而来，自由大肠直出，岂有归大肠而不出，反由漏口徐徐而泄哉。是以痔贵早为培补，益气保元，不可用苦寒内
服外涂淋洗。病者谨戒百日醇酒房劳，再无不收口之理。若不遵禁忌，虽有灵丹，亦难奏效。（《疡医大全》）
宽肠泻热方法 治肠痔，大便常血方。
大麻仁 大黄 枳壳（炙）
上各二两，捣筛蜜和为丸，每日空心芸苔汤下十丸。（《千金月令》）

Various Formulas for Cooling Blood and Regulating Blood: Qin Jiao Dang Gui Tang (Gentian and Angelica Decoction), for treating hemorrhoidal fistulas with dry stools and pain.
Rhubarb (roasted 4 qian), Gentiana Macrophylla Root (Qin Jiao), Immature Bitter Orange (Zhi Shi, each 1 qian), Alisma Tuber, Dang Gui Tip, Soap Pod Kernel, Atractylodes Macrocephala Rhizome (each 0.5 qian), Safflower (a little), Peach Kernel (20 pieces).
Combine as one dose. Decoct in three bowls of water until reduced to one bowl. Strain and take hot before meals. Avoid cold wind locations and straining during defecation/urination. (*From "Lan Shi Mi Cang"*) Another variation, Qin Jiao Cang Zhu Tang, removes Zhi Shi, Bai Zhu, and Hong Hua from the above formula, adding Atractylodes Lancea Rhizome, Saposhnikovia Root, Phellodendron Bark, and Areca Seed.
Zhu Yu Tang (Blood-Stasis Expelling Decoction): Promotes free flow of large and small intestines, expelling black matter.
Ligusticum Chuanxiong Rhizome, Angelica Dahurica Root, Raw Rehmannia Root, Red Peony Root, Trogopterus Dung, Prepared Aurantium Fructus, Donkey-Hide Gelatin (fried), Curcuma Zedoaria Rhizome (roasted), Poria Cocos, Poria Cocos with Hostwood, Large Akebia Caulis, Raw Licorice Root (each 1 fen), Solid Rhubarb, Peach Kernel (soaked, peeled, baked, each 1.5 fen).
Cut into coarse powder. Take three qian per dose. Decoct with one well-water bowl, three slices of ginger, and three spoons of honey. Take until bowel movement occurs. This is a general formula for pain caused by blood stasis. Xiang Qiao Wan (Costus and Aurantium Pill): For treating severe internal damp-heat leading to hemorrhoids due to overeating, which persists into fistulas. Take urgently for quick cure.
Costus Root, Phellodendron Bark (each 3 qian), Prepared Aurantium Fructus (pith removed, stir-fried), Magnolia Bark (half liang), Coptis Rhizome (one liang), Pig Skin (one, burned to retain character), Dang Gui (4 qian), Schizonepeta Spike (3 qian).
Grind into powder, make pills with flour paste, size of Sophora seeds. Take 20-30 pills per dose, washed down with warm water before meals, three times daily. (*From "Yi Xue Fa Ming"* [Note: This formula is slightly mixed, recorded here tentatively])
Huang Lian San (Coptis Powder): For patients with existing hemorrhoidal fistulas who develop a lump near the anus, thick-skinned, swollen, painful, suppurating and draining through the hemorrhoid hole. Treat as food stagnation descending.
Coptis Rhizome, Ferula Resin, Medicated Leaven, Hawthorn Fruit, Peach Kernel, Forsythia Fruit, Sophora Fruit, Rhino Horn (equal parts).

