The Shengbing Zi (Holy Cake) from *Pi Wei Lun* (Treatise on the Spleen and Stomach) treats dysentery with red and white discharge, cramping pain in the umbilical and abdominal regions that persists for a long time without healing.
Ingredients: Huang Dan (Lead Oxide, 2 qian), Ding Fen (Calomel, 3 qian), Mi Tuo Seng (Litharge, 2 qian), Bo Shang Liu Huang (Imported Sulfur, 3 qian), Qing Fen (Calomel, a small amount).
Preparation and Administration: Grind the above ingredients into a fine powder, add 4 qian of wheat flour, mix with a few drops of water, roll into cakes the size of a fingertip, and dry in the shade. Before meals, grind one cake in Jiang Shui (fermented rice water) to dissolve and take orally. It is ideal if the stool turns black.
Yuan Rong’s Sheng Gong Wan (Holy Merit Pill) specializes in treating bloody dysentery.
Ingredients: Ni Fen (Calomel, 3 qian bi), Ding Fen (Calomel, 2 qian bi); another method adds Ge Fen (Clam Shell Powder).
Preparation and Administration: Grind evenly, moisten with water-steamed steamed dough pills to the size of mung beans, and administer with Ai Tang (Mugwort Decoction).
Zhang Zhongjing’s Taohua Tang (Peach Kernel Decoction) treats lower dysentery with pus and blood in the stool.
Ingredients: Chishi Zhi (Red Halloysite, 1 jin 1.5 liang, half crushed, half powdered), Gan Jiang (Dried Ginger, 1 liang), Jing Mi (Rice, 1 sheng).
Preparation and Administration: Take these three ingredients, boil in 7 sheng of water until the rice is cooked, strain the dregs, take 7 he of the warm liquid, add 1 fang cun bi of Chishi Zhi powder, and take three times a day. If cured after one dose, do not take more.
Note: This formula originates from the examples of Yangming channel medications in *Shang Han Lun*. However, red dysentery relates to Bing Fire (Heat), while white dysentery relates to Geng Metal (Cold/Dryness)... (Note: The original text's logic here is obscure, implying different pathogenesis for red and white dysentery, but this formula is primarily used for deficiency-cold slipping-depletion patterns.)
Yan Shi’s Yu Yu Liang Wan (Yu Yu Liang Stone Pill) treats slippery diarrhea due to deficiency-cold of the intestines and stomach.
Ingredients: Yu Yu Liang Shi (Hematite, dosage missing), Chishi Zhi (Red Halloysite, dosage missing), Long Gu (Dragon Bone), Bi Bo (Long Pepper), Gan Jiang (Dried Ginger, processed), He Zi (Chebula Fruit, baked in flour), Rou Dou Kou (Nutmeg, baked in flour), Fu Zi (Aconite Root, processed, skin and navel removed, equal parts).
Preparation and Administration: Grind into powder, make pills with vinegar paste the size of tung tree seeds. Take 70 pills per dose, empty stomach, with Mi Yin (rice drink).
Ju Fang’s Zhen Ren Yang Zang Tang (True Person Nourishing Viscera Decoction) treats dysentery with red and white discharge, tenesmus, abdominal pain, and rectal prolapse due to disharmony of cold and heat.
Ingredients: Ying Su Ke (Poppy Shell, remove stem and cap, honey-fried, 3 liang 6 qian), Ren Shen (Ginseng), Dang Gui (Angelica Sinensis, each 6 qian), Rou Gui (Cinnamon Bark, 8 qian), He Zi Pi (Chebula Peel, 1 liang 2 qian), Mu Xiang (Costus Root, 2 liang 4 qian), Rou Dou Kou (Nutmeg, baked in flour, half liang), Bai Shao (White Peony Root, 1 liang 6 qian), Sheng Gan Cao (Raw Licorice Root, 1 liang 8 qian), Bai Zhu (Atractylodes Macrocephala, 6 qian).
Preparation and Administration: Cut the herbs, take 4 qian per dose, decoct in water. If visceral cold is prominent, add Fu Zi (Aconite Root).
Shui Zhu Mu Xiang Wan (Water-Boiled Costus Pill) treats all types of dysentery with red and white discharge, mixed pus and blood, and tenesmus.
Ingredients: Ying Su Ke (Poppy Shell, remove pulp, 1 liang 4 qian), Qing Pi (Green Tangerine Peel, remove white pith, 2 liang 8 qian), Gan Cao (Licorice Root, 2 liang 4 qian), Dang Gui (Angelica Sinensis, washed and de-rhizomed, 6 liang), He Zi (Chebula Fruit, processed and pitted, 8 liang), Mu Xiang (Costus Root, not exposed to direct fire, 6 liang).
