A man suffered a scald injury to his leg from hot water while intoxicated, resulting in ulceration, fever, thirst with desire to drink. His pulse was surging (Hong), rapid (Shu), and forceful, indicating an affliction of Fire-Toxin. He was treated with Sheng Di Huang (Rehmanniae Radix), Dang Gui (Angelicae Sinensis Radix), Huang Qin (Scutellariae Radix), Huang Lian (Coptidis Rhizoma), Mu Tong (Akebiae Caulis), Ge Gen (Puerariae Lobatae Radix), and Gan Cao (Glycyrrhizae Radix et Rhizoma). After more than ten doses, all symptoms gradually subsided. Subsequently, he was prescribed Ren Shen (Ginseng Radix), [text missing, likely Huang Qi or similar Qi-tonic], Bai Zhu (Atractylodis Macrocephalae Rhizoma), Xiong Qiong (Chuanxiong Rhizoma), Dang Gui, Zhi Gan Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Shao Yao (Paeoniae Radix), Bai Zhi (Angelicae Dahuricae Radix), and Mu Gua (Chaenomelis Fructus) as the new flesh was nearly complete. Due to overexertion, he suddenly experienced alternating chills and fever, which was attributed to deficiency of Qi and Blood. The same formula containing Ren Shen was used again, augmented with Wu Wei Zi (Schisandrae Chinensis Fructus) and Suan Zao Ren (Ziziphi Spinosae Semen), bringing him relief. Over a month later, the wound healed.
Another man suffered burn injuries to both arms, experiencing pain and difficulty in urination and defecation. This indicated that the Fire-Toxin had transmitted to the Lower Jiao. He was treated with Sheng Di Huang, Dang Gui, Shao Yao, Huang Lian, Mu Tong, Shan Zhi (Gardeniae Fructus), Chi Fu Ling (Poria cum Rubrum), and Gan Cao. After one dose, urination and defecation became clear and smooth, and the pain ceased. Thereafter, Si Wu Tang (Four Substance Decoction) combined with Ren Shen, [text missing], Bai Zhi, and Gan Cao was administered. The necrotic tissue sloughed off, and after several more doses, new flesh grew.
A woman suffered a severe scald injury to her chest with extensive ulceration that failed to heal for two months. Her pulse was surging yet deficient, accompanied by dry mouth, fever, and worsening symptoms in the late afternoon (Ri Bu). This was diagnosed as Deficient Fire due to Yin-Blood deficiency, with Toxin taking advantage of the deficiency to cause affliction. She was treated with Si Wu Tang augmented with Chai Hu (Bupleuri Radix) and Dan Pi (Moutan Cortex), which reduced the fever and cooled the body. Subsequently, Xiao Yao San (Free and Easy Wanderer Powder) with added Chen Pi (Citri Reticulatae Pericarpium) was used to nourish Yin-Blood and strengthen the Spleen and Stomach, leading to the sloughing of necrotic tissue and growth of new flesh.
