A patient presented with blood stasis flowing and accumulating, resulting in black discoloration of the lower back and both feet. The patient was immediately given a decoction of child's urine (Tongbian) and wine, followed by scarification (Bian) to release stagnant blood and necrotic tissue. Xiao Chai Hu Tang (Minor Bupleurum Decoction) was administered with Banxia (Pinelliae Rhizoma) removed, and Zhizi (Gardeniae Fructus), Huangqin (Scutellariae Radix), and Lianzi (Coptidis Rhizoma) added to clear Liver fire; subsequently, Bazhen Tang (Eight Treasure Decoction) and Fuling (Poria) were used to strengthen the Spleen and Stomach. The necrotic tissue sloughed off, and new flesh grew. Later, the wound ulcerated again, but after rest and regulation, the patient recovered after more than a year.
Another patient suffered from blood stasis attacking and accumulating in the scrotum, leading to ulceration and fistula formation with thin, clear pus. Previous treatments consisted solely of cold-natured herbs. The Liver pulse was short and choppy, while other pulses were floating and weak. This indicated that Liver Wood was constrained by Lung Metal, compounded by deficiency of Yuan Qi (Original Qi) failing to consolidate. A formula to tonify the Spleen and Stomach and generate Qi and Blood was prescribed, leading to slight recovery of Yuan Qi. However, the patient eventually passed away on a day when Metal energy was at its peak (Autumn).
A third patient presented with swelling, pain, fever, thirst, and sweating. I stated: "This is injury to Yin Blood." First, scarification was used to remove foul matter and unblock obstruction. Then, Siwu Tang (Four Substance Decoction), Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), Zhizi (Gardeniae Fructus), Danpi (Moutan Cortex), and Gusuibu (Drynariae Rhizoma) were used to clear Liver fire, resulting in recovery.
A fourth patient had an injury site where the tissue had been rubbed apart, leaving only persistent swelling and pain. I stated: "There is blood stasis inside; it should be urgently treated with scarification." The patient did not comply. I applied a paste made of Zhizi powder mixed with fresh juice of Raphanus sativus (Radish) externally, and applied Danggui Gao (Angelica Ointment) to the broken skin, along with internal administration of blood-activating agents, leading to recovery. Years later, the area would still itch and hurt during cloudy weather, revealing the error of not using scarification initially.
A fifth patient presented with black swelling of the buttocks and thighs, without rupture, but with severe distending pain and a heavy sensation. Others believed there was no internal blood stasis and suggested applying cooling herbs for pain relief. I palpated the Chi (Cun) pulse as choppy and knotted, indicating that due to obesity and thick fat, the blood stasis was deeply accumulated. Puncture would lead to immediate cure; otherwise, internal ulceration with risks of tendon rotting and bone damage would occur. I inserted the needle four cun deep, draining several sheng (a unit of volume) of black blood, which stopped the swelling and pain. On that day, the patient experienced fever, aversion to cold, irritability, thirst, and headache, signs of dual deficiency of Qi and Blood. Recovery was achieved with Shiquan Dabu Tang (Complete Great Tonifying Decoction).
A sixth patient presented with abdominal distension due to internal blood stasis, pain, fever, dry mouth, thirst, poor appetite, and limb fatigue. I stated: "This is excessive Liver fire constraining Spleen Earth, causing these symptoms." Fortunately, the patient was robust and young. Treatment with herbs to descend fire, clear the Liver, and activate blood led to recovery.