Preparation and Administration: Grind into powder, make honey pills the size of bullet seeds. Take one pill per dose, dissolve in 8 fen of water, and take warm.
Mi Zang’s He Zi San (Chebula Powder) from *Mi Zang* treats diarrhea due to deficiency-cold of the intestines and stomach, undigested food in stool, borborygmus, abdominal pain, rectal prolapse, pus and blood in stool, occurring day and night without limit.
Ingredients: Yu Mi Qiao (Poppy Shell, remove stem and cap, honey-fried, half liang), He Zi (Chebula Fruit, baked and pitted, 7 fen), Gan Jiang (Dried Ginger, processed, 6 fen), Ju Pi (Tangerine Peel, half qian).
Preparation and Administration: Grind into fine powder, divide into two doses, decoct in water, take hot with the dregs, empty stomach.
Note: *Shi Ji* (Ten Categories) states: "Astringents can treat slipping depletion." Using the sour and slightly astringent nature of Yu Mi Qiao (Poppy Shell) to ascend and consolidate Qi to stop slipping is the Sovereign herb; using the slightly sour nature of He Zi Pi (Chebula Peel) to ascend and consolidate Blood to treat physical slipping is part of the treatment; Ju Pi (Tangerine Peel) is slightly bitter and warm, benefiting true Qi and ascending Yang, serving as the Envoy; Gan Jiang (Dried Ginger) is greatly pungent and hot, dispelling cold, serving as the Minister, also addressing the main pathology.
Note: All the above formulas use warming and hot herbs to consolidate Qi, stop bleeding, and tightly astringe the intestines. They are designed for cases of intestinal deficiency with cold invasion leading to slipping. Moreover, dysentery (Zhi Xia) is mostly a Heat pattern accompanied by stagnation. The Ju Fang category specializing in astringing and heating seems difficult to apply universally.
Xuan Ming’s Xiang Lian Wan (Fragrant Coptis Pill) from *Xuan Ming*
Ingredients: Mu Xiang (Costus Root), He Zi Rou (Chebula Flesh, baked in flour), Huang Lian (Coptis Rhizome, fried, each half liang), Long Gu (Dragon Bone, 1 qian).
Preparation and Administration: Grind into fine powder, make pills with water the size of millet seeds. Take 20 pills per dose, with Mi Yin Tang (Rice Drink Decoction).
Ji Yao’s He Zi San (Chebula Powder) from *Ji Yao* treats dysentery with abdominal pain, where diarrhea gradually decreases, making it suitable for this stopping method. It says: "When the major trend has passed, it is appropriate to stop it."
Ingredients: He Zi Pi (Chebula Peel, 1 liang 5 qian, half raw half cooked), Mu Xiang (Costus Root, half liang), Huang Lian (Coptis Rhizome, 3 qian), Gan Cao (Licorice Root, honey-fried, 3 qian).
Preparation and Administration: Grind into fine powder, take 2 qian per dose, with Bai Zhu Shao Yao Tang (Atractylodes and Peony Decoction). If stopping does not work, one should "Gui Er Song Zhi" (tonify and guide out), adding Hou Po (Magnolia Bark, 1 liang) to exhaust the pathogenic qi.
Pi Wei Lun’s He Li Le Wan (Chebulic Myrobalan Pill) from *Pi Wei Lun* treats intermittent dysentery occurring day and night without limit, with a foul odor that cannot be approached, and cramping pain in the umbilical and abdominal regions.
Ingredients: Chun Gen Bai Pi (Toona Root Bark), He Zi Rou (Chebula Flesh, half liang), Mu Ding Xiang (Cloves, 30 pieces).
Preparation and Administration: Grind into fine powder, make pills with vinegar paste the size of tung tree seeds. Take 50 pills per dose, with warm rice drink.
Note: The above formulas use warming and cooling herbs. Compared to the Ju Fang’s exclusive use of astringing and heating, this is a modified method. For slipping, using astringents is logical; for slipping with cold, warming and astringing or heating and astringing is appropriate; for slipping with heat, cooling and astringing is appropriate. Cold and heat should be used according to suitability, and it seems inappropriate to adhere rigidly to one theory.
Xuan Ming’s San Sheng San (Three Saints Powder) from *Xuan Ming* treats postpartum bloody dysentery that does not stop.
Ingredients: Wu Yu Gu (Cuttlefish Bone), Shao Mian Hui (Burned Cotton Ash), Xue Yu Hui (Scalp Hair Ash, equal parts).
Preparation and Administration: Grind into fine powder, take 1 qian per dose, mix with Shi Liu Pi Tang (Pomegranate Peel Decoction), take hot.
Note: This formula specifically focuses on stopping bleeding.
