毒微气血实则顺，毒甚而虚逆自明，毒微若虚恐化险，毒甚逢实变险能。
【注】痘由禀赋毒瓦斯，轻者固顺，然必儿之气血不虚，则始为顺也。重者固逆，亦必
儿之气血虚，则始为逆也。毒微者顺也，若儿之气血虚弱，虽顺恐化险也。毒甚者逆也，
若儿之气血不虚，虽逆能变险也。此以儿之胎毒重轻，气血虚实，定痘之顺逆险也。
毒微天时和者吉，毒甚天时不和凶，毒微不和顺中险，毒甚时和逆或更。
【注】毒微吉矣，然必逢大时之和，始为顺而吉也。毒甚凶矣，然必遇大时不和，则
始为逆而凶也。毒微者顺也，若值天时不和，恐顺中化险也。毒甚者逆也，若值天时之和，
或逆中转险也。此以胎毒之轻重，天时之和不和，定痘之顺逆险也。
险逆时和无病吉，不和有病定然凶，由此故识种痘善，人事能回天命亨。
【注】凡痘险逆之证，固不善矣。若逢天时之和，儿又无病，险者可以变顺，逆者可
以化险，故曰吉也。若逢天时不和，儿又有病，则险者变逆，逆者更逆，故曰凶也。由
此观之，可识种痘之善，以得其天时之和，儿素无病，其中即有毒之重者，而人事克尽，亦
能挽回天命而致亨也。
发热和缓微微汗，饮食如常二便调，睡卧安然神气爽，此为顺证不须疗。
【注】发热和缓，身不甚热，毒轻也。微微汗，身微微有汗，表和也。饮食二便如常，
里和也。神气清爽，睡卧安然，精神气血大和，故为顺证，不须治疗也。
痘毒轻微而气血充实的属于顺证，痘毒严重而气血虚弱的属于逆证，这是显而易见的。痘毒轻微但气血虚弱，恐怕会转化为险证；痘毒严重却逢气血充实，也可能转变为险证。
【注】痘疮是由禀赋中的胎毒所致，轻者固然为顺证，但必须小儿的气血不虚，才算是真正的顺证。重者固然为逆证，但也必须小儿的气血虚弱，才算是真正的逆证。痘毒轻微本属顺证，如果小儿气血虚弱，即使是顺证也恐怕会转化为险证。痘毒严重本属逆证，如果小儿气血不虚，即使是逆证也有可能转化为险证。这是根据小儿胎毒的轻重、气血的虚实，来判定痘疮的顺、逆、险三种情况。
痘毒轻微且天时调和的为吉，痘毒严重且天时不和的为凶，痘毒轻微但不逢天时调和则顺中见险，痘毒严重但逢天时调和则逆中或转险。
【注】痘毒轻微本是吉兆，然而必须遇到天时调和，才算作顺而吉。痘毒严重本是凶兆，然而必须遇到天时不和，才算作逆而凶。痘毒轻微本属顺证，如果遇到天时不和，恐怕在顺证中转化为险证。痘毒严重本属逆证，如果遇到天时调和，可能在逆证中转化为险证。这是根据胎毒的轻重、天时的调和与否，来判定痘疮的顺、逆、险三种情况。
痘疮处于险逆阶段，若逢天时调和且无其他疾病则为吉，若逢天时不和且有其他疾病则必然为凶，由此可以认识到种痘之善法在于顺应天时，通过人事努力可以挽回天命使病情亨通。
【注】凡是痘疮出现险逆的证候，本来就不好。如果遇到天时调和，小儿又没有其他疾病，险证可以变为顺证，逆证可以化为险证（此处指由极逆转为次险或可治之险），所以说它是吉。如果遇到天时不和，小儿又有其他疾病，那么险证会变为逆证，逆证会更加严重，所以说它是凶。由此看来，可以认识到种痘的善法在于获得天时的调和，小儿平素没有疾病，即使其中胎毒较重，只要人事尽力做到极致，也能挽回天命而致亨通。
发热和缓，微微出汗，饮食如常，大小便调畅，睡眠安稳，精神清爽，这属于顺证，不需要治疗。
【注】发热和缓，身体不特别热，说明痘毒轻。微微出汗，身体有少量汗出，说明表气调和。饮食和二便正常，说明里气调和。神气清爽，睡眠安稳，说明精神气血大调和，所以称为顺证，不需要治疗。
When the pathogenic toxin is mild and Qi and Blood are substantial, it is a favorable condition (Shun); when the toxin is severe and Qi and Blood are deficient, it is an unfavorable condition (Ni), which is self-evident. If the toxin is mild but Qi and Blood are deficient, it may transform into a dangerous condition; if the toxin is severe but Qi and Blood are substantial, it may also transform into a dangerous condition.
