The fontanelle bone refers to the area at the vertex of an infant's skull where the bones have not yet fused, which is soft and pulsating, known as the "fontanelle." If injury from falling or striking causes the cranial sutures to separate, but the brain marrow has not been shaken or damaged, and the tendons have not undergone torsion or displacement, the clinical presentation may include swelling and luster on the head and neck, a pale and deficient facial appearance, swollen eyes, a large nose, everted lips, a stiff tongue, drowsiness, and lethargy. In cases where the flesh is swollen but the skin is intact without bleeding, the patient should be supported into an upright sitting position. Immediately apply Dingtong San (Pain-Calming Powder) mixed with green onion juice to the injured area; then cover the medication with mulberry paper dipped in vinegar, and cauterize the paper using a heated iron. The treatment should continue until heat and pain are felt at the injury site and sounds are produced from the mouth. Remove the medicinal patch and apply Wanling Gao (Ten-Thousand-Spirit Plaster), changing it every three days. Once the pain subsides and appetite returns, remove the plaster and wash the area with He Shang Tang (Injury-Healing Decoction). This will dispel wind, dissipate swelling, invigorate blood, and regulate qi. In cases where the skin is broken and bleeding occurs, immediately use Mapi Bo ash (Dog-head Ash) to stop the bleeding first; subsequently, employ the Elm Bark Moxibustion and Cauterization Method, and administer Ren Shen Zi Jin Dan (Ginseng Purple-Gold Pill) internally to strengthen the spleen and stomach, boost primary qi, relieve thirst and generate fluids, enhance spirit, and strengthen the body, with the primary goal of harmonizing and unblocking the tendons and blood. Avoid triggering foods and spicy, roasted items. Wear a cloth cap with a drawstring to avoid wind and cold, and do not leave the room. If the skin is broken with continuous bleeding, the bone is depressed, and the tendons are everted, the brain marrow must be damaged. Symptoms such as flaccidity of the body, difficulty flexing the hands, tendon rigidity, and weak, silent breathing indicate a critical condition that is difficult to treat. If the wound is exposed to cold wind after breaking, it is incurable.
Mapi Bo is commonly known as Dog-head Ash; the variety produced outside the Great Wall is considered superior.
Dingtong San (Pain-Calming Powder): Treats all types of traumatic injuries and wind-pathogen damage; calms pain, reduces swelling, relaxes tendons, and harmonizes collaterals.
Ingredients: Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Ligusticum Chuanxiong Rhizoma), Bai Shao (Paeoniae Radix Alba), Guan Gui (Cinnamomi Ramulus) (each 1 qian); San Nai (Kaempferiae Galangal Rhizome) (3 qian); She Xiang (Moschus) (3 fen); Hong Hua (Carthami Flos) (5 qian); Zi Ding Xiang Gen (Symplocos Racemosa Root) (5 qian); Sheng Ma (Cimicifugae Rhizoma) (1 qian); Fang Feng (Saposhnikoviae Radix) (1 qian).
Preparation and Usage: Grind all ingredients into a fine powder. Mix with juice extracted from crushed old green onions and apply to the affected area, followed by the cauterization method.
Moxibustion and Cauterization Method: This method is specifically used for moxibustion and cauterization of wounds involving broken skin and bleeding. Since blood and bodily fluids cause moisture, bark is placed as a barrier before applying heat. First, place elm bark on the injured area. Then, crush old green onions and mix them evenly with Qi Ai Zhi Tong San (Qiai Pain-Stopping Powder). Place this mixture on the bark and perform moxibustion five times consecutively. Finally, wrap the area with soft silk. Wear a drawstring cloth cap and tie the straps tightly to strictly avoid wind and cold.
Wanling Gao (Ten-Thousand-Spirit Plaster): Treats traumatic injuries; resolves stasis, disperses toxicity, relaxes tendons, invigorates blood, stops pain, and sets bones with miraculous efficacy. It also treats numbness, wind-phlegm, and pain due to cold-dampness.
Ingredients: Guan Jin Cao (Euphorbia Humifusa Herb), Tou Gu Cao (Exocarpium Citri Reticulatae / or specific herb depending on context, often Lycii Cortex or similar penetrating herbs in formulas, here likely referring to a specific local herb), Zi Ding Xiang Gen (Symplocos Racemosa Root), Dang Gui (Angelicae Sinensis Radix, washed with wine), Zi Ran Tong (Pyritum, quenched in vinegar seven times), Gua Er Xue Jie (Dragon's Blood), Mo Yao (Myrrha), each 1 liang; Chuan Xiong (Ligusticum Chuanxiong Rhizoma) (8 qian); Chi Shao (Paeoniae Radix Rubra) (2 liang); Ban Liang Qian (a specific coin or herb preparation, quenched in vinegar, one piece); Hong Hua (Carthami Flos) (1 liang); Chuan Niu Xi (Cyathulae Radix), Wu Jia Pi (Acanthopanacis Cortex), Shi Chang Pu (Acori Tatarinowii Rhizoma), Mao Shan Cang Zhu (Atractylodis Rhizoma), Mu Xiang (Aucklandiae Radix), Qin Jiao (Gentianae Macrophyllae Radix), She Chuang Zi (Cnidii Fructus), Rou Gui (Cinnamomi Cortex), Chuan Fu Zi (Aconiti Lateralis Radix Praeparata), Ban Xia (Pinelliae Rhizoma Praeparatum), Shi Hu (Dendrobii Caulis), Bi Xie (Dioscoreae Hypoglaucae Rhizoma), Lu Rong (Cervi Cornu Pantotrichum), each 3 qian; Hu Jing Gu (Pantheris Os) (one pair); She Xiang (Moschus) (2 qian).
Preparation: Except for Xue Jie (Dragon's Blood), Mo Yao (Myrrha), and She Xiang (Moschus), which are ground into fine powders and packed separately, the remaining twenty-three ingredients are soaked in 10 jin of sesame oil over low heat for three days. Then, add the herbs to the oil and boil until black. Strain the dregs, add 5 jin of Huang Dan (Lead Monoxide), and boil again until the liquid forms beads when dropped into water. Remove from fire. When the medicine cools slightly, add the Xue Jie, Mo Yao, and She Xiang powders, stir evenly, remove, and allow the fire energy to dissipate.
Ren Shen Zi Jin Dan (Ginseng Purple-Gold Pill): This pill boosts primary qi, strengthens the spleen and stomach, relieves thirst and generates fluids, enhances spirit, and harmonizes and unblocks tendons and blood. It is most suitable for those with qi deficiency caused by falls, knocks, or sprains.
Ingredients: Ren Shen (Ginseng Radix et Rhizoma) (3 qian); Ding Xiang (Caryophylli Flos) (1 liang); Wu Jia Pi (Acanthopanacis Cortex) (2 liang); Gan Cao (Glycyrrhizae Radix et Rhizoma) (8 qian); Fu Ling (Poria) (2 qian); Dang Gui (Angelicae Sinensis Radix, washed with wine, 1 liang); Xue Jie (Dragon's Blood) (1 liang); Gu Sui Bu (Drynariae Rhizoma) (1 liang); Wu Wei Zi (Schisandrae Chinensis Fructus) (1 liang); Mo Yao (Myrrha, de-oiled, 2 liang).
Preparation and Usage: Grind all ingredients into a fine powder. Form pills with refined honey. Take 3 qian per dose, morning and evening, dissolved in light yellow wine or child's urine.
