腥臭者,黄 建中汤去桂加当归水煎,吞苦楝丸;久不止,脐腹引阴冷痛者,东垣固真丸;
Li stated: In leukorrhea (leukorrhea) cases involving deficiency-cold with a fishy odor, the etiology is often continuous dribbling of urine (continuous dribbling usually indicates deficiency-cold, though deficiency-heat is also possible), or sudden severe metrorrhagia (sudden heavy vaginal bleeding), or excessive blood loss postpartum. This leads to Yin depletion and Yang exhaustion, failure of Ying Qi (nutritive qi) to ascend, stagnation and obstruction of the meridians, sinking of Wei Qi (defensive qi), and accumulation of essence and turbidity in the Lower Jiao (Lower Burner), resulting from entrapment and accumulation. If the discharge is white, slippery like mucus, and flows out with a fishy odor, treat with Huangqi Jianzhong Tang (Astragalus Decoction for Building the Middle) minus Cinnamon Twig, plus Angelica Sinensis, decocted in water, taken while swallowing Kulian Wan (Toosendan Fruit Pill). If it persists for a long time accompanied by cold pain radiating from the umbilicus and abdomen to the yin region, use Dongyuan Guzhen Wan (Dongyuan's Pill to Secure True Qi). For deficiency with internal heat, commonly use Bu Jing Gu Zhen Tang (Decoction to Tonify Channels and Secure True Qi) or Da Wuji Wan (Large Black Chicken Pill). For Qi deficiency, use Sijunzi Tang (Four Gentlemen Decoction); for Blood deficiency, use Siwu Tang (Four Substance Decoction). Add Phellodendron Bark if there is heat; add Cinnamon Twig and Aconite Root if there is cold. The cold pattern initially arises from losing blood and then losing Yang, leading to extreme Yang deficiency. Leukorrhea with a fishy odor is often accompanied by sadness and unhappiness (due to Yang deficiency); treat with Guifu Tang (Cinnamon and Aconite Decoction). For abdominal pain with coldness in the yin region, use Siwu Tang plus Cinnamon Twig and Aconite Root; commonly employ Jiuzhu Danggui Wan (Wine-Boiled Angelica Pill), Xiao Wuji Wan (Small Black Chicken Pill), or Hupo Tiaojing Wan (Amber Menstruation-Regularizing Pill).
Han stated (Feixia Medical Case): My wife was over thirty years old, having given birth eighteen times, with nine children dying young and eight surviving. Coinciding with the turmoil at Songpan involving my late father, my brothers and I fled westward. Overwhelmed by fear and anxiety, she fell into a coma, developed sores on her lips, tongue, and throat, sometimes blocking the airway, suffered from lower body collapse, and had profuse leukorrhea flowing like a stream. This state lasted for more than forty days. She occasionally regained consciousness but felt such despair she considered suicide, unable to bear her suffering. Some physicians administered cooling agents to clear upper heat, which worsened the lower pathology; others administered heating agents and fumigated the lower body with herbal decoctions, causing her to faint from heat. When my fourth brother returned and took her pulse, he identified this as a syndrome of Yang collapse, crying out, "The family line is not yet established; we almost killed my sister-in-law." He urgently prescribed Da Fuzi (Prepared Aconite Root) nine qian, salt-fried, as the sovereign herb (using hot herbs to counteract heat symptoms is a 'following treatment' method, requiring precise diagnosis; unfortunately, the pulse description is missing). It was moderated with Mint and Saposhnikovia Root, assisted by Dried Ginger, Cinnamon Twig, Ligusticum Chuanxiong, and Angelica Sinensis. The decoction was cooled to well-water temperature before administration. Before finishing the dose, she fell into a deep snoring sleep all night and woke up recognizing people. At that time, she had one son and two daughters; everyone hugged and wept bitterly, and relatives were sorrowful. Her close friend, Commissioner Zhao, exclaimed, "What magic do you possess?" I replied, "Medical texts state: 'Fake matches fake, true matches true.' The upper part presented false heat, so I used false-cooling herbs to follow its nature; the lower part presented true cold, so I used true-heating herbs to oppose it. Thus, the upper and lower harmonized, and the disease resolved. (For instance, a case of postpartum diarrhea with mouth and throat sores was cured using Lizhong Wan coated with Zixue [Purple Snow], illustrating the principle of true-treating-true and fake-treating-fake.)" Subsequently, Nv Jindan (Women's Golden Pill) was used as the main formula, combined intricately with two or three other formulas. This not only eliminated the disease but also regulated Yuan Qi (Primordial Qi). In the Gengwu year, she bore a son who now inherits the title; in the Renshen year, another son was born. Last year, he suffered from malaria for thirteen months; treatment focused on nourishing Yuan Qi and regulating Sheng Qi (Vital Qi derived from food/grain) (Yuan Qi is congenital true Qi; Sheng Qi is acquired grain Qi). Only after dietary intake significantly improved were strong purgative drugs used to expel large amounts of clots. Finally, Fuzi Tang (Aconite Decoction) three qian cured him.
One should not expect immediate results within morning or evening. Using Nv Jindan, which is identical to Shengjin Wan (Victory Gold Pill) (Nv Jindan can induce pregnancy in postmenopausal women, an ancient formula recorded and praised in poetry), obtained from an unusual source. It contains doubled Cyperus Rotundus, and according to Qi and Blood imbalances, adds Turmeric, Scutellaria Baicalensis strips, and doubled Ligusticum Chuanxiong, achieving many successes. I reflect that childlessness is a common issue. Recalling the difficulty and bitterness of my own path to fatherhood, I have sequentially recorded these formulas and Nv Jindan to help others. (Nv Jindan is Shengjin Wan; see the chapter on Seeking Offspring for the formula.)
