Vertigo may be caused by blood stasis (Blood Vertigo), Qi stagnation (Qi Vertigo), phlegm (Phlegm Vertigo), fire (Fire Vertigo), dampness (Dampness Vertigo), or summerheat (Summerheat Vertigo). It can also arise from blood deficiency, Qi deficiency, invasion of wind pathogens, invasion of cold pathogens, or overexertion.
Now, adding "Sha Vertigo" (vertigo caused by Sha toxin) to this list: ancient physicians had no treatment for it, and ancient texts did not discuss it. How do I know? Generally, Blood Vertigo presents with a Kou pulse (Kou Mai, hollow like a scallion stem); Qi Vertigo presents with a Chen pulse (Chen Mai, deep); Phlegm Vertigo presents with a Hua pulse (Hua Mai, slippery); Fire Vertigo presents with a Shu pulse (Shu Mai, rapid); Dampness Vertigo presents with a Ru pulse (Ru Mai, floating and fine); Summerheat Vertigo presents with a Xu pulse (Xu Mai, empty); Vertigo due to blood deficiency presents with a Se pulse (Se Mai, choppy); Vertigo due to Qi deficiency presents with a Wei pulse (Wei Mai, minute); Vertigo due to wind invasion presents with a Fu-Huan pulse (Fu-Huan Mai, floating and moderate); Vertigo due to cold invasion presents with a Xian-Jin pulse (Xian-Jin Mai, string-taut and tight); Vertigo due to overexertion presents with a Fu-Hong pulse at the Right Chi position (Right Cun-Guan-Chi pulse positions correspond to Lung/Heart/Spleen/Stomach/Liver/Kidney; Chi refers to the distal position on the wrist). This is a general overview of vertigo pulses. However, if the symptoms resemble Blood Vertigo but the pulse is instead Short (Duan); resembles Qi Vertigo but the pulse is Floating (Fu); resembles Phlegm Vertigo but the pulse is Choppy (Se); resembles Fire Vertigo but the pulse is Slow (Chi); resembles Dampness Vertigo but the pulse is Tense/Firm (Jin); resembles Summerheat Vertigo but the pulse is Excess (Shi); resembles blood-deficiency vertigo but the pulse is Slippery (Hua); resembles Qi-deficiency vertigo but the pulse is Large (Da); resembles wind-invasion vertigo but the pulse is Deep-Tight (Chen-Jin); resembles cold-invasion vertigo but the pulse is Minute-Slow (Wei-Huan); resembles exertion vertigo but the pulse is Fine-Excess (Xi-Shi)—these are all cases where the pulse and symptoms do not match. Having verified this through experience, I am confident these are Sha Vertigo cases. Thus, the treatment for Sha Vertigo is reliable.
Case Records:
1. Mr. Chen Suyuan suffered from recurrent vertigo, experiencing dizziness day and night upon waking. Upon examination, his Right pulse was Deficient while the Left pulse was Excess, with irregular speed. I stated, "This requires Sha release therapy." I performed cupping/bleeding with over ten needles above and below the popliteal fossa, releasing dark purple-black blood profusely. Over twenty needles were used on the fingers. He took Baohua San (Precious Flower Powder) and Chenxiang Awai Wan (Agarwood and Asafoetida Pill), administered with clear tea slightly cooled and chilled. A prescription of Fangfeng Shengjin Tang (Saposhnikovia Decoction to Win Gold) added with Taoren (Peach Kernel) and Honghua (Safflower) was given, consumed slightly cool. Thereafter, he never suffered from vertigo again.
2. The wife of Jiang Weibin suddenly developed vertigo in the first lunar month, occurring three times a day, causing family panic. Her pulse was Deep and slightly Tight. I instructed her maid to perform Sha release on the fingers with over thirty needles. She took Jiuku Dan (Salvation Pill) mixed with Agarwood powder, administered with clear tea slightly cooled and chilled. A prescription of Jingjie Yinhua Tang (Schizonepeta and Honeysuckle Decoction) added with Boluozi (Radish Seed) and Zhike (Aurantium Fructus) cured her in one dose.
3. The son of Mei Junyu vomited roundworms in the third lunar month and developed vertigo, falling into a coma. All six pulse positions were Hidden (Fu). The Sha veins at the Tiger's Mouth (He Gu area) were blue-green. Eighteen needles were used to release Sha from the fingertips. He took Xixin Dahuang Wan (Asarum and Rhubarb Pill), mixed with clear tea and brown sugar, administered slightly cooled and chilled. He gradually regained consciousness. A prescription of Puhuang Yin (Polygonum Multiflorum Drink) with Jianghuang (Turmeric) removed, added with Chenpi (Tangerine Peel), Wuyao (Lindera Root), and Honghua (Safflower), was taken with brown sugar in a slightly cool decoction, leading to recovery.
4. The wife of Huo Tingxian presented with high fever, red face, thirst, and dry lips in the fourth lunar month. Treatment with herbs for Yangming Stomach Channel symptoms induced vertigo, leaving her unconscious all day. Her pulse was String-Taut and Fine at both Cun positions, Deep and Minute at both Guan positions, and Large on the Left Chi and Tight on the Right Chi. I said, "The pulse and symptoms do not match; this is Sha toxin attacking internally." First, Sumu San (Sappan Wood Powder) and Sharen Tang (Amomum Decoction) were given, slightly cooled and chilled. An elderly woman released Sha from the fingers with over twenty needles; the blood was ink-black, yet she remained unconscious. Then, two needles were used to release Sha near the breast, after which she revived. Other symptoms persisted. Jili San (Tribulus Powder) was given in a slightly warm decoction. Tao Ren Hong Hua Tang (Peach Kernel and Safflower Decoction) added with Zhike was prescribed for four doses. After expelling all foul black toxic matter, she recovered.
5. Sheng Siyu drank alcohol in the sixth lunar month, sweating on the head and developing vertigo. His Cun and Guan pulses were excessively Surging and Large, while both Chi pulses were Hidden. Observing the Sha veins, over ten needles were used, releasing dark purple-black toxic blood. A prescription of Chenpi Nongbo Tang (Tangerine Peel and Magnolia Bark Decoction) added with Gange (Kudzu Root), Shanzha (Hawthorn Fruit), and Boluozi (Radish Seed) was given, slightly cooled and chilled, restoring consciousness. Subsequently, Huoxiang Tang (Patchouli Decoction) added with Huanglian (Coptis Rhizome) and Huangqin (Scutellaria Root) stabilized him.
