(Second Case)
A woman injured her foot in early winter, presenting with blood loss, sweating on the head, internal heat with thirst, shortness of breath, irritability, and occasional fainting or confusion. Her pulse was large and forceful (Hong Da) but weak upon deep pressure.
This condition arises from deficiency of Yin-blood in the lower jiao, causing solitary Yang to flare upwards, thus leading to cold extremities (Jue) and sweating on the head. Initially, Si Wu Tang (Four Substance Decoction) combined with Xiao Chai Hu Tang (Minor Bupleurum Decoction) was administered for one dose, after which the sweating ceased. Subsequently, Si Wu Tang (Four Substance Decoction) was modified by removing Chuanxiong Rhizome (Chuan Xiong), adding Ginseng Radix et Rhizoma (Ren Shen), Atractylodis Macrocephalae Rhizoma (Bai Zhu), Ophiopogonis Radix (Mai Men Dong), Schisandrae Fructus (Wu Wei Zi), and Glycyrrhizae Radix et Rhizoma Preparata (Zhi Cao), with a small amount of Cinnamomi Cortex (Rou Gui). After four doses, all symptoms disappeared. Following more than thirty additional doses, Qi and Blood were restored, and she recovered.
A man fractured his leg in early summer, suffering from excessive bleeding. Initially, he experienced dizziness and blurred vision, followed by fainting and confusion. This resulted from damage to Yin-blood and intense flourishing of Yang-Fire, where the Metal element failed to control Wood, causing Wood to become excessive and generate Wind. Emergency treatment involved administering fresh urine (Tong Bian). Subsequently, a decoction containing Ginseng Radix et Rhizoma (Ren Shen) and Angelicae Sinensis Radix (Dang Gui) each at 5 qian, Schizonepetae Spica (Jing Jie), Chuanxiong Rhizome (Chuan Xiong), Bupleuri Radix (Chai Hu), Paeoniae Radix (Shao Yao), and Atractylodis Macrocephalae Rhizoma (Bai Zhu) each at 2 qian, Gardeniae Fructus (Shan Zhi), Scutellariae Radix (Huang Qin), and Platycodonis Radix (Jie Geng) each at 1 qian, and Glycyrrhizae Radix et Rhizoma (Gan Cao) at 0.5 qian, was given, bringing immediate relief. Further treatment used Si Wu Tang (Four Substance Decoction), Ginseng Radix et Rhizoma (Ren Shen), and Atractylodis Macrocephalae Rhizoma (Bai Zhu) each at 3 qian, Rehmanniae Radix Preparata (Sheng Di), and Bupleuri Radix (Chai Hu) each at 1 qian. After four doses, irritability completely resolved.
(Third Case)
Shen Lizhai, Grand Secretary, suffered a lumbar sprain with pain in early winter, accompanied by obstructed phlegm-Qi in the chest. He was treated with Aurantii Fructus (Zhi Qiao), Citri Reticulatae Pericarpium (Qing Pi), Bupleuri Radix (Chai Hu), Cimicifugae Rhizoma (Sheng Ma), Aucklandiae Radix (Mu Xiang), Foeniculi Fructus (Hui Xiang), Angelicae Sinensis Radix (Dang Gui), Chuanxiong Rhizome (Chuan Xiong), Paeoniae Radix Rubra (Chi Shao), Moutan Cortex (Shen Qu - *Note: Shen Qu is typically Medicated Leaven, likely a typo in original text or specific context, translating as Medicinal Leaven*), and Carthami Flos (Hong Hua). After four doses, he recovered. However, he had poor appetite and slight tidal fever. He was then treated with Ginseng Radix et Rhizoma (Ren Shen), Atractylodis Macrocephalae Rhizoma (Bai Zhu), Chenpi (Citri Reticulatae Pericarpium), and White Peony Root (Bai Shao) each at 1 qian, Dang Gui Shen (Angelicae Sinensis Radix Head) at 2 qian, Chuanxiong Rhizome (Chuan Xiong) at 0.8 fen, Soft Bupleuri Radix (Ruan Chai Hu), Phellodendri Cortex (Di Gu Pi - *Note: Di Gu Pi usually refers to Lycii Cortex, here likely meant Phellodendri or similar cooling herb, but strictly following text "Di Gu" often implies Di Gu Pi/Lycii Cortex or Phellodendri depending on context, WHO standard for Di Gu Pi is Lycii Cortex*), and Glycyrrhizae Radix et Rhizoma Preparata (Zhi Cao) each at 0.5 fen. After more than ten doses, he was healthy.
Liu Shangbao, who was obese, suffered a sprain in the buttocks with pain. After taking Tou Gu Dan (Bone-Penetrating Pill), his joint pains worsened, particularly in the lower body. He was treated with Er Chen Tang (Two-Cured Decoction), Pinelliae Rhizoma Preparatum (Nan Xing), Notopterygii Rhizoma et Radix (Qiang Huo), Saposhnikoviae Radix (Fang Feng), Achyranthis Bidentatae Radix (Niu Xi), Chaenomelis Fructus (Mu Gua), Atractylodis Rhizoma (Cang Zhu), Scutellariae Radix (Huang Qin), and Phellodendri Cortex (Huang Bai). His body pain subsided. The previous formula was modified by adding Dang Gui Wei (Angelicae Sinensis Radix Tail) and Chi Shao (Paeoniae Radix Rubra), along with Jie Geng (Platycodonis Radix), resulting in full recovery.
Zheng Libu, who had chronic damp-phlegm, fell from a horse in early winter. After taking acrid-warm blood-breaking herbs, he developed generalized pain, fever, dry mouth, and a large, slippery pulse. This was caused by hot herbs agitating phlegm-fire. Treatment involved Qing Zao Tang (Clear Dryness Decoction) with Ren Shen (Ginseng Radix et Rhizoma), Dang Gui (Angelicae Sinensis Radix), and Huang Qi (Astragali Radix) removed, and Huang Qin (Scutellariae Radix), Shan Zhi (Gardeniae Fructus), Ban Xia (Pinelliae Rhizoma Preparatum), and Huang Bai (Phellodendri Cortex) added. The heat and pain immediately subsided, and the affected area improved slightly. Finally, Er Chen Tang (Two-Cured Decoction), Qiang Huo (Notopterygii Rhizoma et Radix), Jie Geng (Platycodonis Radix), Cang Zhu (Atractylodis Rhizoma), Huang Bai (Phellodendri Cortex), Jiang Zhi Sheng Di (Ginger-processed Rehmanniae Radix Preparata), and Dang Gui (Angelicae Sinensis Radix) were used, leading to complete recovery.
