-arm to knuckle, with a horizontal hole at the wrist for movement. Add smaller fir bark pieces (finger-width) from arm to palm edge. Two side pieces slightly longer, wrapped in paper. Weave five rope strands: two for the large dorsal splint and side splints, one for the base of the thumb. Secure firmly but loosely on the outside. Sling under neck. After three days, test mobility and take medication.
For crushed palm bones and flesh, take medication as above, realign, apply Baijin Shengji San mixed with sesame oil, and Shenshen San mixed with honey. Wrap with paper, press a large fir bark piece on the palm, cover with paper-wrapped soft bamboo leaf on the dorsum, and bind with cloth. Medication may be omitted.
For broken finger bones, realign tendons and skin, stop bleeding with Taohua San. Wrap the finger in a large soft bamboo leaf, secured with paper. Apply Baijin Shengji San mixed with sesame oil on the leaf, wrap the finger, and bind with silk. Re-moisten with sesame oil when dry the next day. After three days, reapply Baijin Shengji San. Continue taking Huoxue Zhutong San, or apply Shenshen San mixed with honey.
For shoulder dislocation, if the left arm is dislocated, the practitioner crosses the right hand over the left. If the right arm is dislocated, cross the left hand over the right. Kneel on the flank, use a belt for traction. If the bone is upward, support it with the hand. Relax the arm until it aligns with the head and shoulder, then apply dressing. Stuff paper blocks in the armpit. Use two belts: one from the healthy shoulder to the injured armpit, and one from the injured armpit to the healthy shoulder. Take Zhutong San daily for stabilization.
For fractured lumbar or spinal bones, have the patient lie prone on a bench. Place a large rice-pounding hammer under the abdomen. Bind the scapulae to the head of the bench and the feet to the crossbar of the bench legs to curve the waist, lifting the fracture for easier reduction. Use a curved carrying pole aligned with the spine to press the fracture down gently into alignment. Apply Shenshen San, cover with paper-wrapped fir bark, and bind tightly with a warming abdominal binder. Take Jianji Huoxue Zhutong San daily.
For fractured ribs, no splinting is needed; take Jianji Zhutong San daily.
For hip joint (Huantiao) dislocation, this is the most difficult condition. Shortening of the leg is easier to treat than lengthening. Take Jianji Huoxue Zhutong San daily.
For fractured femurs/tibias, take medication as above. Have the patient lie supine, bind chest and armpits to the bench head. If the right leg is injured, extend the left leg and flex the right leg vertically. The practitioner stands by the bench edge, lifts the right foot, presses the right buttock with the foot, while an assistant ties a belt around the right tibia and pulls from the head of the bench to apply traction. The practitioner palpates and reduces the bone, holds it steady, extends the leg to check alignment, applies dressing, and splints. Take Jianji Huoxue Zhutong San daily.
For comminuted or dislocated kneecaps, take medication as above. Use a bamboo hoop sized to fit the kneecap, wrapped in silk, with two straps. Have the patient lie supine with legs extended. Palpate and align the bone, secure the hoop over the kneecap without untying. Apply Shenshen San inside the hoop, wrap with straw paper to prevent soiling. Take Huoxue Zhutong San daily.
For comminuted tibial fractures, treatment is similar to femoral fractures.
For dislocated ankles and feet causing limping, take medication as above. Use two large fir bark pieces, length from calf to sole, with a round hole opposite the ankle to encircle it. Use one large piece to support the sole, from toes to behind the heel, leaving side margins. Use three to four small pieces (finger-width) woven into a splint cage for the front of the tendons/bones. Oil and wrap all bark pieces in paper. Weave binding ropes: two above the ankle, two below the foot. Traction and reduce first, then splint. Use cloth to support the forefoot, tie under the knee to prevent hyperextension, and encourage frequent flexion/extension. Take Huoxue Zhutong San daily.
For broken toes, the method is the same as for fingers. The following formulas are listed subsequently.
