骨乃足趾本节之骨也。其受伤之因不一,或从陨坠,或被重物击压,或被车马 砑,若仅伤筋肉,尚属易治;

The malleolus is the bone located below the tibia and above the dorsum pedis, presenting as a prominent high bone on both sides. The one on the medial side is named the medial malleolus, commonly known as Hegu; the one on the lateral side is the lateral malleolus, commonly known as Hegu (Note: In this context, "Hegu" refers to local colloquial names for medial/lateral malleoli, distinct from the acupoint LI4).
If injured by falling from horseback or stepping incorrectly while walking, the calcaneus may shift forward, the toes point backward, tendons are everted, muscles swell, and pain persists incessantly. Initially, manual manipulation should be used to adjust the tendons and realign the bones to restore their position. Then, bamboo splints should be applied to fixate the calcaneus and bound above the tibia. After three days, remove the binding for inspection. Support the posterior foot with a pillow, hold the tendons with hands, and use fingers to press and massage the sites of tendon knots, ensuring they are perfectly aligned. Internally administer Zhenggu Zijin Dan (Purple Gold Pill for Bone Setting), externally apply moxibustion and hot compresses combined with Dingtong San (Pain-Relieving Powder), wash with Haitongpi Decoction (Phellodendron Bark Decoction), and regularly take Jianbu Huqian Wan (Tiger-Hiding Pill for Strengthening Steps). If labor is undertaken prematurely after slight improvement, causing the distal end of the tibia to deviate inward, the medial malleolus will protrude and swell; if it deviates outward, the lateral malleolus will protrude and swell. Blood stasis will accumulate and coagulate, resulting in weakness during ambulation and tilting of the sole, making treatment difficult. Therefore, movement should only resume after qi and blood have fully recovered and circulation is unobstructed.

The dorsum pedis (Fu) refers to the top of the foot, also called Zudu, commonly known as the instep. Its bones are those at the proximal phalanges of the toes. Causes of injury vary: falling from height, being struck or crushed by heavy objects, or being run over by vehicles or horses. If only muscles and tendons are injured, it is relatively easy to treat; however, if the bone structure itself is damaged, it is often difficult to cure. Initially, gentle manual rubbing and kneading should be performed to align the bones and relax the tendons. Wash with Haitongpi Decoction (Phellodendron Bark Decoction) or Baxian Xiaoyao Decoction (Eight Immortals Free-and-Easy Decoction), apply Wanling Plaster (Ten Thousand Spirits Plaster), internally administer formulas that soothe tendons and relieve pain, as well as Jianbu Huqian Wan (Tiger-Hiding Pill for Strengthening Steps) and Bujin Wan (Tendon-Nourishing Pill).