Si Wu Tang (Four Substance Decoction) added with Scutellaria Root, Phellodendron Bark, and Sophora Flower. Main formula for hemorrhoidal fistulas with bleeding due to internal heat. (*From "Fang Kao"*)
Experience Formula Hei Wan Zi (Black Pills): Specifically for long-standing hemorrhoidal fistulas with bleeding, repeatedly proven effective.
Dried Ginger, Carbonized Rice Dust (one liang), Ficus Hirta Inflorescence (two liang), Dried Plum, Rotten Palm Fiber, Arborvitae Leaf, Oiled Hair (each five qian. The above seven ingredients are burned to ash retaining character. Then add the following.) Cinnamon Twig (three qian), Angelica Dahurica Root (five qian, none exposed to fire).
Grind nine ingredients into powder, make pills with vinegar paste, size of Sophora seeds. Take thirty pills on an empty stomach with rice drink. (*From "Pu Ji Fang/Da Cheng"*)
Zhu Danxi stated that for hemorrhoids unhealed for 20-30 years, three doses of Lian Hua Rui San (Lotus Stamen Powder) can stop them.
Lotus Stamen, Black Morning Glory Seed Head Powder (each one and a half liang), Dang Gui (half liang), Alum Red (a little).
Grind into powder. Abstain from meat for 5-7 days first. Eat one meal of meat on an empty stomach, then take three qian washed down with warm wine. After about two hours (liang shi chen), expelling pus, blood, or worms indicates efficacy. Rhino Horn, Ivory, Frankincense, Myrrh (each one liang), Alum, Yellow Beeswax (each five qian).
Melt beeswax in a copper vessel, add medicines, make pills size of Sophora seeds. Decoct Forsythia Fruit and Honeysuckle Flower in good wine for half a day, strain, and take twenty-one pills washed down with wine for self-healing. (*From "Yang Yi Da Quan"*)
(Another method: Two jin of ivory dust, ground extremely fine. Each morning, dip a cooked egg in the powder and eat it, or mix into thin porridge. Finish one course for recovery.)
Formulas for Drawing Out Toxin: Huai Jiao Zi Wan (Sophora Seed Pill): For treating hemorrhoids formed by wind excess and solid heat, presenting as rat-milk nodules with pain.
Soap Pod (roasted in rice vinegar to purple-black), Astragalus Root (roasted), Schizonepeta Spike, Sophora Seed, Pangolin Scale (fried in clam powder to yellow), Costus Root, Wasp Nest (burned to retain character), Cortex Phellodendri (burned to retain character), Turtle Shell (vinegar-roasted), Platycodon Root (remove radicle), Red Peony Root, Rhubarb.
Equal parts of all ingredients. Grind into fine powder, make pills with refined honey, size of Sophora seeds. Take fifty pills per dose, washed down with warm boiled water before meals. (*From "Yang Shi Fang"*)
Hu Lian Zhui Du Wan (Anabasis Pill for Pursuing Toxin): For treating hemorrhoidal fistulas, regardless of duration, where fistulas connect to the intestine and discharge filth. Use this pill first to exhaust all pus and toxin, followed by other pills for stable efficacy.
Anabasis Rhizome (one liang, sliced, stir-fried with ginger juice), Cortex Phellodendri (one liang, roasted, sliced, stir-fried yellow, ground), Musk (2 fen).
Make pills with soft rice, size of hemp seeds. Take one qian per dose, washed down with wine before meals. Increased pus/water discharge after medication indicates drug action reaching the site; do not fear. (*From "Wai Ke Zheng Zong"*)
Huang Lian Bi Guan Wan (Coptis Closing Tube Pill).
Anabasis Rhizome (pure powder one liang), Pangolin Scale (boiled in sesame oil to yellow), Halimodendron Gypsum (calcined), Sophora Flower (lightly stir-fried, powder five qian each).
Make pills with refined honey, size of hemp seeds. Take one qian per dose, washed down with clear rice soup on an empty stomach. Twice daily, morning and evening. Severe cases heal in forty days. This formula uses no needles, knives, or hanging threads, sparing the patient suffering; it is indeed an excellent formula for curing chronic diseases. If hard flesh protrudes around the four sides of the fistula, add twenty fried silkworm cocoons (ground), mixed into the medicine. Effective for this and all bodily fistulas. (Same source)
Qi-Regulating Method: Ju Pi Tang (Tangerine Peel Decoction): For treating Qi-type hemorrhoids.
Tangerine Peel, Prepared Aurantium Fructus, Ligusticum Chuanxiong Rhizome, Sophora Flower (stir-fried, half liang each), Areca Seed, Costus Root, Peach Kernel (soaked, peeled, fried), Perilla Stem and Leaf, Cyperus Rhizome, Licorice Root (roasted, each 2.5 qian).
Cut into pieces. Take three qian per dose, decoct with ginger and jujube. (*From "Zhi Zhi Fang"*) Another formula, Jing Zhi Tang, for Qi-stagnation induced hemorrhoids, removes Tangerine Peel, Ligusticum, Areca Seed, Costus, and Peach Kernel from the above formula, adding Schizonepeta Spike (equal parts). Grind into powder, take two qian per dose, mixed with rice soup.
Tonifying and Regulating Formulas (Nourishing Yin): Modified Si Jun Zi Tang (Four Gentlemen Decoction): For treating five types of hemorrhoids with bleeding, sallow complexion, palpitations, tinnitus, weak legs, fatigue, bland taste, and loss of appetite.
Ginseng Radix, Poria Cocos, Atractylodes Macrocephala Rhizome, Licorice Root (roasted), Astragalus Root, White Hyacinth Bean (steamed, equal parts).
Grind into powder. Take two qian bi per dose, mixed with boiling water. People initially doubt this formula, but those who take it find it quite effective. It is said: "Though it may look unimpressive, it possesses miraculous powers." (*From "San Yin Fang"*)
Er Miao San (Two Marvels Powder): For treating hemorrhoids.
Si Jun Zi Tang (Four Gentlemen Decoction), Astragalus Root, Jian Zhong Tang (Establish the Middle Decoction).

A male patient suffered from hemorrhoids that swelled and pained upon exertion. Used Aurantium Fructus Soup for fumigation and washing, internally took Fang Feng Qin Jiao Tang (Wind-Dispersing and Blood-Nourishing Decoction). After several doses, swelling and pain decreased significantly. Advised him to frequently wash with the previous soup, 5-6 times monthly, while taking Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) added with Phellodendron Bark and Anemarrhena Rhizome. Avoid cold, pork, raw fish/vegetables, and sexual intercourse. Only consume dry white meat. After recovery, abstinence from sexual activity for one hundred days is permitted. When Qi and Blood sink downward, clashing to form hemorrhoids, sitting is impossible, walking is intolerable. Standing worsens the sinking sensation.
