Jaundice caused by plague (epidemic febrile disease) occurs only in the Yangming and Taiyin meridians. Yellow is the proper color of Earth. Since both these meridians belong to Earth, jaundice manifests. This is because the exterior cannot be resolved through sweating, and the interior cannot be drained through urination; the Earth element of the Spleen and Stomach is steamed by heat, similar to the fermentation process of making qu (starter for brewing), thus manifesting externally as yellow. If urination is smooth, heat does not accumulate internally, so jaundice cannot develop.
Jaundice occurring in other meridians also results from the Earth element of the Spleen and Stomach being affected by pathogens, but the shades of yellow vary. Jaundice due to cold-dampness presents with a body color like that of cured tobacco, dark and dull. Jaundice due to excessive heat presents with a bright color like orange peel or Phellodendri Cortex (Huangbai), staining clothes upon sweating; this distinguishes them. The causes of jaundice are diverse. There is jaundice caused by blood stasis accumulating in the Lower Jiao; jaundice caused by damp-heat stagnating internally; jaundice caused by cold-dampness; jaundice transformed into Yin Jaundice due to excessive purgation; and Yin Jaundice caused by dampness within the Taiyin Meridian without prior purgation. However, jaundice specifically caused by plague is limited to only two categories: damp-heat and blood stasis.
For jaundice due to stasis-heat, the pulse is floating, slippery, firm, and rapid; symptoms include sweating only on the head up to the neck, slight abdominal fullness, difficult urination, and thirst. For jaundice due to blood stasis, the pulse is faint, deep, or knotted; the patient exhibits manic behavior, tightness and hardness in the lower abdomen, free urination, and black stools. As for jaundice accompanied by a body color like cured tobacco, fixed staring gaze, shaking of the head, cold air issuing from the nose, and blackening around the mouth, these are all untreatable conditions.
In various manifestations of Shanghan (Cold Damage) and plague, concurrent symptoms are common, but it is rare for macules (ban) and jaundice (huang) to appear simultaneously. My cousin, Bingqin, suffered from jaundice which was quickly followed by the eruption of macules. I examined him and found the situation quite startling. Given his constitution was inherently deficient, I prescribed Tuo Li Ju Ban Tang (Support Interior Macule-Elevating Decoction) and Yinchen Wuling San (Artemisiae Scopariae Five-Ingredient Powder with Poria), selecting and modifying ingredients from both formulas to treat both macules and jaundice concurrently, hoping for efficacy. After one dose, he experienced chills and sweating the next morning, after which both macules and jaundice receded, and his condition cleared abruptly. Thus, I named this formula Ban Huang Shuang Jie San (Double-Resolving Macule and Jaundice Powder). It is recorded below for reference and has been expanded. If macules are severe and jaundice mild, treatment should focus primarily on macules and secondarily on jaundice; if jaundice is severe and macules mild, treatment should focus primarily on jaundice and secondarily on macules. Furthermore, there are cases where macules appear first followed by jaundice, jaundice appears first followed by macules, or jaundice occurs with rashes. The presentations of macules and jaundice vary, and their ingenious treatments differ accordingly. Synthesizing and analyzing to exhaust their variations, and navigating complexities to master their subtleties, allowing for flexible application, depends on the practitioner's ability to adapt to the circumstances.
