[Verse] Headwind (Toufeng) is inherently caused by heat and phlegm, suddenly invaded by wind-cold into the brain marrow. Why do women suffer from this disease more often? It is because they lack head coverings to ward off wind and cold.
[Discussion] "Headwind" (Toufeng) is also known as "Shoufeng" (Wind of the Head). Sun Simiao stated: Exposing oneself to wind immediately after bathing causes Shoufeng. The Classics state: The symptoms of Shoufeng include excessive sweating on the head and face, aversion to cold, and worsening of the condition one day before windy weather; the headache is so severe that one cannot go out or stay indoors. On the day of the wind, the condition improves slightly. It is also stated that Toufeng originates from wind-cold invading the brain marrow. Since the head is the confluence of all Yang meridians, if a person has pre-existing phlegm-fire in their body, and wind-cold external pathogens lodge within, the heat becomes stagnant causing dull pain. Therefore, women are prone to this disease due to the lack of head coverings to resist wind and cold. Toufeng can manifest as unilateral or central pain. Zhu Danxi stated: Most cases of Toufeng are related to phlegm, accompanied by heat, wind, or blood deficiency. Pain on the left side indicates wind and blood deficiency; pain on the right side indicates phlegm and heat. Skilled practitioners must investigate the pathology of this condition, relying entirely on flexible methods and appropriate modifications of prescriptions, achieving efficacy without fail.
Wind and Blood Deficiency Type
When Toufeng pain is on the left, it belongs to wind and blood deficiency. For wind, herbs such as Jingjie (Schizonepetae Herba) and Bohe (Menthae Haplocalycis Herba) are used. For wind-heat, Xiaofeng San (Eliminate Wind Powder) or Chatiao San (Tea-Powdering Powder) are used. For blood deficiency, Xionggui Tang (Ligusticum and Angelica Decoction) or Siwu Tang (Four Substances Decoction) added with Jingjie, Fangfeng (Saposhnikoviae Radix), Baizhi (Angelicae Dahuricae Radix), Jiuhuangqin (Scutellariae Radix fried with wine), Bohe, and Manjingzi (Vittariae Spora) are used. (All the above formulas are listed under the section on Headache.)
Phlegm-Dampness Type
When Toufeng pain is on the right, it belongs to phlegm-dampness, treated with herbs such as Cangzhu (Atractylodis Rhizoma) and Banxia (Pinelliae Rhizoma). For headwind caused by damp-phlegm, use Jiuhuangqin (3 qian), Cangzhu (4 qian), Chuanxiong (2 qian), Xixin (Asari Radix et Rhizoma, 2 qian), and Gancao (Glycyrrhizae Radix et Rhizoma, 1 qian), grind into powder, mix evenly with pounded ginger and tea, and take with water. Alternatively, use Erchen Tang (Two Aged Things Decoction) added with Jiuhuangqin, Bohe, Chuanxiong, and Xixin; or Daotan Tang (Guiding Phlegm Decoction) added with Jiuhuangqin and Cangzhu. (The above two formulas are listed under the section on Phlegm.)
Fire Type
When Toufeng belongs to fire, Jiuhuangqin is the main herb. Alternatively, use Erchen Tang added with Jiuhuangin, Bohe, Chuanxiong, and Xixin (formula see Phlegm section); or Fangfeng Tongsheng San (Wind-Barrier Unblocking Strength Powder) (formula see Headache section); or Xiaofeng Baijie San (Eliminate Wind Hundred-Release Powder). Whenever Huangqin (Scutellariae Radix) and Lian (Coptidis Rhizoma) are used, they must be fried with wine, as they have an ascending action to the temples and head.
Xiaofeng Baijie San
Treats unilateral headwind pain caused by upward rushing of fire-heat.