[Note] Smallpox arises from congenital fetal toxin. Mild cases are naturally favorable, but they must be accompanied by sufficient Qi and Blood in the child to truly constitute a favorable condition. Severe cases are naturally unfavorable, but they must be accompanied by deficient Qi and Blood in the child to truly constitute an unfavorable condition. A mild toxin indicates a favorable condition; however, if the child's Qi and Blood are weak, even a favorable condition may transform into a dangerous one. A severe toxin indicates an unfavorable condition; however, if the child's Qi and Blood are not deficient, even an unfavorable condition may transform into a dangerous one. This determines the favorable (Shun), unfavorable (Ni), and dangerous (Xian) states of smallpox based on the severity of the fetal toxin and the sufficiency or deficiency of Qi and Blood.
When the toxin is mild and the heavenly timing (environmental climate) is harmonious, it is auspicious; when the toxin is severe and the heavenly timing is disharmonious, it is ominous. When the toxin is mild but the heavenly timing is not harmonious, danger may arise within the favorable condition; when the toxin is severe but the heavenly timing is harmonious, the unfavorable condition may potentially turn towards danger.
[Note] A mild toxin is auspicious, yet it requires harmonious heavenly timing to truly be considered favorable and auspicious. A severe toxin is ominous, yet it requires disharmonious heavenly timing to truly be considered unfavorable and ominous. A mild toxin indicates a favorable condition; however, if the heavenly timing is disharmonious, it may transform into a dangerous state within the favorable condition. A severe toxin indicates an unfavorable condition; however, if the heavenly timing is harmonious, it may transform into a dangerous state within the unfavorable condition. This determines the favorable, unfavorable, and dangerous states of smallpox based on the severity of the fetal toxin and the harmony or disharmony of the heavenly timing.
In dangerous or unfavorable conditions, if the heavenly timing is harmonious and there are no other diseases, it is auspicious; if the heavenly timing is disharmonious and there are other diseases, it is certainly ominous. From this, we can understand the excellence of vaccination [or inoculation], where human effort can restore the favorable course of destiny.
[Note] Any presentation of dangerous or unfavorable smallpox conditions is inherently poor. However, if the heavenly timing is harmonious and the child has no other illnesses, dangerous conditions can transform into favorable ones, and unfavorable conditions can transform into manageable dangerous ones; thus, it is termed auspicious. If the heavenly timing is disharmonious and the child suffers from other illnesses, dangerous conditions will worsen into unfavorable ones, and unfavorable conditions will become even more severe; thus, it is termed ominous. Observing this, one can recognize the virtue of proper inoculation practices: by aligning with harmonious heavenly timing and ensuring the child is free from prior illness, even if the internal toxin is heavy, complete human effort can reverse the adverse fate and achieve prosperity.
Fever is mild and gentle with slight sweating, diet remains normal, and urination and defecation are regulated. Sleep is peaceful and spirit/Qi is refreshing; this constitutes a favorable condition requiring no treatment.
[Note] Mild fever without excessive heat indicates a light toxin. Slight sweating indicates the harmonization of the exterior. Normal diet and bowel/bladder functions indicate the harmonization of the interior. Refreshing spirit and peaceful sleep indicate the great harmonization of spirit, Qi, and Blood. Therefore, it is called a favorable condition requiring no treatment.
