Erythema (macules and papules) in cold damage is caused by excessive Yang qi. The pathogen is at the exterior level. It may be that sweating has not yet been induced, or it has already been induced, or after emesis and purgation, the evil heat has not been eliminated. The toxic gas accumulates internally. Due to deficiency of the exterior, the heat-toxin takes advantage of the deficiency to emerge into the skin, manifesting as erythema and rashes, resembling brocade patterns. If the color is red and appears within five days, it is treatable. If the color is black and appears after seven days, it is difficult to treat. In severe cases, ulcers form on the throat, tongue, and body.
The verse states: Warm toxin causing erythema scorches the muscles; one dose of Shigao (Gypsum Fibrosum) eliminates the toxin. If black toxin emerges due to deficiency, there are other specific treatments for rashes, bean-like eruptions, and sores.
Generally speaking, erythema should not be treated with exterior-releasing herbs. This is because the exterior is deficient while the interior is excess. Forcing sweating would further open the pores, leading to ulceration of the chest and abdomen. For those with sore throat, Xuanshen Shengma Tang (Scrophulariae Radix and Cimicifugae Rhizoma Decoction) is used. For those where the rash has already emerged, Daqing Siwu Tang (Prunellae Spica, Lithospermi Radix, Bupleuri Radix, and Rehmanniae Radix Decoction), also known as Ejiao Daqing Tang, is used. Zhudan Jizi Tang (Pig Gallbladder and Chicken Egg Decoction) can also be used. Recently, Shengma Gegen Tang (Cimicifugae Rhizoma and Puerariae Radix Decoction) is most commonly used and yields excellent results. For sores in the lower body, diarrhea, vomiting of green fluid, red eyes, and mouth ulcers, Huanglian Jupi Tang (Coptidis Rhizoma and Citri Reticulatae Pericarpium Decoction) is indicated. For pea-sized eruptions, a decoction of Huanglian (Coptidis Rhizoma) alone, boiled thick, is taken orally with great effect. Dadingdai (Indigo Naturalis), sized like a date, can be swallowed. Alternatively, Qingmuxiang (Aucklandiae Radix) decoction can be taken. For pain and irritability upon eruption, Wuwu Muxiang Tang (Five-Ingredient Aucklandia Decoction) or Xijiao Daqing Tang (Rhinoceros Horn and Prunella Spica Decoction) is used. Mangxiao (Mirabilite) and pig gallbladder juice can be applied topically to the sores. According to *Zhengzhi Lun*, Huaban Tang (Macule-Eliminating Decoction) is another name for Baihu Jia Renshen Tang (White Tiger Decoction plus Ginseng). Sun Zhao uses Zixue (Purple Snow). For patients with fu-organ excess syndrome, Tiaowei Chengqi Tang (Regulating the Stomach and Restoring Order Decoction) is used.
It is also said: Warm toxin causes the whole body to resemble brocade patterns, with erythema and urticaria accompanied by frequent vomiting. Sometimes there is cough, counterflow of qi, irritability, and stifling sensation. This occurs when cold is contracted in winter but manifests in spring.
For warm toxin-induced erythema, Heigao (Black Plaster) is used. For vomiting of green fluid, Gegen Jupi Tang (Puerariae Radix and Citri Reticulatae Pericarpium Decoction) is indicated.
When heat disease is at the exterior and remains unresolved after sweating, or after emesis and purgation, the heat-toxin does not dissipate, leading to irritability and delirious speech. This indicates exterior deficiency and interior excess, with heat and dryness pressing outward, causing the body to develop erythema resembling brocade patterns. In all cases of erythema, exterior-releasing herbs must not be used, as they will cause the sores to open and worsen the ulceration due to exterior deficiency.
Another verse states: When blood is affected by warm toxin, Heigao is effective; for heat disease with double Yang, there is a separate decoction. Erythema arises from interior excess and exterior deficiency; inducing sweating will only increase the sores.
The syndrome belongs to Yang. Misapplication of warming herbs, or failure to induce sweating when indicated, or failure to purge when indicated, or persistence of the disease after sweating and purging, leads to internal burning of Yang heat, steaming and forcing outward. Heat-toxin enters the stomach, causing erythema. Heat inevitably injures the blood; if blood heat does not dissipate, resulting in interior excess and exterior deficiency, the heat qi takes advantage of the deficiency to emerge into the skin. Mildly, it resembles rashes; severely, it resembles brocade patterns. Those treating erythema must strictly avoid sweating, as sweating reopens the pores and increases the number of macules. Whenever one or two spots appear, treatment must be sought early. Delay allows the condition to progress until isolated Yang fails and Yin is exhausted, making it incurable. Black erythema indicates extreme heat and gastric rot, which is beyond remedy. However, early erythema resembles mosquito bites, so medication must not be misapplied. Erythema mostly appears on the chest and abdomen, while mosquito marks appear on the hands and feet. A surging and large Yang pulse at the cun position, combined with confusion and disorientation, turning from red to crimson initially, indicates erythema. A non-surging Yang pulse, quiet patient, turning from red to yellow initially, indicates mosquito marks. Erythema belongs to Yang and Yang toxicity. If the Yin pulse is excessively large, with spontaneous diarrhea, or oppression and shortness of breath with restlessness and prolonged constipation, even Lu Bian reborn could not apply his skill. Warm toxin erythema occurs when stomach cold in winter transforms into Yang excess in spring, triggering the disease. After emesis and purgation, if the exterior is not resolved and the toxin is not cleared, Heigao is the primary treatment. For muscle ulceration, cough, chest stuffiness, diarrhea, vomiting of clear fluid, muttering (Zhengsheng), and mouth ulcers, Huanglian Jupi Tang or Gegen Jupi Tang is suitable. For heat disease erythema and epidemic erythema, Daqing Siwu Tang or Zhudan Jizi Tang is used. For Yang toxicity, termed Double Yang, with erythema and sore throat, Xuanshen Shengma Tang is used. Shengma Gegen Tang, Baidu San (Toxin-Resolving Powder), and Xijiao Dihuang Tang (Rhinoceros Horn and Rehmanniae Decoction) are generally used for erythema. For greater heat, Xuanshen Shengma Tang is used with added Shengjiang (Fresh Ginger) and Wumei (Mume Fruit). Huanglian Yiwu Tang (Single-Ingredient Coptidis Decoction) or Qingmuxiang Yiwu Tang (Single-Ingredient Aucklandia Decoction) can be selected. For fu-organ excess, a small amount of Tiaowei Chengqi Tang is given. Sun Zhao uses one dose of Zixue. *Zhengzhi Lun* recommends Baihu Jia Renshen Tang or Faban Tang (Erythema-Decoction) with Zicao (Arnebiae Radix et Rhizoma) and Chuanxiong (Ligustici Chuanxiong Rhizoma) as assistants. For internal binding of blood heat, Xiao Chaihu Tang (Minor Bupleurum Decoction) is used. Small, sparse spots with fresh, fine color are easy to treat. Brocade-like eruptions that collapse are difficult to treat. Initial red color fading to tiny spots, followed by darkening of the face and muscles to black, is incurable. Initial appearance resembling black moles is also incurable. Red erythema has a mortality rate of five deaths to one survival; black erythema has ten deaths. Internal and external blazing heat, unresolved by sweating or purging, accompanied by irritability, cough, vomiting, cold feet, and deafness, constitutes the syndrome of erythema.
